GENERAL.01 DEFINITIONS - ALL MEMBERS Consecutive calendar day Each day in a continuous period that an employee does not report for work because of disablement, whether or not it is a regularly scheduled working day. For platoon duty personnel on IOD or NIOD medical leave, the first day starts at 0800 hours and the last day ends at 2000 hours. For other leaves, refer to the appropriate manual sections of the specific leave. Disabled Unable to perform assigned duties because of any sickness or injury. Extended Leave Military Leave more than 30 days Civil Service Leave more than 15 days Disciplinary Leave more than 15 days Family Leave First-care Initial treatment for an illness or injury. Follow-up care Continue care after the initial treatment for an illness or injury which is authorized by the Workers' Compensation Officer or Medical Liaison Unit. IOD Illness or injury on duty arising or caused by duties performed in connection with employment. Officer Normally an officer or member in charge on duty at the member's place of assignment. NIOD Illness or injury not arising from or caused by duties performed in connection with employment. Qualified emergency facility Shall consist of one of the following. (Refer to Book 35). Comprehensive emergency service hospital. Basic 24-hour emergency service hospital. Released for duty The date and time that a disabled member is officially available for duty. For platoon duty personnel this shall be 2000 hours. Exceptions shall be approved by the Medical Liaison Officer. For other than IOD or NIOD medical leaves, refer to the appropriate manual sections for the specific leave. For Special Duty Personnel, the nearest whole hour to the time of reporting for duty in uniform or to the time of release to duty by Central Receiving Clinic when such release is required. 3/5 MEDICAL 2 -10. MEDICAL LIAISON UNIT.01 FUNCTIONS The Unit arranges medical assistance for members, maintains medical records, coordinates bi-annual medicals with the Occupational Health and Safety Division, represents the Department at pension hearings, responds as a part of staff to major emergencies and responds to members hospitalized due to IOD injury or illness. The Unit arranges necessary emergency care 24 hours a day and follow up care during regular business hours. It also assists members in their dealing with the Workers' Compensation Division of the Personnel Department. When first care or assistance is needed for IOD injury or illness, contact this Unit..11 METHODS FOR NOTIFYING THE UNIT A. During business hours, call direct. If no phone is available, notify Metro. B. Other hours, obtain the phone number of the "on call" Medical Liaison Unit Officer from Metro. If the Officer cannot be contacted by phone, call Metro and request the Officer be paged..21 NOTIFICATION REQUIRED 1. All members shall notify their commanding officer of any illness, injury, condition or limitation that may affect their ability to safely perform the full range of duties and responsibilities of their position. Any officer notified of such a condition or limitation of an assigned member shall contact the Medical Liaison Unit. 2. To assure that members will receive proper care, supervisors on duty (usually the commanding officer on duty at member's place of assignment) shall notify Medical Liaison Unit as soon as possible in all of the following cases: A. All cases where IOD or possible IOD requires first care or follow-up care. B. Member's emergency admission to any hospital whether IOD or NIOD. C. Members who are scheduled for admission to any hospital, sanitarium or rest home. -20. SPECIFIC INCIDENTS - ALL MEMBERS.01 ANIMAL BITES When a member on duty is bitten by an animal: A. Follow IOD procedures for the member. B. If possible, arrange to keep the animal available. Attempt to locate the owner and have him restrain the animal. Otherwise, keep the animal in the immediate area until the Rabies Control Center is notified through Metro and arrives. C. Include on F-166A: 1. Address where injury occurred. 2. Name, address and phone number of animal's owner, if obtainable. 3. Whether animal was chained, confined, or running loose. 3/5 MEDICAL 3.11 CONTAGIOUS/INFECTIOUS DISEASE If a known or suspected exposure to an infectious disease has occurred, the member shall: A. Immediately cleanse the exposed area. B. Notify an officer on duty at their place of assignment. C. Officer shall refer to Training Bulletin 82 to determine the "level" of exposure. D. Level III Exposures requires immediate notification of Medical Liaison Unit. Medical Liaison will direct the member to an appropriate facility for first care, base line testing and counseling. E. Complete an F-225 or F-166A (State DWC Form 1) for all Level II and III Exposures. F. Detailed Journal entry and documentation in the member's Personal Record Book. G. Complete and forward F-420. H. Check Inquiry Box on