    Gallbladder Anatomy

    • Size: 20-45cc
    • Origin: Ventral bud (hepatic diverticulum) of the foregut.
    • Independent bud development.
    • Ampulla (dilated portion): Present in 5% of cases.
    • Distal CBD (common bile duct): Sporadic longitudinal weak muscles, not a cause of pain.
    • Sphincters of CBD, pancreatic duct, and common channel exist; present in 80% of cases.
    • Duct relationships: Cystic and common hepatic ducts run parallel.
    • Blood supply: Exclusively arterial (various arteries detailed). Importantly, avoid injury to the thin fibrous capsule of the head of the pancreas during mobilization of the duodenum. Submucosa is absent in the gallbladder.

    Gallbladder Physiology

    • Nutrient absorption: Long-chain fatty acids (LCFAs) are the only nutrients that bypass the liver's first-pass metabolism immediately, absorbed by chylomicrons.
    • Bile components: Sludge (cholesterol crystals, calcium bilirubinate, mucin gel matrix).

    Bile Duct Injury

    • Healing: Collateral blood flow begins approximately 10 hours after injury and stabilizes around 4 days.
    • Obstruction effects: Obstruction leads to decreased portal venous supply. Bile duct ligation with hepatic artery ligation can cause necrosis, but hepatic artery ligation alone doesn't.

    Biliary Pain

    • Parts: Fundus, body, infundibulum, neck, cystic duct.
    • Infundibulum detail: Hartman's pouch is the neck emergence point.
    • Duct pathways: Cystic duct drains to the right side, and is parallel to the common hepatic duct.
    • Common abnormalities: Phrygian cap (4-10% cases) — no treatment necessary.
    • Key structures: Cystic artery (important to divide first when necessary), cystic node (sentinel node is significant), cystic and common bile duct lengths (4 cm for cystic duct, 8 cm for the common bile duct, etc.)

    Chronic Cholecystitis

    • Acute-on-chronic: 5.5 OR
    • Symptom duration: > 9 days (5.3 OR)
    • Liver disease history: 8 OR
    • Gallbladder neck features: Necrosis or obstruction in the neck is clinically relevant.
