We will define what is medical nutrition therapy and its relevance to clinical nutrition and dietetic practice. During the course of the module, we will discuss the role of the nutritionist-dietitian in the delivery of medical nutrition therapy (MNT) in various disease conditions. We will explore how patient-centered care is applied in MNT, in a collaborative multi-disciplinary team working. We will also start to define the different dietary modifications that we will be referring to when planning diet management of different disease conditions e.g. Type 1 Diabetes Mellit The main textbook is Mahan , LK and Raymond, JL, 2017. Nutrition diagnosis (e.g. inconsistent carbohydrate intake) should not be confused with medical diagnosis (e.g. diabetes), which could be defined as a disease or pathology of specific organs or body systems that can be treated or prevented.1 2 Registered Nutritionist-Dietitians use nutrition assessment data to identify, ascertain and describe the nutrition diagnosis to provide a link to setting realistic, attainable, and measurable outcome, selecting appropriate intervention, and tracking progress in attaining those expected outcomes. It describes the problem, its root cause, and the assessment data that provides the evidence for the nutrition diagnosis. What is the difference between a medical diagnosis and nutrition diagnosis? STEP 3: NUTRITION INTERVENTION 1,3,6,8 Nutrition intervention is “ purposefully planned actions designed with the intent of changing a nutrition-related behavior, risk factor, environmental condition, or aspect of health status.” It is directed towards resolving and improving the identified nutrition diagnosis(es) or nutrition problems by planning and implementing appropriate intervention to eliminate nutrition etiology and/or relieving the signs and symptoms of the nutrition problem. 1 Nutrition interventions are selected and tailored to the needs of the individual patient/client, group, community, or population. The nutrition diagnosis and its etiology determines the selection of a nutrition intervention for the purpose of changing/modifying nutrient intake, nutrition-related knowledge or behavior, access to supportive care and services or environmental conditions, organizational process, and public policies that impact the nutritional problems.2 Nutrition intervention goals provide the basis for monitoring progress and measuring outcomes. It is accomplished in two distinct and interrelated steps. Delivery Meals and snacks, supplements, enteral and parenteral nutrition, feeding assistance, feeding environment, nutrition-related medication management Nutrition Education A formal process to instruct a patient/client/group/community in a skill or to impart knowledge to help them voluntarily manage or modify food, nutrition and physical activity choices and behavior to maintain or improve health. e.g. diet instruction at bedside or out-patient clinic. Nutrition Counseling A supportive process characterized by a collaborative counselor-patient relationship, to establish food, nutrition, and physical activity priorities, goals, and individualized action plans that acknowledge and foster responsibility for self-care to treat an existing condition and promote health. e.g use of cognitive behavior therapy (CBT) techniques and motivational interviewing (MI) Coordination of care Plan Consultation with, referral to, or coordination of nutrition care with other health care providers, institutions, or agencies that can assist in treating or managing nutrition-related problems. Discharge and transfer of nutrition care in new setting or provider. Population Base Nutrition Interventions designed to improve the nutritional well-being of a population. Action STEP 4: NUTRITION MONITORING AND INTERVENTION 1,3,6,8 Nutrition monitoring and evaluation identifies outcomes and indicators relevant to the nutrition diagnosis and nutrition intervention. It determine and measure the amount of progress made for the nutrition intervention and whether nutrition related goals/expected outcomes are being met. RND should select nutrition care indicators that will reflect a change as a result of nutrition care. The monitoring and evaluation phase should be considered during the assessment phase whilst determining the nutrition diagnosis and nutrition intervention. Additional factors to consider when monitoring and evaluating are medical diagnosis, health care outcomes goals, nutrition quality management goals, practice setting, patient/population disease state and/or severity.1,2 Study Question: 1. The following patient information is gathered by the RND during the consultation process: Anthropometric measurements Diet history Knowledge and skills Medical history Medical and biochemical tests Psycho social history Assessment against standards Physical indicators of disease The physical indicators of the disease is usually related to some nutritional inadequacies and excesses that can affect the different parts of the boy/systems. Dietary modification are changes made during food preparation, processing, and consumption to increase or decrease nutrient content of food for the purpose of correcting/improving nutritional status. A modified diet is any diet altered to include or exclude certain components of a diet, such as calories, fat, protein, carbohydrates, vitamins, minerals, fiber. Diets are modified for therapeutic reasons, for example for the treatment of high blood pressure, diabetes mellitus, dysphagia, vitamins and mineral deficiencies. Therapeutic diets are prescribed to patients as part of the nutrition intervention plan. Standard or regular diet is a diet that includes all foods and meet the nutrient needs of a healthy individual whilst modified or therapeutic is a diet that is altered by changing food consistency or nutrient content, by including or eliminating specific foods, by increasing or decreasing the energy value of the diet, by changing the flavour (e.g. bland), or modify the interval of feeding.