F-902M. I. Follow up with Infection Control Nurse at Receiving Hospital..21 CONTAMINATED NEEDLE STICK Members receiving a puncture from a contaminated needle shall: A. Immediately cleanse the wound. B. Check the "Inquiry Requested" box on the F-902M. C. Notify the Receiving Hospital of the exposure. D. Complete and forward an F-420. E. Concerned officers shall immediately notify Medical Liaison Unit. F. Detailed journal entry and documentation in the concerned member's Personal Record Book. G. Complete F-225, F-166A, and (State DWC Form 1)..25 ASSAULT/BATTERY ON MEMBERS Anytime a Department member is assaulted, the member’s Battalion/Section Commander shall be notified and police response requested. Medical Liaison shall be contacted and the assaulted member shall be sent to a first care facility and be evaluated by a doctor to determine the extent of injury. First care shall be sought, even though the member feels they were not injured. A request shall be made to the investigating officer(s) that the assailant, if identified, be arrested and charged with assaulting a firefighter while performing their assigned duties. In all cases, verbal notification of the assault shall be made through channels to the Bureau Commander followed with F- 225’s from concerned parties through channels to the Arson Unit to document the incident..31 HEART, HERNIA, PNEUMONIA, PNEUMONITIS Notify an officer on duty at your place of assignment when an illness or injury has been diagnosed as heart trouble, hernia, pneumonia, or pneumonitis. The officer shall notify the Medical Liaison Unit. 1. Presumptions for any of these medical problems is based on being job related and must be evaluated and confirmed by a Workers' Compensation physician. A congenital defect is not considered to be presumptive under the law. For pneumonia to be presumptive it must be confirmed by X-rays and will be evaluated by the radiologist at the Occupational Health and Safety Division. 3/5 MEDICAL 4.41 TOXIC SUBSTANCES A member who is exposed while on duty to toxic dust, fumes or vapor such as those present in nitric acid spills, chlorine leaks, organic phosphate insecticides and halogenated hydrocarbons, shall be handled as follows: A. Treat a member with any symptoms as a litter case while awaiting an ambulance or medical assistance. B. The responsible officer shall follow the procedures of IOD, including complete records and reports for each member exposed. (Refer to Vol. 5, 9/7-00.00 Form Instructions F-2, F-166A, 66-WC10, F-734, F-735, and F-225.) -25. BLOOD DONATIONS.01 GENERAL Member participation in the American Red Cross Blood Bank Program is voluntary and must be exercised during off-duty time. Platoon duty personnel shall arrange to donate blood at a time which will permit 24 hours to elapse before returning to duty. EXCEPTION Members of this Bargaining Unit, who are either assigned or detailed to Special Duty, who wish to donate blood shall be granted one hour of on-duty time from work. Members may donate blood, on duty, a maximum of four times per year..50 REQUEST FOR BLOOD A member desiring information concerning the blood bank program should contact a Relief Association trustee or the Relief Association office. If unable to contact either of the above, a member should advise their doctor that they only want American Red Cross blood and that the Relief Association will guarantee replacement. -27. VACATION DISABILITY - ALL MEMBERS.01 VACATION If disability occurs while on vacation, the members shall notify the Medical Liaison Unit and be guided by their instructions. -30. POLICY - IOD - ALL MEMBERS.01 GENERAL An employee, ill or injured from any cause, wants to recover and wants the necessary medical care to do so. The City also wants the employee to recover. The City will furnish the necessary medical care for IOD, but cannot legally furnish medical care for NIOD. When the cause of illness or injury is clearly IOD or clearly NIOD, the responsibility for medical care is obvious. When there is doubt as to IOD-NIOD status: A. In non-emergency case, it is better to seek advice from Medical Liaison Unit as to status before securing medical treatment. B. In emergencies the first consideration is proper care. A decision as to who pays can be made later. In cases which are probably IOD the City will usually pay expenses until the case is ruled NIOD. The member will receive the best possible care when they follow applicable procedures set forth in this medical chapter of the Manual of Operation. 