Job Safety Analysis (JSA) Is a systematic approach to identify, analyze and record the steps involved in performing a Job task. It is a method of prevention for identifying and controlling or eliminating the hazards associated with the job task, It focuses on the relationship between the worker, task, tools, and work environment, JSA is a technique that focuses on a job tasks to identify hazards before they occur. Control Measure A control measure is any measure taken to eliminate or reduce the risk by way of engineering, substitution, elimination, behavioral, administrative or reduction of risk through personal protective equipment. ALARP The residual risk shall be As Low As Reasonably Practicable. Consequence A result or effect, typically one that is unwelcome or unpleasant after an event or action has taken place. 5 5 Classification: Tasnee - General Information Risk A Combination of the probability of occurrence of an adverse effect on people, environment, assets, profits, quality or reputation of the Company and the consequences of that effect. Reduced Risk Risk remaining after the proposed mitigation. Residual Risk Risk remaining after additional control measures required. Task Supervisor It is the person that is responsible for supervising/overseeing the actual Task execution (i.e. Operation Shift Supervisor, Maintenance Supervisor, Site Project Engineer / Assigned responsible person etc.). Risk Acceptance Decision to accept a risk as per the limits of authority as described in the method section below. Company Risk Matrix (EHSS & Business) Is a document that utilizes consequence and probability to determine the risk score.6 6 Classification: Tasnee - General Information Superintendent/Section Head (Area Owner) ❑ Area owner superintendent Shall accept and approve final JSA. Task Supervisor ❑ Shall initiate the JSA through the electronic system eRM. ❑ Shall ensure the completion of the JSA. ❑ Shall ensure JSA is completed by cross functional teams as per the method. ❑ Shall ensure that JSA briefing is carried out before and after taking the signature specially for the employees (example labor / Forman / craftsman/ welder) who did not attend JSA meeting. ❑ Shall ensure to upload records of JSA into electronic system. Task Specialist /Engineer ❑ Shall review Task Steps, Description of Hazard, Control Measures Required Responsible Person, Signature after Implementation in JSA form. Area Owner Specialist /Engineer ❑ Shall review JSA. 7 7 Classification: Tasnee - General Information Permit Issuer ❑ Shall ensure the controls identified and required from the JSA are implemented to ensure the Work Area is safe. PTW Receiver ❑ Shall ensure that the required control measures from the JSA are implemented and mitigation measures are signed off. ❑ Shall ensure that personnel carrying out the Task are oriented in the associated hazards. ❑ Shall ensure the attendance sheet on the JSA form is signed by all craftsmen and contractor carrying out the task. ❑ Shall ensure JSA is reviewed and approved. 8 8 Classification: Tasnee - General Information ❑ Controlling and or eliminating the hazard associated with the task identification is mandatory for all different type of work. ❑ HAZARD Recognition is mandatory for all type of work permit activities. Risk Control Strategies (Hierarchy) ❑ Elimination of the hazard. ❑ Substitution of the hazard with less hazardous equipment, materials and/or processes etc. ❑ Engineering controls (E.g. Guarding, active/passive fire protection, shutoff-valves, etc.). ❑ Behavioral controls (E.g. Reliance on awareness of hazards and personal judgment regarding actions to reduce the associated risks). ❑ Administrative controls (E.g. Procedures, signs, emergency control plans, training, etc.). ❑ Reduction of risk through personal protective equipment. 9 9 Classification: Tasnee - General Information 10 10 Classification: Tasnee - General Information JSA is Mandatory for all critical activities including: ❑ All Confined space activities ❑ All Hot work (open flame/welding/grinding) activities inside the process/production areas ❑ All Radioactive source activities ❑ All activities on Rotating equipment ❑ All repair on lifeline ❑ All activities executed by none ARC contractors ❑ All hydro jetting/Sand blasting activities ❑ All activities using Man-lift with cage ❑ All activities require Diving 11 11 Classification: Tasnee - General Information JSA is optional based on the permit issuer decision as per the below criteria: ❑ Jobs with a potential for severe accidents/incidents, including, as appropriate, routine jobs. ❑ Catastrophic potential: fire, explosion, chemical release, toxic atmospheres, oxygen deficient atmospheres ❑ Tasks with a history of injuries, or near misses. ❑ New jobs and jobs where changes to the work method/process have been made, or when using new/modified equipment (Tasks done in new environments). ❑ Task result on incident/potential incident (task specifically identified during root cause analyses from incident investigations). ❑ Rarely performed jobs. ❑ Jobs complex enough to require written instruction. ❑ Job conducting 1st time in TPC. ❑ Big Maintenance Crew involved in the work (More than 10 employees). ❑ Job requires multi craft. ❑ Preparation for work on tank /vessel/pipelines. 