General Operations - 45 Published with permission by Hanover Fire EMS Procedure Hanover Fire EMS 300 Procedures-Guidelines Manual Apparatus Minimum Staffing (SOG) 300.1 PURPOSE The purpose of this policy is to establish the minimum staffing requirements of personnel on apparatus responding to an emergency incident. 300.2 SCOPE This policy regulates the daily, routine staffing of our apparatus by career/volunteer personnel. The Fire-EMS Chief, Assistant Chief of Operations or designee may suspend the provisions of this procedure whenever special circumstances warrant. Establish policy that are intended to provide the community with the highest quality combination service possible. Minimize the health and safety risk of personnel by assembling an effective response force at every emergency incident to bring the incident to a safe and satisfactory conclusion. 300.3 PREREQUISITES Personnel must have knowledge and understanding of the department’s tactical guidelines and qualified personnel to operate in a capacity required in such positions on responding apparatus. 300.4 RESPONSIBILITIES It shall be the responsibility of all Hanover County Fire-EMS personnel to ensure minimum staffing on apparatus and qualified personnel filling such positions are maintained in accordance with policy. 300.5 PROCEDURE (a) To ensure compliance with this policy, the company officers/acting officers for each company shall coordinate all staffing for their respective crews. (b) Company officers/acting officers shall be responsible for maintaining staffing levels at prescribed minimums. (c) Battalion Chiefs along with company officers will oversee the responsibility of ensuring the qualified effective response force of personnel operating on apparatus is maintained. Staffing levels for apparatus are listed below in the staffing matrix (appendix 1). (d) Minimum Staffing was established based on the following: 1. The effective response force necessary to maintain health and safety of responders, and task analysis to be performed on an emergency incident. 2. The department’s territorial range of authority as provided by the local government.Hanover Fire-EMS Department’s tactical guidelines, evolutions, apparatus deployed, and expected results to mitigate an emergency incident. (e) Qualified personnel must maintain compliance with the departments training standards and certified to fill such positions on emergency apparatus. This also requires qualified FF/EMT responding on licensed fire apparatus to ems emergencies. 300.6 REFERENCES None 300.7 DEFINITIONS Response time = Turnout time + Travel time Hanover Fire-EMS Department Response Goals: Arrive on scene of Priority 1 EMS calls in less than 9 minutes 80% of calls Arrive on scene of Priority 2 EMS calls in less than 13 minutes 80% of calls Arrive on scene of Priority 3 EMS calls in less than 15 minutes 80% of calls Arrive on scene of fire incidents within the suburban service area in less than 9 minutes 80% of calls Arrive on scene of fire incidents outside the suburban service area in less than 15 minutes 80% of calls Travel Time - Begins: When unit(s) is (are) responding to an incident scene and ends when unit(s) arrive(s) at an incident scene. Turnout/Enroute Time - Begins: When unit(s) and personnel have been alerted/dispatched and ends when unit(s) is (are) responding after personnel have donned gear and seatbelts and communicated to ECC via radio. Turnout time for primary on shifted units (Engine, Truck, Rescue, Ambulance/Medic, and QRV) at Stations 1, 3, 4, 6, 7, 8, 10, 13, 14, 15, and 16 as sixty (60) seconds during daytime hours 7am-7pm, and ninety (90) seconds during night time hours 7pm-7am. Turnout time for primary on shifted units (Engine) staffed by volunteer personnel at Stations 2, 5, 9, 11, and 12 as five (5) minutes before replacement or units may be replaced at the discretion of the battalion chief based on system resources, call criteria, or overall system status management. When career personnel are staffing primary on shifted units at Stations 2, 5, 9, 11, and 12, the turnout time will remain as sixty (60) seconds during daytime hours 7am-7pm, and ninety (90) seconds during night time hours 7pm-7am. Apparatus Minimum Staffing (SOG) - 48 Published with permission by Hanover Fire EMS Procedure Hanover Fire EMS 301 Procedures-Guidelines Manual Incident Command (SOG) 301.1 PURPOSE Provide for the safety of members operating at emergency incidents through improved command and control. Improve the use of resources and tactical effectiveness. Meet the OSHA/EPA regulations requiring the use of an Incident Command System for hazardous material incidents. Meet NFPA 1500 requirements for the use of an Incident Command System for operations at all emergency incidents. 