3/5 MEDICAL 5.11 IOD RIGHTS AND CARE This subsection summarizes the employees' legal rights to IOD care and outlines the City's policy for meeting its responsibilities. A. ENTITLEMENT You are entitled to medical, surgical and hospital care that may be reasonably required to cure or relieve from the effects or aggravation of any illness or injury caused by employment. The provision of medical attention is part of Workers' Compensation and is administered by the City's Workers' Compensation Officer (Personnel Department). B. REPORTING Every injury must be reported to your supervisor promptly; if you fail to do so, you may lose your rights to Workers' Compensation. The supervisor shall promptly take action to provide necessary treatment and protect the employees' rights. Supervisors shall provide the employee, within 24 hours or one business day, with the State DWC Form 1. A supervisor should give this form to the injured employee as soon as medical treatment is sought. The form must be completed and forwarded, in addition to the F-166A, whenever the member receives medical treatment. C. MEDICAL TREATMENT URGENTLY REQUIRED In a serious emergency first-care may be rendered by any doctor, but quick, qualified care is most probable at the nearest qualified emergency facility. The employee may request and receive emergency first-care at a reasonably located qualified facility unless the EMS personnel determine immediate emergency care is needed at the nearest facility. D. CHANGE OF DOCTORS If you desire a change from your treating physician, you may request such change through Medical Liaison Unit to the Workers' Compensation Officer. Within twelve days, he will furnish you the names of five competent physicians. You may select one of the five to attend to you. The attending physician will ordinarily select the treating facility. Until such time as you are examined by the newly selected physician, you will remain under the care of the previous attending physician. E. CONSULTING PHYSICIANS In a serious case you are entitled to a consulting physician of your choice at City expense. A request for this consultant may be made through Medical Liaison Unit to the Workers' Compensation Officer. You may have any other consulting physicians you choose at your own expense. F. HANDLING YOUR OWN CASE If you are prepared to prove that, after demand on the Workers' Compensation Officer, the City unreasonably neglected or refused to provide reasonable, timely medical attention, you may obtain medical care independently and compel the City to pay for it. G. Any of the above requests for IOD care shall be made directly to the Fire Department Medical Liaison Unit. 3/5 MEDICAL 6 -40. CENTRAL RECEIVING CLINIC HOURS.01 GENERAL Members reporting to Central Receiving Clinic on a nonemergency basis, shall make their visits conform to the established time schedules..20 MEDICAL SERVICE Medical services are available at the Central Receiving Clinic, 1401 West Sixth Street, Los Angeles. Central Receiving Clinic business hours are 0730 to 1600. The clinic is closed on Saturdays, Sundays, and all legal holidays. The following services are available during business hours: Return to duty for NIOD injuries/illness. Medical Record Office Pharmacy Note Workers' Compensation cases: Prescriptions issued for medicine or equipment by Workers' Compensation doctors may be filled by visiting, telephoning, or mailing prescriptions to the Central Clinic Pharmacy, date of injury and Workers' Compensation number will be required. For further instructions, call the Pharmacy at 485-4622. Appointments shall be made through the Medical Liaison Unit for any of the following services: First Care treatment Follow-up treatment Doctor consultation and examination Orthopedic Clinic Cardiac Clinic -50. RECOVERY - ALL MEMBERS.01 GENERAL Members off duty due to any illness or injury shall: A. Concern themselves with recovery, not engage in any activity which might hinder or delay their recovery, and make every effort to facilitate their return to duty. B. Not leave their general area of residence without securing permission in writing from their treating physician with final approval from the Department through Medical Liaison Unit. C. All members on platoon duty shall contact their immediate supervisor once each segment to provide information regarding their duty status (IOD or NIOD). D. All members on special duty shall contact their immediate supervisor on a weekly basis to provide information regarding their duty status (IOD or NIOD). E. All members shall notify the Medical Liaison Unit when going on or off duty due to an IOD injury or illness. F. All members off duty due to IOD injury or illness shall contact the Medical Liaison Unit after each doctor's visit to advise of their prognosis and projected duty status. G. Civilians who are off duty NIOD more than 25 consecutive working days shall forward a report of their medical condition, prepared by their physician, to Medical Liaison Unit. (L.A.A.C. 4.126G) 3/5 MEDICAL 7.02 RECOVERY - ALL MEMBERS The responsibility for tracking members off duty IOD/NIOD will be that of the Station or Unit Commander at the member's assignment. A. Station and Unit Commanders shall, on the first business day of each month, contact the Medical Liaison