Tonsillitis: Causes and Pathophysiology
- Tonsillitis is the inflammation of the tonsils, typically caused by viral or bacterial infections.
- It's common, especially in children, and often presents with sore throat, fever, and difficulty swallowing.
Viral Tonsillitis
- Most common causes: Rhinovirus (common cold), Influenza, Hepatitis A, HIV, Herpes Simplex Virus (HSV), Epstein-Barr Virus (EBV), Coronavirus, and Adenovirus.
- Generally less severe than bacterial tonsillitis and often resolves without treatment.
Bacterial Tonsillitis
- Most common cause: Group A Streptococcus ( Streptococcus pyogenes).
- Accounts for 15-30% of cases, often causing more severe symptoms than viral tonsillitis.
Pathophysiological Mechanism - Viral Infection
- Viruses infect tonsil epithelial cells, leading to cell proliferation.
- The immune response activates T and B lymphocytes, releasing cytokines.
- This results in inflammation, swelling, and redness of the tonsils.
- Lymphoid hyperplasia (enlargement of lymphoid tissue) traps pathogens, causing swelling.
- Infected cells and immune cells create an exudate, visible as white spots or pus on the tonsils.
Pathophysiological Mechanism - Bacterial Infection
- Streptococcus pyogenes produces exotoxins (e.g., streptolysin O and S).
- These exotoxins damage epithelial cells, leading to tissue necrosis.
- The immune response to the toxins further recruits neutrophils, causing purulent exudate (pus) on the tonsils.
- Swelling and inflammation of the throat and difficulty swallowing result from these immune reactions.
