    Study Notes

    Personnel Accountability System

    • The Passport Accountability System is used to ensure safety and accountability during operations at emergency incidents and hazardous training exercises.
    • It is used to identify individual team members, account for the assignment of teams and units, and comply with OSHA regulation 29 CFR 1910.134 and NFPA standards 1500 and 1561.

    Key Components

    • Command Status Board: a tool used by the Incident Commander or Section Chiefs to identify units on an incident and their assignments.
    • Control Officer: responsible for maintaining a record of personnel entering and exiting a dangerous environment.
    • Control Point: a location where a Control Officer is physically located to gather and record information on all personnel entering and exiting.

    Emergency Incident and Perimeter

    • Emergency Incident: any situation to which the fire department responds and delivers emergency services, including fire suppression, rescue, medical treatment, and hazard control and mitigation.
    • Emergency Incident Perimeter: an area where the public is not allowed access during operations for safety reasons.

    Hot Zone and Make-up Kit

    • Hot Zone: an area within the Emergency Incident Perimeter requiring self-contained breathing apparatus (SCBA), protective clothing, or where a firefighter is at risk of becoming missing, lost, trapped, or injured.
    • Make-up Kit: a kit used to expand the Passport Accountability System at complex incidents or to provide immediate replacement for lost or damaged passport hardware.

    PAR and Roll Call

    • PAR (Personnel Accountability Report): a means by which Command quickly verifies that all personnel are accounted for.
    • Roll Call: a means by which the Control Officer(s) quickly verify that all personnel have evacuated a dangerous environment when an evacuation order is announced.

    Procedure for Control

    • The Incident Commander (IC) maintains an accurate tracking and awareness of where units and individuals are committed using the Command Status Board.
    • Division/Group Supervisors use the Passport Accountability System to account for all personnel within their direct span of control.
    • All Supervisors, Unit Officers, and Team Leaders supervise and command their subordinates to comply with the Passport Accountability System.

Maintenance of Helmet Shields and Passports

    • At the beginning of each shift, or when a change of personnel occurs, or when personnel report directly to an incident, the following procedure is followed:
      • Each member places and maintains their helmet shield on their helmet.
      • Each member places and maintains their name tags on the correct primary and back-up passports.
    • Unit Officers and/or Team Leaders are responsible for ensuring the maintenance and proper placement of helmet shields and passports during the entire tour of duty, at emergency incidents, and during hazardous training exercises.

    Passport Hardware

    • The Passport Accountability System consists of removable helmet shields, primary passports, back-up passports, reserve passports, name tags, command status boards, and portable status boards.
    • Each unit or team has a reflective helmet shield for each assigned on-duty member.
    • Passports are plastic tags that consist of three sections: Unit, Team, or Staff designation, name tags, and a blank space for recording information.
    • Passport color codes: white (primary), yellow (back-up), and green (reserve).

    Implementation

    • Implementation of the Passport Accountability System is the responsibility of all personnel.
    • Unit Officers refer to the IC for guidance on where to turn in passports.
    • Team members maintain continued contact with each other by at least one of the following ways: voice, sight, or touch.
    • If a team member is in trouble, the other member(s) of the team take appropriate steps to provide help, call for help, or go and get help.
