Week 8 Lecture November 6, 2023 1:25 PM Week 8 Page 1 - Week 8 Page 2 Week 8 Page 3 - Yale Uni, 135 teachers --to look for problematic behaviour (but no problematic behaviour in video) White teachers paying closer attention to black children "learned helplesslness":when ppl feel they have no control over life, and learn to accept their fate Why black students less likely to enter nursing profession i.e. go to school Week 8 Page 4 - Why black patients are distrusting of health care system White med students or residents - pain report and their effects of the pain prescription Bold statements are false 3 areas boxed in red are the most problematic Sample size = 92 Loses trust in health care system, and impacting health in short and long term Need to challenge our own beliefs about black people "black patients" is the more appropriate terms Week 8 Page 5 - Collecting data was a challenge? Why? - Race based data helps us give a better picture, and will prevent gaps in health and help us develop solutions - 88% RNs/students face 88% racism - 62.9% mental health moderately affected by discriminiation - 40.9% reported incidents Week 8 Page 6 - - Evening courses - if second or third career, or single mother, can take in evenings - Hiring practices - more black/indigenous tutors in nursing as teachers - Equity, diversity inclusion training--to inc cultural competence Week 8 Page 7 Week 8 Page 8 - Medical bias: the reduce effectives of medical equipment on non-white people and women - Bias seen in health care professionals and medical equipment - Pulse oximeters are over-estimating O2 levels - GFR - eligibility for dialysis vs not eligibility Week 8 Page 9 - Mind the gap - handbook, how diff conditions can present on diff skin tones - Ex. Pallor - compare hand to family member's to see pallor - Ex. Kawasaki's disease - presentation on white vs black skin - Advocate more BIPOC standardized pt's to practice before entering a pt setting Week 8 Page 10 - eGFR - measure of kidney function If AA, need to multiply GFR value by 1.15 BUT Is there any validity to this?? Research finding: black people have higher muscle mass, thus thinks it increases creatinine level ○ Black people have higher risk of ESKD ○ If this wasn't applied, more black people would be eligible for specialist referrals, required further treatment, dialysis etc. ○ Only stopped using this calculation a few years ago --ORN  But still included in our hospital system i.e. EPIC Week 8 Page 11 - Africa blamed for cause of monkey pox - Search tool showing that it more commonly affects black people - CDC- third most affected group is AA, 1st is white, 2nd hispanic ○ Inequity in testing and reporting of disease - Stigma - shame-based messaging - Data showed black and hispanic more effected than white people - White received only 1/3, but over 50% received vaccinations - Impt to show equity in treatment and prevention, and need to take action against stigma and break down barriers to receive treatment Week 8 Page 12 Week 8 Page 13 Week 8 Page 14 - Learning - Research - Levels of advocacy: ○ Macro ○ Meso ○ Micro - Partner Recommendations for Pillars: Week 8 Page 15 Week 8 Page 16