301.2 SCOPE To meet these goals the department shall implement the Incident Command System appropriately at all incidents for which it has management responsibility. 301.3 PREREQUISITES Qualified personnel must meet and adhere to the officer and acting officer standards and qualifications as outlined within the Department’s SOGs. 301.4 RESPONSIBILITIES It is the responsibility of Command to implement an organizational structure, using standard operating guidelines (SOGs) and standard operating procedures (SOPs) to effectively manage fireground/rescue operations. The implementation of the organizational structure should begin with initial tactical control measures and may grow to include a number of levels depending on the size and complexity of the situation. The ideal organizational structure expands at a pace and in a direction that stays ahead of or even with the tactical deployment of companies. 301.5 PROCEDURE (a) Communications 1. All communications shall be in plain English. (b) Assumption of Command 1. Command shall be established at all incidents. 2. The ranking member of the first arriving engine company or the first arriving officer shall assume command. (c) Declaration of Operational Mode 1. Specifics can be found in the Tactical Guidelines and in the Operations Guide. 301.7 DEFINITIONS None 301.8 QUALITY AND ACCOUNTABILITY All Chief Officers, Company Officers, and Acting Officers will conduct appropriate risk benefit analysis at working incidents and coordinate tasks and tactical objectives according to the Department’s Strategies and Tactics Guidelines while using a command structure that safely manages all personnel and risks. The main goal of any incident command system is to create an organizational framework that can safely mitigate an incident based upon strategic objectives and incident priorities while maintaining safe operations within appropriate and reasonable span of control relationships. Incident Command (SOG) - 50 Published with permission by Hanover Fire EMS Procedure Hanover Fire EMS 302 Procedures-Guidelines Manual Tactical Priorities (SOG) 302.1 PURPOSE The operations tactical guide is intended to provide a consistent approach to tactical priorities on the fire ground. 302.2 SCOPE The operations tactical guide is a guide to aid training and provide a consistent thought process for operating on the fire ground. It is important to realize that this is not a "cookbook" that applies to all incidents. All fire grounds present unique challenges. 302.3 PREREQUISITES All Company Officers, Acting Officers, and firefighters should be intimately familiar with the tactical guidelines. 302.4 RESPONSIBILITIES Officers are required to think and act in a manner that is consistent with the situation they encounter and should not rely on the guide as an answer-all solution for every incident. They are expected to take aggressive action to mitigate the emergency while leveraging all available research and tactics to ensure firefighter and citizen survival. 302.5 PROCEDURE (a) Incident priorities are as follows, in order or priority: 1. Life Safety 2. Incident Stabilization 3. Company officers should understand all company assignments and how each unit works within the large fire ground strategies. All personnel should know the guidelines well enough, so that when they have to deviate from the guidelines, they realize the impact their actions will have on other operating companies. Tactical Priorities (SOG) - 52 Published with permission by Hanover Fire EMS Procedure Hanover Fire EMS 303 Procedures-Guidelines Manual Structural Fire Tactical Guidelines (SOG) 303.1 PURPOSE The purpose of this guideline is to provide a consistent approach to firefighting strategies and tactics. These guidelines are written to provide fire officers the ability to adjust tactics to a specific emergency. These guidelines are in no way intended the replace one of the most important assets on the fireground, the thinking fire officer. These guidelines are written to provide a standardized vision of strategies that will be used on the fireground. Personnel are allowed to deviate from the guidelines when conditions or situations warrant and should immediately notify the Incident Commander of their actions. 303.2 SCOPE All personnel should be familiar with the tactical guidelines. Company officers should understand all company assignments and how each unit works within the large fireground strategies. 