12 12 Classification: Tasnee - General Information JSA is optional based on the permit issuer decision as per the below criteria: ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ ❑ Preparation for work on pumps. Preparation for work on Machinery and prime movers. Preparation for rotating equipment alignment. Removal of material for equipment preparation. Welding, cutting activities outside the production/process area. Hot work activities outside the production/process area. Excavation. Lifting operations with cranes. Electrical activities, Electrical/Cable installation through cable tray or High Voltage Cable Vault and Manhole Entry. Working at height (more than 2 M) without proper platform or guardrails. 13 13 Classification: Tasnee - General Information ❑ JSA shall be completed by cross functional teams consisting out of the following: ❑ ❑ ❑ ❑ Task Supervisor. Craftsperson. PTW Issuer. Other interested parties as applicable. ❑ Break down the task into Sequence steps: ❑ Breakdown of the task should contain enough steps to describe accurately the work is being done based upon experience and common sense. ❑ These steps shall represent the actions required to do the jobs. ❑ Steps should be as brief as possible and not too fine or too general. ❑ List down the steps in sequence as required for performing the job. ❑ Identify the hazards from activity, surroundings, tools and equipment: ❑ Identify the hazards related to fire, safety of working people and environment related issues. ❑ Repeat the observation as often as necessary until all hazards have been identified. ❑ List down the hazards identified in second column of JSA form. 14 14 Classification: Tasnee - General Information ❑ Recommend Risk Control Measures to overcome identified hazards. ❑ Review each job steps and main contributors to hazards for determining the ways to eliminate/control the hazards identified. ❑ Recommendation shall be job or procedure specific, general statements may add no value. ❑ Eliminate/Mitigate the source or cause of the hazard and potential consequences by using the following technique, Hierarchy of Control, (Highest effectiveness - Elimination and Lowest effectiveness - PPE). ❑ List down the recommendations identified in third column of JSA form. ❑ List down the responsible person for each recommendation in the fourth column of JSA. ❑ Ensure JSA recommendations are implemented during site preparation and performing the work, as applicable. ❑ Area owners should review and approve final JSA. 15 15 Classification: Tasnee - General Information 16 Classification: Tasnee - General Information 17 Classification: Tasnee - General Information JSA Preparation, Review and Approval process Initiation Task Supervisor shall initiate the JSA through the electronic system (eRM) Preparation Task Supervisor Completion Task Supervisor PTW Issuer Craftsman Others Review Quality check Task Specialist / Engineer PTW Area Owner Specialist / Engineer Approval Area Owned Manager / Superintendent or delegate Execution Craftsman and crew involved in the activity. They need to sign off in the JSA. JSA Upload Task Supervisor shall ensure to upload records of JSA into electronic system. 18 Classification: Tasnee - General Information 19 18 Classification: Tasnee - General Information What is a Job Safety Analysis? What is a Risk Assessment? 20 19 Classification: Tasnee - General Information So…What’s the Difference? While they do sound quite similar, the most important difference is that a job safety analysis looks at job-specific risks while the risk assessment looks at a bigger picture. The risk assessment will identify risks throughout the facility, and not just those that may directly impact an employee. For example, risk assessments would look to identify environmental hazards, and put processes in place to mitigate that risk, while a job safety analysis would not. The job safety analysis looks at individual tasks, which often allows the safety professional to look much deeper into the potential hazards. Companies can benefit from using both of these great tools to identify potential hazards and come up with controls to keep everyone safe. In many cases, a risk assessment will be performed first to find and address higher level hazards, and then a job safety analysis will take place next to look for more granular dangers. 21 20 Classification: Tasnee - General Information Risk Assessment? ❑ There are many different styles of doing Risk Assessment and almost all of them start by asking key questions: ❑ How Bad? ❑ How Likely? ❑ Risk assessment involves making a formal judgement on the consequence and probability using: Risk = Severity(How Bad) x Likelihood ❑ How Likely? Assume something has already gone wrong and the worker is exposed to the hazard ❑ Will the exposure to the hazard result in injury? ❑ Does all mechanical systems fail ? 22 21 Classification: Tasnee - General Information 23 Classification: Tasnee - General Information Let’s try an example ?