Unit and relay all pertinent information relating to the off-duty members' medical and duty status. B. Station and Unit Commander (after each contact) shall make a comprehensive journal entry (F-2) containing all details on the status of the off duty member (IOD or NIOD). C. Station and Unit Commanders shall monitor all members off duty for an IOD/NIOD (injury or illness) for compliance with Department policy on leaves. D. Any suspected abuse of medical leave shall be reported to the Medical Liaison Unit immediately for review and follow up. All questions shall be directed to the Medical Liaison Unit at (213) 485-6083..11 MEDICAL APPOINTMENTS A. IOD: Members shall schedule appointments on an off duty day. Otherwise, members shall notify their immediate superior as soon as practical of the conflict with their work schedule. B. NIOD: Any appointment for medical, dental, optical or similar service should be scheduled when the member is off duty. When this is not possible, not more than 16 hours a year and not less than one hour at a time may be used for this purpose. Such time will be charged to allowable NIOD leave. Notify the immediate superior as soon as possible. Refer to Vol. 2, 3/3-25.22 Firefighter and 3/4-50.35 Civilian..21 PAYMENT REQUESTS A. Members who receive hospital and doctor bills for IOD injuries and illness shall immediately forward them to Medical Liaison Unit by Department mail. A memo should be attached indicating: 1. Member's name and assignment. 2. Date of injury. 3. Workers' Compensation case number. B. Members who want reimbursement for prescriptions that have been filled at their own expense due to IOD injuries and illness shall forward the receipt to Medical Liaison Unit by Department mail. A memo should be attached indicating: 1. Members name and assignment. 2. Date of injury. 3. Workers' Compensation case number. C. Members are entitled to reimbursement for travel expense incurred while securing treatment for IOD injuries and illness at the current rate per mile. Application forms (ESV 19) are available from Supply and Maintenance. Forward completed forms to Medical Liaison Unit. 3/5 MEDICAL 8.31 OFFICERS AND SUPERVISORS A. Supervisors: Have the responsibility to see that members do not abuse the rights and privileges provided by the City for the ill and injured. They also have the responsibility to provide members with guidance and assistance. They may require the member to secure the services of a physician for any NIOD disablement. B. Visitation: Members off duty will be visited for both official and humanitarian purposes. Such visits may be in person or by phone. 1. IOD: Visitation will be made by Medical Liaison Unit but this does not preclude visits by the members' superior officers. 2. NIOD: Members shall be visited if the superior officer considers it necessary to carry out the intent of this section. (a) Personnel under the command of fire officers: The immediate superior is responsible for visitation. When the visit will be in person, arrangements shall be made through the Battalion or Section Commander. Battalion Commanders nearest the member's place of confinement may be requested to make the visitation. (b) Civilian Personnel: The immediate superior is responsible for visitation. C. First care assistance: Officers are dispatched to emergency facilities where members are receiving first care for IOD. Their purpose is two-fold: 1. To assure that members receive necessary care, the officer should contact Medical Liaison Unit when assistance is needed and whenever follow-up care will be required. 2. To provide Metro with the information needed to notify the next of kin as required by Vol. 2, 4/6-80.84..35 WORKER'S COMPENSATION CASE NUMBERS Medical Liaison will inform the officer at injured member's assignment of the 10 digit Workers' Compensation case number. When this number is received, the on-duty Station Commander shall enter the number in the following places: 1. Margin of the station journal on same page and date of reported injury. 2. On member's copy of F-166A. 3. In member's Personal Record Book. 4. On Cal/OSHA Form 300. 5. On Cal/OSHA copy of F-166A. 6. SASSY computer program. If member has transferred to another assignment, record the number at the assignment where the injury occurred on the Cal/OSHA Form 300 and the Journal, then relay the information to the member's new assignment. If the Workers' Compensation number has been issued to an NIOD leave that is being evaluated by Workers' Compensation for possible conversion to IOD, it is being used as a tracking device only and DOES NOT confirm a conversion to IOD. Contact Medical Liaison with any questions. 