Manual Name: Document Number: S rin Fire De artment Policies & Procedures SFD.059 Document Title: Date: Health and Wellness Pro 10/25/2017 Chief's Signature: Revision Number: 01 PURPOSE The Spring Fire Department Wellness Program is designed to encourage and facilitate optimal wellness for all Spring Fire Department members, while ensuring each member can meet the physical demands of their position. This is achieved through a non-punitive incentivized program which includes nutritional counseling and/or education, consistent participation in fitness training, fitness assessments, annual physicals, and professional assistance and/or advice from an on- duty peer fitness trainer. This program is geared to be progressive, constructive, and non-punitive based on the current recommendations of the International Association of Firefighters (IAFF), International Association of Fire Chiefs (IAFC) Wellness-Fitness Initiative, National Fire Protection Association (NFPA) 1582 Standard on Comprehensive Occupational Medical Program, and NFPA 1583 Standard on Health Related Fitness Programs for Firefighters SCOPE All SFD members who are subject to the use of self-contained breathing apparatus (SCBA) shall be required to participate in all components of the SFD Wellness Program. Participation in this program will ensure that each member is capable of meeting the physical demands of the position and reduce their risk of injury while maintaining optimal health and longevity. Members that are not subject to the use of SCBA (administration staff, support members, etc.) are still required to participate in the annual medical physical. REFERENCES/DEFINITIONS Member: Any fulltime, part time, or volunteer personnel associated with the Spring FD. Wellness: The state of being healthy, especially as the result of deliberate effort. Wellness Program Coordinator (WPC): Directs Wellness Program. Peer Fitness Trainer (PFT): Fire department personnel trained to provide wellness education, nutritional guidance and assistance in the design programming of fitness routines. Health Care Provider (HCP): Agency contracted by the Spring Fire Department to conduct medical physicals. Annual Medical Physical: A thorough examination conducted annually by the contracted health care provider to determine one's overall wellness. Annual Fitness Assessment: Comprehensive annual fitness analysis to measure muscular strength, muscular endurance, flexibility, body composition, and aerobic capacity. Physical Training (PT): On-duty fitness training. Exercise/Nutrition Prescription: A customized 90-day PT regimen, designed by a PFT based on the results of an individual's previous fitness assessment. Proper diet/nutrition will also be discussed and recommended as part of the prescription. REVIEW The Executive Committee is responsible for reviewing this procedure and assuring its accuracy. PRIMARY COMPONENTS OF THIS PROGRAM The Primary components of this program: 1. Annual Medical Physicals 2. Annual Fitness Assessments 3. Fitness Training Annual Medical Physicals: The Spring Fire Department, through a contracted HCP, shall provide all Spring Fire Department members an annual medical physical. The medical examination will include, a health risk appraisal, a hands-on physical exam including vital signs, pulmonary function test, hearing test, blood chemistry lab tests, and specific cardiopulmonary and cardiovascular assessments. Following the medical exam, the HCP will provide the Spring Fire Department HR department with a document certifying each member is medically certified to perform emergency operation duties. In the event the health care provider identifies a health problem or condition that requires further evaluation, but would not prevent the member from performing their normal duties, the contract HCP will refer the member to his or her personal health care physician. Costs incurred with the referral will be the member's responsibility. If the contract HCP identifies a health condition that may prevent the member from performing their normal duties safely, the HCP will immediately notify SFD HR. The medical condition will not be revealed to HR or anyone else, other than the member. The HCP will recommend to the Department that the member be thoroughly evaluated by a physician and be administered a fit for duty evaluation, (conducted by a competent professional) before returning to duty. Annual Fitness Assessments: All SFD members who are subject to the use of self-contained breathing apparatus (SCBA) shall be assessed physically on a yearly basis. This non-punitive assessment shall comply with NFPA 1583 guidelines. The assessment will be conducted during the annual medical physicals by the HCP staff. The assessment will include measurements of muscular strength, muscular endurance, body composition, aerobic capacity and flexibility. The scoring for the assessment will be age relevant. The age brackets will be divided into 10-year increments. Those categories are: Poor, Fair, Good, Excellent, and Superior. Various incentives will be offered to any member obtaining Good, Excellent or Superior. Any member not obtaining at least fair will be prescribed an Exercise/Nutrition prescription which will be developed by the WPC or one of the PFTs. The member will work closely with an assigned PFT for a 90-day period. At the end of the 90-day period, the member will participate in a follow- up fitness assessment. This fitness assessment will be conducted by the WPC with assistance from the member's assigned PFT. If the member fails to obtain at least fair, they will again be assigned an exercise/nutrition prescription and will be assigned a different PFT for the next 90 days. This process will continue until the member obtains at least a "fair" on the assessment. Fitness Trainin g: All members, staffing from 0600-1800 shall participate in 60-90 minutes of fitness training each and every day while on duty. All members, staffing from 1800-0600 shall make every effort possible to do the same, unless they have already participated in fitness training at some point during the day. Fitness training is given the same priority as other forms of training. It is the supervisor's responsibility to make a reasonable effort to provide at least a 60-minute block of time for all members to participate in fitness training each day while on-duty. Supervisors may request guidance directly from the WPC or Peer Fitness Trainers. If a supervisor is concerned about a member's fitness level, the WPC should be contacted. A Peer Fitness Trainer may be assigned to develop an exercise/nutrition prescription for the member. Confidentiality of all medical information: It shall be the responsibility of The Spring Fire Department, WPC, Peer Fitness Trainers, the HCP, and each individual member to maintain the highest level of confidentiality pertaining to all medical information. Under the Health Insurance Portability and Accountability Act (HIPPA), any document containing medical information about any personnel is considered a medical record and is regarded as confidential. Records of fitness for duty evaluations shall be treated as confidential medical records. This information may be shared with the Spring Fire Department only to the extent necessary to protect the health and safety of the Employee and others, and to make determinations regarding fitness for duty, to the extent allowed by the ADA When choosing fitness activities, members shall select activities that: Improves muscular strength Improves muscular endurance Increases flexibility Improves anaerobic and aerobic capacity Team sports shall not be considered appropriate fitness training During the time allotted for fitness training, supervisors must: Preserve crew integrity to assure immediate availability to respond to calls. Make certain, their crew is familiar with fitness equipment and training principles; if necessary, request an orientation session from a PFT.
