Pre-term Labour Sunday, 11 August 2024 14:10 Definition Pre-term Labour is Labour (uterine contractions and cervical changes) that begin between 24 weeks and 37 weeks of gestation. It can be spontaneous or preceded by premature rapture of membranes/ iatrogenic due to maternal or fetal causes. PTL increases the risk of neonatal morbidity and mortality. Causes Infection: chorioamnionitis and sub-clinical chorioamnionitis Maternal factors: Pyrexia Uterine factors: cervical insufficiency Fetal factors: Multiples, Polyhydramnios Placental: previa, abruption placentae Impacts of PTL on Perinatal Outcomes & Long-term Health Increased Perinatal death Increased morbidity ○ Short term: Respiratory distress syndrome, Intraventricular hemorrhage, Necrotizing enterocolitis, hypothermia, Hypoglycemia, Jaundice and infections. ○ Long term: Cerebral palsy, Vison and Hearing loss. Management in ANC Hx: Risk factors History may reveal important risk factors such as: ○ Previous PTL ○ Previous 2nd trimester miscarriage ○ Smoking ○ Poor socioeconomic status ○ Low BMI (