3/5 MEDICAL 9 -60. PHYSICAL CONDITION - EXAMINATIONS: UNIFORM PERSONNEL.01 PHYSICAL FITNESS The following requirements shall be adhered to by all members: A. Members shall follow the recommendations of Medical Liaison Unit, private physicians and/or physicians employed by the Personnel Department concerning the maintenance of physical fitness. B. Commanders, at all levels, shall concern themselves with the physical fitness of members under their supervision. Stamina, strength, maintenance of proper weight and any other condition which affects a member's performance of assigned duties, shall be considered. C. When it becomes apparent that a need for special instructions, guidance or evaluation exists (including weight evaluation), the Battalion or Section Commander shall notify the Medical Liaison Unit and be guided by their instructions. D. All on-duty members shall participate in the Department's physical fitness training program, including members working SOD or exchange of time. Unless excused by a written medical restriction from a private or City physician and approved by Medical Liaison Unit..12 MEDICAL EXAMINATIONS A. Uniformed personnel shall be scheduled on duty to receive their periodic medical examination. B. Medical examinations are scheduled every two years or: 1. Return from extended leaves. 2. DL51A renewal. 3. IOD or NIOD work fitness determination. 4. Exit Medical - Optional The examination is scheduled by: 1. Battalion and Section Commanders of uniformed members assigned to Emergency Services Bureau. 2. Bureau Commander of uniformed members assigned to Fire Prevention Bureau and Training and Support Bureau. 3. Medical Liaison Unit will schedule Periodic Medical Examinations for all uniformed members not assigned to the above listed bureaus. Once an examination is scheduled, deviation is permitted only with the approval of the member's Battalion Commander or other scheduling officer who shall call directly to the Medical Liaison Unit and request the change. 4. Members scheduling medicals should schedule them within 60 days of the member's birth date. C. Extended Leaves: Personnel Services Section shall request Medical Liaison Unit to schedule a medical examination for members returning from an extended leave of absence prior to their returning to duty. Refer to Vol. 2, 3/3-01.69. D. Driver's license examinations as required by the Department of Motor Vehicles: to obtain, renew or maintain a class A or class B driver's license, the driver must possess a DL51A (Medical Certificate). This certifies that the driver has had a medical examination and met the medical standards of the Department of Motor Vehicles within the past two years. Members who are required to have a class A or class B driver's license will have a DL51A to be completed by the examining physician at the time of the periodic medical examination: 1. Regardless of the expiration date on their current DL51A. 2. If the DL51A is original issue. The completed DL51A will be given to member at time of examination or forwarded to place of assignment by the Medical Liaison Unit. The DL51A must be 3/5 MEDICAL 10 carried with the driver's license at all times. E. Officers shall request a medical evaluation when a member is returned to duty from any IOD or NIOD leave, if there is concern about the member's ability to perform the unrestricted duties of the assignment. Officer shall contact their Battalion or Section Commander who shall notify Medical Liaison Unit and request a medical evaluation. Officers must be prepared to make a statement as to their concerns about the member's physical condition or ability to perform the unrestricted duties of the assignment. F. Recording examination results: Member's immediate supervisor shall make entry in Personnel Record Book (F-733) of all applicable information when medical examination information is forwarded to member's place of assignment. NOTE All indicated irregular findings will be entered in RED INK. -70. PROCEDURES - IOD - UNIFORM PERSONNEL.01 GENERAL These procedures provide a means for members to receive proper medical care for IOD. They do not spell out all legal rights. Refer to Vol. 2, 3/5-30.01..11 MEMBER A. NOTIFICATION OF IOD: Members on or off duty who incur an illness or injury which they believe to be IOD shall, as soon as possible and regardless of the hour, notify an officer on duty at their place of assignment. Members shall, when possible, provide information to complete the F-166A if a medical examination or treatment is required. NOTE This notification shall include any conflict in anticipated length of leave and scheduled overtime duty days. B. SECURING MEDICAL CARE FOR IOD: 1. While on duty: a. In quarters, or in a non-urgent case out of quarters, a commanding officer at the place of assignment will arrange necessary care. If immediate treatment is not necessary, notify the Medical Liaison Unit prior to sending member for first care. b. Out of quarters, urgent care needed: The commanding officer or the member in charge shall arrange transportation to the nearest qualified medical facility, and notify Medical Liaison Unit as soon as possible. 