- A prescription is a formal communication from a healthcare provider to a pharmacist authorizing dispensing medication.
- Prescriptions must contain specific patient and drug-related information like the patient's name, date of birth, the name of drug and dosage, dosage form, frequency of dose, refills (if applicable).
- Controlled substance prescriptions require a DEA number.
- Prescriber's signature (either physical or electronic) is also necessary.
