Describe the prioritized groups to benefit and those implementing the intervention 4. Obtain the clients' input. Steps For Planning Community Intervention 5. Analyze the problem or goal to be addressed by the intervention 6. Set goals and objectives for successful intervention. 7. Identify and assess "best practices" or "evidence-based interventions" in addressing the problem. Steps For Planning Community Intervention 8. Specify the core components and elements of the intervention. 9. Identify the mode of delivery of the component of the intervention 10. Develop an action plan for the intervention Steps For Planning Community Intervention 11. Apply a Pilot-test for the intervention on a small scale. 12. Implement the intervention, monitor and evaluate the process. 1. Identify the community problem/goal to be addressed It includes: • Clear statement of the community problem or goal. • The specific behaviors that needs to be changed. • The intended improvements in communitylevel outcomes. 2. Assess the level of the problem or goal Types of assessment include: • Direct observation of the problem or goal. • Conducting behavioral surveys. • Interviewing key people in the community. • Reviewing archival or existing records. 3. Describe the prioritized groups to benefit and those implementing the intervention It Includes: • Targeted groups in need to change. • Targeted behaviors to be changed. • Agents of the change. • Who will implement the intervention. 4- Indicate how you will obtain clients' input Consider how you will use: Personal contacts Interviews - What questions you will ask about the problem or goal and possible interventions? Focus groups - From what groups you will seek information and type of information? 5. Analyze the problem or goal to be addressed by the intervention Use client input to specify: For whom the current situation is a problem. The negative (positive) consequences for those directly affected and the broader community. 5. Analyze the problem or goal to be addressed by the intervention Personal and environmental factors to be influenced. The behavior or lack of behavior that causes or maintains the problem. 5. Analyze the problem or goal to be addressed by the intervention Who benefits and how from the situation staying the same. The conditions that need to change. 6. Set goals and objectives for successful intervention Include: A description of what success would look like. Determination of specific objectives of the intervention to be achieved. How will you know if your intervention is successful? (SMART) 7. Identify and assess "best practices" or "evidencebased interventions" in addressing the problem Indicate: Potential or promising “best practices” for situation. E.g(Best practice for DM patient) How strong is the evidence for best practice 7. Identify and assess "best practices" or "evidencebased interventions" in addressing the problem Whether the “best practice” could achieve the desired results in your community. 8. Specify the core components and elements of the intervention By: 1. Providing information and enhancing skills. 2. Modifying access, barriers, exposures, and opportunities. 8. Specify the core components and elements of the intervention 3. Enhancing services and supports (e.g., increase the number of centers that provide health care). 9. Identify the mode of delivery of the component of the intervention It will be delivered in the community by: • Workshops. • Skills training. • Outreach approach 10. Develop an action plan for the intervention Include: Specific change or aspect of the intervention. • Goal and objectives • What steps are to be taken - list of activities • Who will be responsible for each step - person in charge • What materials, equipment, people, funds will be necessary for each step - resources • When each step is to be completed - target date. 11. Apply a Pilot-test for the intervention on a small scale To determine how to: • Test the intervention and with whom. • Assess the quality of implementation of the intervention. • Assess results and consequences or side effects. • Collect and use feedback to adapt and improve the intervention. 12. Implement the intervention, monitor and evaluate the process Check: • The Quality of implementation. • The Satisfaction. • Attainment of objectives and outcomes.