2. While off duty: In case of serious IOD, proceed to the nearest medical facility for first care treatment. For any follow-up care or non-urgent first care, notification made as required by "A" above will provide for necessary care. C. ON OR OFF DUTY STATUS: When medical care is required for IOD, the treating physician will determine on or off duty status. Obtain a completed Medical Treatment Certificate from the hospital or attending physician (see Note). The member shall promptly notify the commanding officer on duty at the member's place of assignment of the member's status. Members who obviously cannot perform their duties will be placed off duty, pending determination and notification as required above. NOTE Medical Treatment certificate may be F-1000, PDAS F-165 (Used by Central Receiving Clinic) or form used by the treating physician. Non-urgent cases, a commanding officer at the place of 3/5 MEDICAL 11 assignment will contact Medical Liaison Unit prior to arranging for member's first care..21 OFFICER ON DUTY - IOD An officer on duty shall: A. When IOD requiring medical care occurs on duty, provide transportation to the nearest qualified emergency facility (or one reasonably located if member requests and member's condition permits), or in a serious emergency, may secure immediate services of any available physician, whichever will provide the best care. 1. Promptly notify Medical Liaison Unit at any hour of IOD or possible IOD requiring medical care. If immediate treatment is not necessary, notify Medical Liaison Unit prior to sending member for first care. NOTE: When contacting Medical Liaison Unit to report IOD injuries or illnesses, be prepared to give the following information: 1. Name 2. Home Address 3. Social Security Number 4. Home Telephone 5. Assignment 6. Rank 7. Date and Time of Injury 8. Date Employer Notified 9. Type of Injury - Body Part - Cause 10. Incident Number 11. Address where injury occurred. B. Supervisor shall provide the injured member with a State DWC Form 1 within 24 hours or one business day of the date the supervisor becomes aware that the member is requesting medical treatment. This form must be completed and forwarded whenever the member receives medical treatment. C. DUTY STATUS: When medical care is required, the treating physician will determine on or off: duty status and should complete a Medical Treatment Certificate used by the treating physician. Members who obviously cannot perform their duties shall be placed off duty by the officer, subject to confirmation by the treating physician. When member returns from first care treatment, contact Medical Liaison Unit and give the following information: Doctor's diagnosis. Were x-rays taken and copies obtained? Follow-up care if needed. If follow-up care is needed: Members are reminded that they must remain near their telephone until contacted by Medical Liaison Unit with information concerning their Workers' Compensation doctor's appointment. D. NOTIFICATIONS: 1. Medical Liaison Unit, when member is placed off duty, including recurrence or supplemental information. E. REPORTS & RECORDS - IOD LEAVES 3/5 MEDICAL 12 1. Make journal entry of the facts of any IOD injury. If a Workers' Compensation number is issued, enter it in the margin of the station journal. 2. Prepare and forward F-166A within 24 hours of time first informed of member going off duty or receiving first care and continuing duty. When available, attach Medical Treatment Certificate (F-1000) and "Employee's Claim for Workers' Compensation Benefits" (State DWC Form 1). If new facts are discovered or if there is a significant change in the original diagnosis, submit a supplementary F-166A and notify Medical Liaison Unit. If a member originally continues duty and an F-166A is forwarded, and then subsequent to that, the member is placed off-duty, a supplemental F-166A is required. If a member suffers a recurrence of the original injury and is placed off-duty, forward a new F-166A. To receive treatment at City expense for IOD, a F-166A must be forwarded within thirty days after the occurrence. 3. Make appropriate entries in member's personal record book. 4. Duty certificate (PDAS F-165) is required for all IOD illness or injury return to duty. Uniform Personnel: PDAS F-165, Duty Certificate, is initiated by the on-duty officer at the member's place of assignment. A release form, signed by their Workers' Compensation Treating Physician, must accompany the PDAS F-165 when forwarded. NOTE If a supervisor questions the ability of a member to perform their duties upon a return to duty by the member's Workers' Compensation Doctor, Medical Liaison Unit shall be contacted for guidance. Example: Member returns to duty with stitches still in hand. 5. Official return to duty: Members shall promptly notify the on-duty officer at their place of assignment of the date and time they will be