HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: ADMINISTRATIVE – 003 January 1, 2022 NIMS COMPLIANT SOG Title: Departmental Reports Revision: 5 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 4 PURPOSE Establish a form to report and record all important activities which occur on a shift and at each station on a daily basis. A. Shift Commander Planner B. Company Station Notes C. Emergency Incident Reports D. Daily Pass On Sheet E. Record of Accident/Personal Injury F. Village of Hoffman Estates Time Report (HETR) G. Training Reports H. Review of Reports I. Sick Time Call in Logs POLICY PROCEDURE A. Shift Commander Planner - The Shift Commander is to complete a daily InTime schedule for attendance/payroll purposes to include the following: 1. All officers and firefighters on duty and the stations to which they are assigned. 2. The reasons for the absence of a firefighter or officer from duty. 3. Personnel working out of classification. 4. Personnel receiving a premium pay status. B. Company Station Notes - The Company Officer shall utilize Target Solutions to list 1 all company activities involving: Training Station maintenance Roll Call Apparatus Radio and Lock Box Inventory Status Equipment failure Apparatus Checks Ambulance 22 Status SCBA Daily Check Physical Fitness Acting Officer Status Company Officer Daily Paperwork C. Emergency Incident Reports - Emergency incident reports shall be completed in accordance with Administrative SOG A24. Each time an engine, tower, squad, ambulance, Chief, or Battalion Chief responds to an emergency medical service call and patient contact occurs, it will be documented on the currently used electronic patient care report. All EMS reports will be completed according to the NWCH System and Department standards and reporting systems. D. Daily Pass-On Sheet - The Company Officer and Battalion Chief on duty shall complete a Station Pass-on Sheet daily. The Daily Pass-on Sheet is divided into two sections and should contain the following information: 1. Section 1 - Out-of-service status of the following: Apparatus Apparatus equipment Streets Hydrants Station equipment Other information pertaining to the daily operations of the Department NOTE: These items will be carried on the sheet until repaired or replaced or until removed for appropriate reason as determined by the Division Head. 2. Section 2 - Miscellaneous. All apparatus and equipment that has been put back in-service and any other information pertaining to the daily operation of the Department, for example: Policies or SOGS Equipment changes Personnel information 2 NOTE: These items will be carried on the sheet for at least three consecutive days. This information should be shared with personnel every duty day during roll call. E. Record of Personal Injury. The purpose is to establish a system to collect data on personal injuries and to assist us in correcting or alleviating contributing factors. The forms to be completed are: Employers First Report of Injury or Illness (Form 45) Accident/Injury Report All injuries shall be reported and a personal injury report completed by the firefighter who was injured, co-signed by that person's immediate supervisor or person in charge and the Battalion Chief utilizing the ImageTrend Accident/Injury Report. In addition, all injuries sustained by a firefighter on shift shall have a run number assigned to it by dispatch and a NFIRS report completed. All injury reports will be completed and forwarded through channels to the Deputy Fire Chief at the end of their tour of duty. F. Personnel Status Forms - The Battalion Chief shall be responsible for the completion of all Hoffman Estates Time Report forms (HETR), approving and/or disapproving, by his signature, forwarding this report to his immediate supervisor at the end of his tour of duty. All reports shall be forwarded at the completion of the duty day to the Deputy Fire Chief for final approval. The forms include Sick leave G. Training Reports: The purpose is to establish a monthly report that will identify the type, amount and location of training being done within the Department. This report will also maintain a record of attendance to meet state requirements for certification, along with documenting training hours required by the Department and state. 1. The Company Officer will be responsible for: a. Filling in all the necessary information on the monthly company training report for each member in his company. This information will include specific subject areas, state training sections and signatures of all company members. This will also include all on-duty training and outside of the Department training and college classes. b. Entering all training indicated on the daily company training report into the designated computer software program. c. Validating all probationary completions for the candidate’s rook book. 2. The Shift Commander is responsible for: 3 a. Documenting daily Training Report for all his/her continuing education and technical training. b. Forwarding all Training Reports to his/her Shift Training Coordinator, on a monthly basis. 3. The Firefighter is responsible for: a. Signing the monthly training report and noting that all training recorded is accurate. b. Reporting to the Company Officer (on a daily basis) any completed study logs for Advanced Technician Firefighter education or any other outside training. c. Reporting all classes attended and training not included in the Monthly Training Schedule and paid for as Departmental Continuing Education. d. Although not mandatory, the Department encourages the reporting of classes attended outside of the Departmental Continuing Education Program. e. The firefighter has the option, and is encouraged, to log their training/outside training into the Records Management System utilized by the department. 4. The Training Officer is responsible for: a. Establishing a recording and filing system to document all training (excluding EMS education). This system must meet both the state, OSHA and Department's requirements. b. Seeing that all monthly and technical reports are recorded on a timely basis. c. Tracking and logging all assigned outside training activities into the Records Management System utilized by the department. H. The Shift Training Coordinator will check all company-training reports for accuracy, completeness, consistency and legibility according to Department standards. Once approved, the Shift Training Coordinator will send all monthly training reports to the Training Officer. DEFINITIONS EXCEPTIONS REFERENCE End of Document 4
