Standards of Nursing Practice National Nursing Core Competency Standards (NNCCS) Heightened by the escalating complexity of globalization, dynamics of information, technology, demographic changes, health care reforms and increasing demands for quality nursing care from consumers, expectations for contemporary nursing practice competencies emerged National Nursing Core Competency Standards (NNCCS) 2005 - as an output of a key project, Board of Nursing Resolution no. 112 Series 0f 2005, adopted and promulgated the Core Competency Standards of Nursing Practice in the Philippines. 2009- the Board of Nursing created the Task force on Nursing Core Competencies Revisiting Project in collaboration with the Commission on Higher Education Technical Committee on Nursing Education with the primary goal of determining the relevance of the current nursing core competencies to expected roles of the nurse and to its current and future work setting. National Nursing Core Competency Standards (NNCCS) 2012- Public hearing on the revised and modified core competency standards of nursing practice in the Philippines was conducted in January, 2012 in the cities of Manila, Davao, Cebu and Baguio. Integrating all the comments, suggestions and recommendations for the improvement of the revised and modified core competency standards, the last phase of the refinement of the core competency standards was done in May 2012. National Nursing Core Competency Standards (NNCCS) Legal bases: Article III, section 9 (c) of Republic Act No. 9173 or the Philippine Nursing Act of 2002 states that the Professional Regulatory Board of Nursing is empowered to “monitor and enforce quality standards of nursing practice in the Philippines and exercise the powers necessary to ensure the maintenance of efficient, ethical and technical, moral and professional standards in the practice of nursing taking into account the health needs of the nation.” It is, therefore, incumbent upon the Board of nursing to take the lead in the improvement and effective implementation of the core competency standards of nursing practice in the Philippines to ensure safe and quality nursing care, and maintain integrity of the nursing profession National Nursing Core Competency Standards (NNCCS) 2012 NATIONAL NURSING CORE COMPETENCY STANDARDS (2012 NNCCS) I.BEGINNING NURSE’S ROLE ON CLIENT CARE Responsibility 1: Practices in accordance with legal principles and the code of ethics in making personal and professional judgment. Responsibility 2: Utilizes the nursing process in the interdisciplinary care of clients that empowers the clients and promotes safe quality care. Responsibility 3: Maintains complete and up to date recording and reporting system. Responsibility 3: Applies the research process in improving client care in partnership with a quality improvement /quality assurance/nursing audit team Philippine Professional Nursing Practice Standards (PPNPS) The Standards of Professional Nursing Practice are authoritative statements of the duties that all registered nurses, regardless of role, population or specialty are expected to perform competently (ANA, 2010, p. 2). The ANA further states that the standards can change as the dynamics of professional nursing evolve and that specific clinical circumstances or conditions might affect the application of the standards at any given time. Philippine Professional Nursing Practice Standards (PPNPS) Thus, in September 1994, the PRBON, then chaired by Aurora S. Yapchiongco, called for a consultative meeting of presidents/representatives of national nursing associations and concerned groups and individuals on "Safe Nursing Practice." Thereafter, it organized a two-day workshop, the output of which was incorporated in the document on Standards of Safe Nursing Practice. After several meetings and discussions, BON Resolution No. 82 issued on September 30, 1998, adopted and promulgated the Standards. Patient Care Safety Standards Patient safety is a framework of organized activities that creates cultures, processes, procedures, behaviours, technologies and environments in health care that consistently and sustainably lower risks, reduce the occurrence of avoidable harm, make error less likely and reduce its impact when it does occur. Standards for reporting incidents Incident reporting is the act of documenting all incidents and near incidents, ideally using an online form and workflow to capture details and share the account digitally. (Near) Incidents are also reported through complaints, audits, and safety rounds. Patient Care Safety Standards 2. Standards for sharing learnings A study of 30 adverse events in cardiovascular surgery pinpointed system design problems, rather than negligence or incompetence by individual clinicians. The researchers’ conclusion? Embracing a systems-theoretic accident analysis technique—rather than assigning blame to individual clinicians or technicians—helps to better understand the root causes of adverse events. However, mere discovery of adverse events, without sharing those learnings among key stakeholders, is unlikely to drive quality improvements. Patient Care Safety Standards 3. Adopting a uniform terminology Understanding medical terminology, the “language of medicine,” allows clinicians and staff to communicate effectively for maximum safety. For healthcare providers, it starts with their initial education and training and never stops—as they must keep up to date with new regulations and medical advancements once on the job. Standards for analyzing events The ability to report incidents internally and externally and to create action plans for improvement requires the use of standard analytical methods. Patient safety, quality improvement, and risk management professionals have numerous methods to analyze incidents through a variety of retrospective and prospective methods. Patient Care Safety Standards 5. Aligning departments and facilities Ensuring alignment across today’s multi-faceted healthcare organizations is challenging. To ensure alignment around patient safety, all team members must take responsibility for quality improvement. In addition, quality, risk, and patient safety professionals must assess the organization's safety culture, understand the relationship between organizational objectives and patient safety, and help the facility learn structurally from incidents. Automating quality and risk processes Using a digital quality management system—one that includes an improvement-tracking system that supports the management of improvement projects—has a powerful impact on patient safety. This approach can be initiated as a result of a specific adverse event, a trend, or a specific issue. Quality, risk, and patient safety leaders can design workflows within the digital system to create tasks, actions, and lead times to ensure the implementation of quality improvement initiatives.