available for duty. a. For Platoon Duty: 2000 hours following notification by 1800 hours. b. For Special Duty: The nearest whole hour to the time member is released to duty by their Workers' Compensation doctor. -75. PROCEDURES - NIOD - UNIFORM PERSONNEL.01 GENERAL This section outlines NIOD procedures for uniform personnel..11 MEMBER A. NOTIFICATION OF NIOD: Members on or off duty who become disabled NIOD shall notify the commanding officer on duty at their place of assignment as soon as possible. While off duty, such notification need not be made between the hours of 2300 and 0630 but shall be made before the time members are scheduled for duty. Provide information to complete F-166B if possible. NOTE This notification shall include any conflict in anticipated leaves and scheduled overtime duty days. B. SECURING MEDICAL CARE FOR DISABLING NIOD: 1. While on duty: In emergencies, officers shall arrange for the necessary care at the member's own expense. In all other cases, member shall arrange for treatment by their private physician. 2. While off duty: a. Injury: In all cases secure private medical treatment. 3/5 MEDICAL 13 b. Illness: When member anticipates being disabled for more than three consecutive days, or whenever member is disabled more than three consecutive days, member shall secure private medical treatment. The Department may require the member to secure services from and release by a private physician for any NIOD disability. EXAMPLE 1. Member's notification is given between 0800 hours, December 1, 1992, and 0800 hours, December 2, 1992, and the third consecutive calendar day ends at 2000 hours on December 4, 1992. EXAMPLE 2. Member's notification is given at 0630 hours, December 1, 1992. The first day begins at 0800 hours on December 1, 1992, and the third consecutive calendar day ends at 2000 hours on December 3, 1992. C. RETURNING TO DUTY: 1. Members off duty shall, when they become available for duty, promptly notify the commanding officer on duty at their place of assignment. Official return to duty shall be: a. For platoon duty personnel, at 2000 hours following notification by 1800 hours. b. For special duty personnel, the nearest whole hour to the time of reporting for duty in uniform, or to the time of release to duty by Central Receiving Clinic when such release is required. 2. Members who have been under the care of a private physician shall obtain a medical release stating: a. Proof of illness or disability, including work restrictions from the employee's treating physician. A supervisor may request an assessment by the employee's treating physician or the Occupational Health and Safety Division (OHSD), as to the employee's ability to return to work and whether the employee faces any functional restrictions as a result of the illness, injury or medical condition, and what those restrictions or limitations are. b. Date of contact with physician. c. Date and time available for duty. d. Physician's signature, address and phone number. 3. Members who anticipate being off duty over 30 days shall: a. Call Central Receiving Hospital to ensure that a specialist in the field of their injury or illness will be available to examine them. b. Bring copies of medical reports and X-Rays. c. Bring medical release from private physician containing information listing in "2". D. REPORTS & RECORDS - NIOD LEAVES: 1. NIOD Illness a. Three consecutive calendar days or less: PDAS F-165 initiated by on-duty officer. Private medical treatment certificate or (F-1001) not required but may be required at Commanding Officer's discretion. b. Four through 30 consecutive calendar days: PDAS F-165 initiated by on-duty officer. Private medical treatment certificate or (F-1001) required. c. Thirty-one or more consecutive calendar days: PDAS F-165 initiated by Central Receiving Clinic. Private medical treatment certificate or (F-1001) required. PDAS F-165 taken to place of assignment by the member, where form is completed and forwarded by the officer on duty. 2. NIOD Injury 3/5 MEDICAL 14 a. One through 30 consecutive calendar days: PDAS F-165 initiated by on-duty officer. Private medical treatment certificate or (F-1001) required. b. Thirty-one or more consecutive calendar days: PDAS F-165 initiated by Central Receiving Clinic. Private medical treatment certificate (F-1001) required. PDAS F-165 taken to place of assignment by the member, where form is completed and forwarded by the officer on duty. E. MEMBERS REPORTING SICK ON ASSIGN HIRE AND SOD DAYS: 1. Assign Hire Day - Uniform A member who reports in sick on an assign hire day shall go to a physician that day for treatment and secure a medical treatment certificate or (F-1001) signed by the physician, including date of treatment and a statement regarding proof of illness or disability, including work restrictions from the employee's treating physician. A supervisor may request an assessment by the employee's treating physician or the Occupational Health and Safety Division (OHSD), as to the employee's ability to return to work and whether the employee faces any functional restrictions as a result of the illness, injury or medical condition, and what those restrictions or limitations are. Upon return to duty, attach the physician's certificate and forward it THROUGH CHANNELS to the Medical Liaison Unit. In all cases, the following statement shall be included in the body of the F-166B: "Sick on Assign Hire Day. Hired: (date and time) for (date of Assign Hire Day); reported sick (date and time)." 