MODULE 2: Hospital and its Organization Lesson 2 Organization of a Hospital, Medical Staff and Different Supporting Services Organizational Structure The Standard Hospital Organizational Chart HOSPITAL ORGANIZATIONAL CATEGORIES 1.Administration Services (often referred to simply as “administration”) 2.Informational Services / Social Services 3.Therapeutic Services / Clinical Services 4.Diagnostic Services / Ambulatory Services 5.Support Services / Administrator Services Administration Services ❑These includes business people who “run” the hospital. ❑Hospital Administrators generally includes the Hospital President, Vice Presidents, Executive Assistants, and Department Heads. ❑Oversee budgeting and finance, establish hospital policies and procedures, and perform public relation duties Informational Services Responsible for the documents and processing of information. 1. Admissions 2. Billing and Collection Departments 3. Medical Records 4. Information Systems 5. Medical Social Service or Health Education 6. Human Resources Informational Services 1. Admissions often the public’s first contact with hospital personnel that checks patients into hospital obtaining vital information of the patient assign in-house patients their hospital room 2. Billing and Collection Departments responsible for billing patients for services rendered Informational Services 3. Medical Records Responsible for maintaining copies of all patient records Basis for planning & continuity of patient care Provide data for use in research education Serves as basis for review & evaluation of the care 4. Very important liaison between the Hospital & patient and his community Provide information to patients regarding the risks and benefits related to medical and surgical treatment 6. Human Resources Responsible for recruiting/ hiring employees and employee benefits. Therapeutic Services 1. Physical Therapy (PT) 6. Social Services 2. Occupational Therapy (OT) 7. Pharmacy 3. Speech/Language Pathology 8. Dietary 4. Respiratory Therapy (RT) 9. Sports Medicine 5. Medical Psychology 10. Nursing Physical Therapy (PT) They provides treatment to improve large-muscle mobility and prevent or limit permanent disability. Treatments may include: ❑exercise, ❑massage, ❑hydrotherapy, ❑ultrasound, ❑electrical stimulation, ❑heat application. Occupational Therapy (OT) The goal of treatment is to help patient regain fine motor skills so that they can function independently at home and work. Treatments may include: ❑arts and crafts that help with hand-eye coordination, ❑games and recreation to help patients develop balance and coordination, ❑social activities to assist patient’s with emotional health. Speech/Language Pathology ❑They identify, evaluate, and treat patients with speech and language disorders. ❑They also help patients cope with problems created by speech impairments. Respiratory Therapy (RT) They treat patient’s with heart and lung diseases. Treatment might include: ❑oxygen, ❑medications, ❑breathing exercises. Medical Psychology Concerned with mental well-being of patients. Treatment might include: ❑talk therapy, ❑behavior modification, ❑muscle relaxation, ❑medications, ❑group therapy, ❑recreational therapies (art, music, dance) Social Services They aids patients by referring them to community resources for living assistance (housing, medical, mental, financial). Social worker specialties include: ❑child welfare, ❑geriatrics, ❑family, ❑correctional (jail) Pharmacy ❑They dispense medications per written orders of physician, dentists, etc. ❑They also provides information on drugs and correct ways to use them while ensuring drug compatibility Dietary ❑Responsible for helping patients maintain nutritionally sound diets Sports Medicine ❑Provide rehabilitative services to athletes and teaches proper nutrition. ❑They prescribe exercises to increase strength and flexibility or correct weaknesses. ❑They apply tape or padding to protect body parts and administer first aid for sports injuries Nursing ❑They provide care for patients as directed by physicians. ❑Many nursing specialties include: ❑nurse practitioner, ❑labor and delivery nurse, ❑neonatal nurse, ❑emergency room nurse, ❑nurse midwife, ❑surgical nurse, ❑nurse anesthetist. Diagnostic Services / Ambulatory Services Determines cause(s) of illness or injury. It includes the following departments: 1. Medical Laboratory (MT) and Pathology 2. Radiology or Imaging 3. Emergency Medicine Medical Laboratory (MT) and Pathology ❑Studies body tissues to determine abnormalities. ❑Conducts cytological & gross anatomical analysis. ❑Here samples of blood, urine, sputum, feces are tested for the presence of pathogenic infections or abnormality in biochemical parameters like sugar and urea content Medical Laboratory (MT) and Pathology Blood Bank ❑Generally under the supervision of a licensed physician who has a basic interest in hematology. ❑It collects, processes and supplies blood and its product like blood plasma and platelets. Radiology or Imaging Image body parts to determine lesions and abnormalities. It includes the following: ❑Diagnostic Radiology, ❑MRI, ❑CT, ❑Ultra Sound. Emergency Medicine Provides emergency diagnoses and treatment Support Services Provides support to entire hospital. It includes the following departments: 1. Central Supply 2. Biomedical Technology 3. Housekeeping and Maintenance Central Supply ❑They are in charge of ordering, receiving, stocking and distributing all equipment and supplies used by healthcare facility. ❑Responsible to sterilize instruments or supplies as well as cleaning and maintaining hospital linen and patient gowns. Biomedical Technology ❑They design and build biomedical equipment (engineers). ❑They diagnose and repair defective equipment (biomedical technicians). ❑They provide preventative maintenance to all hospital equipment (biomedical technicians). ❑Responsible for pilot use of medical equipment to other hospital employees (biomedical technicians) Housekeeping and Maintenance ❑Responsible for maintaining safe clean environment. ❑These includes cleaners, electricians, carpenters, and gardeners. MEDICAL STAFF TYPES OF MEDICAL STAFF MAIN TYPES SPECIFIC TYPES ❑Open Staff ❑Honorary ❑Closed Staff ❑Consulting ❑Active/ Attending ❑Associate ❑Courtesy ❑Resident OPEN STAFF ❑One in which certain physicians, other than those on the attending or active medical staff, are allowed to use the private room facilities, providing they comply with all rules & regulations of the institution. ❑These physicians are termed members of the “Courtesy Medical Staff”. CLOSED STAFF ❑One in which all professional services, private & charity are only provided & controlled by the “Attending” or “Active Medical Staff”. Honorary ❑Has been active and consists of former staff members, retired or emeritus, & of other practitioners whom the medical staff chooses to honor. Consulting ❑Recognized specialists who passed the specialty boards. ❑It consists of practitioners of recognized professional ability who are not members of the preceding categories of staff membership. ❑They are consultant to other medical staff. Active/ Attending ❑Concerned w/ direct patient care & counseling. ❑They are responsible for the delivery of the pre- ponderance of medical service within the hospital. ❑They are most involved in the organizational & administrative duties pertaining to the medical staff. Associate ❑They are junior or less experience interns, clinical clerks. ❑It consists of individuals who are being considered for advancement to the active medical staff. ❑They were appointed & assigned to the various services in the same manner as are members of the active medical staff. Courtesy ❑Attend to private patient but are not members of the active medical staff. ❑They consists of practitioners who are eligible for staff membership, who are given privileges to admit an occasional patient to the hospital. ❑They may neither vote nor hold office in the medical staff organization. Resident ❑Receiving specialized clinical training in a hospital, usually after completing an internship.
