This document is the Controlled Substances Inventory, Storage, and Security Procedures as approved by the Medical Director. 1055.15.01 CONTROLLED SUBSTANCES STORAGE The main storage location for all controlled substances will be at the department’s main station, at which the Medical Director is registered with the U.S. Department of Justice and D.E.A. The controlled substances are stored in a metal keyed lock box located in a lockable file cabinet within the administrative office. The only individuals having keys to the box will be the Deputy Chief of EMS or his or her assignee, at the discretion of the Medical Director. All purchased controlled substances will be logged in and placed inside the metal lock box. Any time substances are removed, the date, substance, amount, expiration date, unit number to which the substance was assigned and the amount left shall be documented. The individual removing the substance and a witness will print and sign the Controlled Substance Form. 1055.15.02 CONTROLLED SUBSTANCE PLACED ON UNIT All narcotics will be stored in a secured vault with dual authentication on the unit. Any errors made in the log or with the controlled substance policy shall be reported immediately to the shift supervisor, the Deputy Chief EMS, and the Medical Director if necessary. Each time controlled substances are placed on a licensed ALS unit, the following is to be documented on the Units Daily Controlled Substance Form: A. B. C. D. E. F. Date Name and the amount of substance added Expiration date of substance added Signature/printed name of the paramedic adding the substance Signature/printed name of witness of individual adding substance Total amount of substance on unit 1055.15.03 DAILY CHECKING OF CONTROLLED SUBSTANCE The following is to be documented by the on-coming paramedic and the off-going paramedic on the Daily Controlled Substance Form: (Arriving check) A. B. C. D. E. F. Date Time Printed name of off-going and on-coming paramedics Signature of off-going and on-coming paramedics Total amount of each controlled substance on-board the unit The expiration date of each controlled substance on-board unit 1055.15.04 USAGE OF CONTROLLED SUBSTANCES Each time a controlled substance is used, the following is to be documented on the Daily Controlled Substance Form: A. B. C. D. E. F. G. H. I. Incident Number Date Time Given Name of substance given Amount of substance given Amount of substance wasted Signature and printed name of the witness seeing the substance being wasted Signature and printed name of the paramedic administering and/or wasting the substance The amount of substance now on the unit 1055.15.05 EXPIRED CONTROLLED SUBSTANCES Upon finding expired controlled substances, the said substance will be documented on the Daily Controlled Substance Form. The Deputy Chief EMS will remove the expired substance and waste it. The following will be documented on the Daily Controlled Substance Form: A. B. C. D. E. F. G. H. I. Date Time Name of expired substance Amount of controlled substance expired Expiration date of expired substance Amount of expired substance wasted Printed name of wasting paramedic and witness Signature of wasting paramedic and witness Amount and expiration of substance replaced 1055.15.06 PATIENT/PARAMEDIC/DRUG USAGE: All paramedics with the Bonita Springs Fire Control and Rescue District that function with full privileges are authorized to use any controlled and non-controlled drugs carried by the system. The administration of these drugs will be as directed by the Medical Director following the Lee County Common Treatment Guidelines Manual (Standing Orders and General guidelines), or as directed by a patient’s private physician, or an emergency room physician at a medical facility. Any and all problems found will be investigated by the Deputy Chief of EMS and a report of such made.
