Subject: Adopted: ESO Reports 07/01/2019 Effective Date: 07/01/2019 Last Revised Date: 7/10/2023 Revision Number: 4 Due for Revision Date: 10/01/2027 # 1015.07.00 Table of Contents 1015.07.00 ESO 1015.07.01 Purpose 1015.07.02 Scope 1015.07.03 EHR 1015.07.04 Fire 1015.07.01 Purpose The purpose of this document is to ensure that an ESO report is completed for all emergency/non-emergency calls. To provide the District with accurate records, all calls shall have a Fire/NFIRS report and an EHR (Electronic Health Record) when applicable. 1015.07.02 Scope The SOP/SOG covers ESO reports generated by suppression personnel for emergency and non-emergency incidents. All ESO entries will be reviewed by the Battalion Chiefs. 1015.07.03 EHR An EHR MUST be completed for every medical call BSFD is dispatched to. See Attached Appendix A All ALS reports are to be completed by the paramedic in charge of the incident. The narrative section is intended to “paint the picture of the scene” and to document anything that could not be documented anywhere else. BLS reports may be completed by an EMT under the direction of the Company Officer / paramedic in charge. An EMT cannot be lead on any call. The Company officers shall review his/her reports. Time, incident numbers and all statistical information shall be checked for accuracy. FTO’s are to review all high priority EHR reports (to include NIFRS) for quality of medical documentation. FTOs shall advise the FIRE & EMS Deputy Chiefs and Training Captains of any ongoing deficiencies. Once the EHR has been locked, no changes are to be made to the original document. An addendum may be added to the narrative section if something needs to be added. If time sensitive documentation or incorrect documentation was found, the EHR can be unlocked by contacting the Battalion Chief, Deputy Chief, or Training Captains. A Patient Refusal Form shall be filled out and attached to the report on all patients who refuse transport. Citizen assist, Assist the Invalid and Lifting Help calls are medical calls, therefore a patient assessment must be done and a refusal filled out if applicable. When encountering a patient that needs assistance not included in emergency services go to forms and fill out the Mobile Integrated Healthcare referral form. 1015.07.04 Fire Fire reports shall be completed by the initial responding Company Officer. “Working Fire” reports are to be completed by the initial responding Company Officer. The Company officers shall review his/her reports. Time, incident numbers and all statistical information shall be checked for accuracy and completeness. The Battalion Chief is to ensure all reports are completed for each shift. A NFIRS narrative shall be completed on all calls. (Do not simply state see patient narrative.) 1015.07.05 False Alarms Alarm nuisance reports shall be filled out on all listed applicable false alarms and the white copy left with the owner or property manager and documented in the NFIRS narrative. (Form attached: Appendix B) o o o o o Construction Unable to Determine Weather Related Mechanical Failure Service Required 1015.07.06 Completed Reports and Quality Management (QM) B/C's: As the off-going shift, your task is to review the completed reports and ensure all EHR and NIFRS Incident reports are locked in the system. It is crucial to double-check the reports for any errors or missing information before locking them to maintain data integrity. Lt's: Ensure all EHR and NIFRS Incident reports are completed and accurate before being locked. This includes verifying the accuracy and completeness of information, proper categorization, and inclusion of all relevant details. o Before submitting the reports, it is important to lock all calls to prevent any unauthorized modifications. By locking the calls, we maintain the integrity of the data and ensure that no changes are made to the reports once they are finalized. o As the primary point of contact, Lt’s are responsible for sending emails to the FTO email group for all Alerts, P-1 calls, unusual incidents, or arrests. Make sure to include the type of call and run number in these emails. FTO's: While reviewing ESO reports, pay close attention to any Alerts, P-1 (priority one) calls, Unusual Incidents, or Arrests where an email has not been sent to the FTO email group. If any such cases are identified, make sure to promptly communicate them to the relevant individuals or departments. Furthermore, ensure that the FTO email group receives notification of the Alerts, P-1 calls, Unusual Incidents, and Arrests for documentation purposes and Quality Management (QM). o Upon receiving email notifications for Alerts, P-1 calls, Unusual Incidents, or Arrests, carefully review all categories, content, and take appropriate actions as necessary via Quality Management (QM) in ESO. Additionally, please ensure that all required forms are attached and properly filled out for accurate documentation. o NIFRS – Fire Incidents Reports: Fire Incidents, Motor Vehicle Collisions (MVCs), and Special Operations incidents shall be reviewed to verify the accuracy of the response times, accuracy and completeness of information, proper categorization, and inclusion of all relevant details.
