PURPOSE: To establish a standard system to be used when recording information in the Journal in PSTrax. PREFACE: The PSTrax journal is a legal document recording events relevant to the functions of the fire station, Rescue Lieutenant (LR45), and District Commanders. The electronic journals are to serve those needing information while protecting a patient’s privacy rights. All locations will be given access and shall maintain a station electronic company journal. The officer in charge of each station and shift shall be responsible for the upkeep and accuracy of this journal. 603.1 Procedures A. The journal will be completed each day by the on-duty Lieutenant or Acting Lieutenant. B. PSTrax is where all electronic journal entries will be made. See following directions: 1. Sign into PSTrax. https://app1.pstrax.com/index.php 2. Select appropriate Station. 3. Click on Add Post on right side of screen near Journal. 4. Ensue Correct Date is selected. 5. Pick Category from drop down menu. 6. Add details under Post and Add any attachments if needed. 7. Select where Post is to display…either in selected station or entire department. 8. Choose if you want Post to remain on home page until archived. Yes or No. 9. Hit Save to finish. 603.2 Categories Station Pass on Report Administrative – evaluations, monthly reports, etc. Citizens – Assist, Complaint, Blood Pressure, First Aid, Incident Damage Critical Incident (Any incident that could have a negative psychological effect on our personnel) Department Vehicle Accident/Damage – All units SUBJECT: EFFECTIVE DATE: 08/19/99 S.O.P. 603 Station Company Journals REVISION DATE: 04/23/21 CATEGORY: SUBCATEGORY: PAGE 2 OF 2 Operations Equipment Damage – Tools, PPE, etc. Employee Injury/Issue – Injury, PBMP, etc. General – Building and Maintenance, Public Records Request, Media Inquiries, etc. Public Education – Station Visits, Tours, Parades, Block Parties, etc. Serious Incident Report – Criminal Acts, Disturbances, Workplace and Environmental Disasters, Theft, Threats, Violations, Death, Major Monetary Loss to the City Training – Personal or Crew Assist Vehicle Swaps Other Sick Leave (District Commander only) Station Visits (District Commander and LR only) Controlled Substances – Delivered, Picked Up, etc. (LR Only) Pick Up Crew from Hospital (LR Only) Placed Special Rescue (SR) in Service (LR Only) QAR – Related Duty (LR Only) Report QA (LR Only)