303.3 BACKGROUND These guidelines are the result of tremendous planning by the chief officers and company officers throughout the department. Some sections are subject to regional tactical efforts through the Central Virginia Fire Chiefs Association 303.4 SINGLE FAMILY DWELLING STRUCTURE FIRES Dispatch Assignment: Structure Box Alarm: (4) Engines, (1) Truck, (1) Ambulance/Medic, (1) Safety Officer, (1) Battalion Chief (ERF 19 personnel) Upgrade-Residential/Working Fire: (1) Heavy Rescue, (1) Ambulance/Medic, (1) Air/Light Unit, FE Administration. Non- hydrant area will receive (1) Engine, (3) Large Capacity Tankers (Total ERF, 12 personnel) Effective Response Force (31 personnel) First Engine Identify and communicate primary water source as soon as possible while responding. Crews with 45 minute high pressure SCBA should turn around and exit when 2000 psi left in a crew member’s bottle.If any crew member reaches 50% of air capacity, the crew should exit the building. If moderate to heavy smoke is showing at the ceiling, and hose line crews have not reached the seat of the fire – Consider evacuating the building and initiate a defensive mode operation.This smoke is fuel, and can quickly ignite.We cannot generate sufficient gpm from hoselines to overcome fuel burning in the ceiling area over the entire length of a “Big Box” building. If heat and smoke conditions are getting worse during an offensive attack, consider evacuating the building and initiating a defensive mode operation.Keep in mind the lead time it takes to put heavy fire streams in operation. High rise structures typically have multiple standpipes on each floor. Connect to a separate standpipe from the attack handline when available DPO remains with apparatus until requested or assist other DPO’s Tactical Considerations: Supply 1st arriving ladder truck or 2nd engine Rapid Intervention Crew – One floor below fire floor o If the fire floor is below grade, The RIC crew shall position outside of the IDLH in close proximity to fire floor.This may be one floor above fire floor but not below. o RIC crew performs proactive measures to increase the safety of crew members operating inside IDLH environment and prepare necessary equipment such as tools for forcible entry, RIC Pack, and spare SCBA bottles while maintaining readiness if activated by Incident Commander o RIC should receive face to face strategic brief from incident commander and o RIC should maintain an immediate readiness and remain a fresh crew throughout the incident. On-deck rotations of RIC will only take place when deemed necessary by Incident Commander. Those rotations should be fresh personnel o RIC crews should remain in place until the incident has been placed under control or deemed no longer necessary by Incident Commander Fifth Engine Secure the 3rd Engine’s water supply if needed. Arriving units need to be aware of the locations of standpipe and sprinklers systems or even if they exist in the structure.Numerous factors will determine when to attack the fire and what tactics will be used in meeting this objective. It is vital that situations encountered are communicated to the Incident Commander Note: opening a door or stairwell during the initial attack might release products of combustion into protected areas being used for evacuation. 303.8 INCIDENT SIZE UP It is critical that the company officer communicate a concise size up of every situation. This information sets the tone for the incident and prompts ECC to ensure the appropriate resources allocated are based on the incident type. The Incident Commander should complete a (360 Survey) of the complete structure or incident. A typical size up should include the following: Unit Number Conditions (Verify situation or declare a “Working Fire” and specify the type) Building construction type Establish Command Declare operational mode Situation Type Definition High Life Hazard Any location that poses multiple life threats due to difficulty exiting or lack of mobility of the inhabitants: (Churches, Hospitals, Hotels, Nursing Homes, Schools) Multi-Family (High Life Hazard) A building that can or does contain a large number of people. An apartment complex is an example, as long as people can easily exit. High Rise Procedure Procedure used in a building where total evacuation is not practical, ground based operation is not possible or when resources are stretched beyond the capacity of one jurisdiction. Commercial-Big Box Structure A structure in which the primary purpose is to conduct activities of business, industry, or trade. Residential Fire A building occupied by a single family. This mode is necessary to accomplish extinguishment using one of two accepted concepts. The 750/Fast concept is a Fire Attack Mode concept to rapidly deliver a water stream to the seat of the fire. The SLICE-RS concept is a Fire Attack Mode concept where personnel will “Reset” the fire from the exterior to reduce temperatures inside before an interior fire attack begins. Rescue Mode: Command option when there is entrapment of occupants or firefighters. This should also be considered on non-fire related emergencies.