2. SOD Day The Commanding Officer on duty at the member's place of assignment may require the member reporting sick to go to a physician that day for treatment and secure a medical treatment certificate or (F-1001) signed by the physician, including date of treatment and a statement regarding proof of illness or disability, including work restrictions from the employee's treating physician. A supervisor may request an assessment by the employee's treating physician or the Occupational Health and Safety Division (OHSD), as to the employee's ability to return to work and whether the employee faces any functional restrictions as a result of the illness, injury or medical condition, and what those restrictions or limitations are. In all cases, when a member reports in sick on a SOD day he shall include the statement, "Sick on SOD Day," in the body of the F-166B. It shall be the responsibility of the Battalion Commander to ensure compliance to the aforementioned procedures and, if indicated, to make a visit or arrange for a visit by another Chief Officer. Following sick visitations Chief Officers shall forward an F-225 THROUGH CHANNELS to the Bureau Commander that reflects the results of the visit..21 OFFICER ON DUTY - NIOD An officer on duty shall: A. WHEN NOTIFIED OF DISABLING NIOD OCCURRING ON DUTY: 1. If urgent medical care is needed, arrange transportation to the nearest qualified emergency facility or to a reasonably located facility of the member's choice, or 2. If urgent medical care is not needed, member may use private transportation. B. NOTIFICATIONS WHEN A MEMBER IS DISABLED NIOD: 1. If member is hospitalized, call Medical Liaison Unit (Ext. 6151), when member is placed off duty and also when the member returns to duty. Information required: a. Off-duty member's name and rank. b. Make appropriate entries in member's personal record book. 4. Prepare and forward PDAS F-165 if applicable. -90. ILLNESS OR INJURY - IOD CIVILIAN PERSONNEL.01 GENERAL Supervisor A. If the illness or injury arises out of the course of employment, the immediate supervisor shall: 1. In the event medical attention is needed, contact Personnel Department Workers' Compensation Division (213) (847-9405) for designation of a treating facility. 2. Make proper notes or records of: date, time, nature of illness or injury, and circumstances. 3. Forward F-166A to Personnel Department Workers' Compensation Division for all IOD illnesses or injuries within 24 hours if possible. 4. Forward PDAS F-165 when a civilian employee is returned to duty. The official return to duty time will be the nearest whole hour to the time the employee reports to work. 5. When a supervisor is notified that an employee has suffered or may have suffered a work related injury or illness, the supervisor shall provide the injured employee, within 24 hours or one business day, a claim form (State DWC Form 1). This form must be completed and forwarded whenever the member receives medical treatment. Refer to Vol. 5, 9/7-00.00 instructions for F-166A, PDAS F-165, and State DWC Form 1. Employee A. Civilian employees who are unable to report to work because of illness or injury which is believed to have arisen out of the course of employment shall: 1. Call their immediate supervisor at place of assignment on the first day off, not later than the time scheduled to report for work, and give an estimate of time required to be off duty. 2. Provide information necessary for completion of F-166A by the immediate supervisor. 3. All IOD injuries, whether medical treatment is needed or not, shall be reported to the immediate supervisor. 3/5 MEDICAL 16 -95. ILLNESS OR INJURY - NIOD CIVILIAN PERSONNEL.01 GENERAL Supervisor A. No payment, however, for sick leave allowance in excess of three consecutive working days shall be made until a doctor's certificate or other suitable and satisfactory proof showing the fact of the illness and the necessity for the absence, together with such other satisfactory proof of the probity of the claim as may be required has been received, accepted and approved by his appointing authority and reported to the Controller. Nothing in this subsection shall prevent the appointing authority at his discretion from requiring a doctor's certificate or proof of illness at any time.
