HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: EQUIPMENT GUIDELINES – 014 January 1, 2022 NIMS COMPLIANT SOG Title: Missing or Damaged Equipment Revision: 3 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2026 Fire Chief 2 PURPOSE All personnel have a responsibility to ensure that all department equipment is accounted for and in proper working condition. In addition to completing morning apparatus and equipment inventories, each of us is responsible to return all equipment used during work assignments to its proper location. Any damaged equipment will be removed from service and a work order will be written. In an effort to improve accountability when equipment is misplaced or damaged, the following guideline is created. POLICY Upon discovering that department equipment is missing or damaged, notifications will be made, utilizing the procedure below. PROCEDURE Missing Equipment Whenever department personnel discover equipment missing, the employee must verbally notify their Company Officer. If the equipment is not located by the end of the shift, the Company Officer will send an e-mail to the Shift Commander outlining the circumstances regarding the missing equipment, what attempts were made to locate the equipment and any other pertinent information regarding the incident. The Company Officer must also copy the e-mail to the Division Head responsible for the missing equipment. Additionally, a work order and an entry on the station pass- on sheet are also required. On the next shift day, the Shift Commander is expected to forward a memo and all supporting documentation to the Deputy Fire Chief-Administration. 1 Damaged Equipment Whenever department personnel discover damaged equipment, the employee must verbally notify their Company Officer. The equipment must be removed from service, a work order written and an entry on the station pass-on sheet is also required.
