PERIOPERATIVE NURSING MODULE 1 PREPARED BY: MJ CABRALES, RN, LPT 659 Perioperative nursing- spans the entire surgical experience, consists of three phases that begin and end at particular points in the sequence of surgical experience events. Things to know during perioperative stage Technology and anesthesia Surgical classifications Preadmission testing Medications taken by the patient (beta blockers and blood thinners) Preoperative Nursing Management Preoperative care takes place from the time a client is scheduled for surgery until care is transferred to the operating suite. Assessment of risk factors is one of the major aspects of preoperative care. Preoperative care includes a thorough assessment of the client’s physical, emotional, and psychosocial status prior to surgery. NURSING CARE Informed consent Once surgery has been discussed as treatment with the client and significant other, family member, or friend, it is the responsibility of the provider to obtain consent after discussing the risks and benefits of the procedure. The nurse is not to obtain the consent for the provider in any circumstance. The nurse can clarify any information that remains unclear after the provider’s explanation of the procedure. The nurse cannot provide any new or additional information not previously given by the provider. Intraoperative Nursing Management Intraoperative Nursing Care Precautions Preparation After Care Complications Postoperative Nursing Management The postoperative period extends from the time the patient leaves the operating room (OR) until the last follow-up visit with the surgeon. This may be as short as a day or two or as long as several months. During the postoperative period, nursing care focuses on reestablishing the patient’s physiologic equilibrium, alleviating pain, preventing complications, and educating the patient about self-care. PACU ASSESSMENT Upon receiving a client from the operating suite, immediately perform a full body assessment with priority given to airway, breathing, and circulation. MONITORING AND MANAGEMENT Airway and breathing Circulation Vital Signs Positioning Response to anesthesia (sedation, nausea, vomiting) Surgical wound, incision site, dressing Pain Mentation ALDRETE SCORING Monitor recovery from anesthesia by using the Aldrete scoring system. Each of five factors is given a score based upon observations of the client. The scores are totaled to determine the Aldrete score. UNIT ASSESSMENT Upon receiving the client from the PACU, immediately perform a full body assessment with priority given to airway, breathing, and circulation.
