MICROBIOLOGY AND PARASITOLOGY IMMUNOLOGIC ASSAY FOR TYPHOID FEVER Dr. Fritz Von T. Gella, MD | Oct. 23, 2024 CLINICAL MANIFESTATIONS OF SALMONELLA SUSPECTED TYPHOID FEVER CASE MORPHOLOGY 1. Any individual who lives in or who has history of travel from tropical or subtropical areas; ENTEROBACTERIACEAE 2. Who presents with fever of more than 5 days; Gram negative bacilli fever documented at > 38ºC Non-encapsulated, Non-sporeformers 3. WITH ANY OF THE FOLLOWING SYMPTOMS: Measures 2-4um x 0.6um in size ○ Headache, diarrhea, body Motile rods with peritrichous flagella malaise/weakness, abdominal Facultative intracellular anaerobe distention/pain, gastrointestinal bleeding Pathogenic for humans and changes in orientation/confusion should be considered a suspected typhoid CLASSIFICATION fever case (Strong recommendation, High quality of evidence) Salmonella bongori → cold blooded animals → poorly pathogenic to humans LABORATORY TESTS THAT CAN BE Salmonella enterica → human infections USED TO CONFIRM THE DIAGNOSIS OF TYPHOID FEVER 3 main serovars being pathogenic to humans DIRECT DETECTION METHOD Salmonella typhimurium gastroenteritis ○ Culture and Isolation (Strong recommendation, High quality of evidence) Salmonella enteritidis gastroenteritis/bacter emia INDIRECT METHOD ○ Antibody Detection (Conditional Recommendation, High quality of Salmonella typhi with S. typhoid fever evidence) paratyphi TUBEX TYPHIDOT TYPHIRAPID CLASSIFICATION The WIDAL TEST is no longer recommended for diagnosis of typhoid fever (Strong recommendation, SALMONELLOSIS High quality of evidence) 1. THE ENTERIC FEVER GROUP ○ Causes Undulating fever, Chills, Malaise, Headache, Myalgia, Constipation and LABORATORY diarrhea, Rose spot Moderately Anemic ○ Salmonella serovar Typhi and Paratyphi Leukopenia:
