9/7-00.00 REPORTS & RECORDS/PROCEDURES A 04-21 NOTICE TO IMPROVE PERFORMANCE F-1104 FORM NAME: NOTICE TO IMPROVE PERFORMANCE INITATED BY: Any supervising authority with direct knowledge of a deficiency. NUMBER OF COPIES: Three PERIOD RETAINED: Until Deficiency Corrected, not longer than 6 months from date of issue FORWARD: As needed REFERENCE: Manual of Operation, Vol. 2, 3/2-20.20 ROUTING: Original - Member Duplicate – Member’s immediate supervisor Triplicate – Member’s Bureau Commander APPLICATION: When a deficiency has been observed in a member's appearance, attitude, work habits, performance or actions, any supervisor with direct knowledge of the deficiency may use a Notice to Improve Performance (NTI) as a tool to secure correction. The form shall be completed in triplicate, signed by the issuing supervisor, and the matter discussed with the member. The member shall sign all copies of the form to verify that the notice was received and that the deficiency and the corrective action being taken were discussed. The member’s signature does not denote concurrence, or non-concurrence, with the Notice to Improve Performance. The first copy of the F-1104 will be given to the member, the second copy will be filed by the member’s immediate supervisor and the third copy will be forwarded through channels to the member’s Bureau Commander. If the issuing supervisor is not assigned to the member’s chain of command (e.g. either the supervisor or the member is working SOD from another chain of command), the issuing supervisor shall ensure that the copies are directed to the member’s chain of command, the issuing supervisor’s chain of command shall not retain any copy of the F-1104. If the member refuses to sign the document, the issuing supervisor shall sign the document in member's presence (with date and time), documenting the member's refusal. 9/7-00.00 REPORTS & RECORDS / PROCEDURES A 04-21 PAGE 2 F-1104 When the deficiency is corrected, and in no case longer than 6 months from the date of issue, the member’s immediate supervisor shall indicate so on both the first copy and the second copy. The member’s immediate supervisor shall forward the second copy through channels to the member’s Bureau Commander. Upon receipt of the annotated second copy at the Bureau office, the member’s Bureau Commander shall cause the second and third copies to be returned to the member. If the deficiency is not corrected, subsequent notices to improve may be issued. If it becomes necessary to refer the matter for disciplinary action, the fact that F-1104 – Notice(s) to Improve Performance had been issued to correct the deficiency may be included in the Complaint Tracking System entry; however, the Notices themselves shall not be attached to the complaint.
