Common pediatric infectious diseases /Part one Common viral infections “In times of change, learners inherit the earth, while the learned find themselves beautifully equipped to deal with a world that no longer exists.” Eric Hoffer Common viral infection Incubation peroid Mood of transmission Clinical presentations Complication Management Prevention N A U L O C S M Z E K S E D Measles (Rubeola) Measles Incubation period : 8-12 days , range of (7-21) Mood of transmission : Direct droplet contact ,and airborne Clinical presentation :Fever,cough,coryza and conjunctivitis (3 C) Followed by maculopapular rash spreading cephalocaudally. Koplik spot (prodromal period) Complications: Common -Otitis media -Pneumonia (Most common cause of death from measles) -Diarrhea Rare -Acute encephalitis -Sub-acute sclerosing pan- encephalitis (SSPE): Degenerative CNS disorder, Occurs 7-11 year after measles infection Wants to know more about SSPE? Season 1 , Episode 2 Management No specific antiviral treatment Ribavirin in immunocompromised children?? Vitamin A help to modulate immune response in resource limited country but now recommended by WHO for all infected children Prevention: Vaccine B N W M C V P M G O S U Differential diagnosis of viral parotitis : EBV CMV Influenza A Parainfluenza 2 &4 Enterovirus HIV Mumps Incubation period :16 to 18 days , usually from (12 to 25 days) Mood of transmission: Droplet (usually in winter and spring) Clinical presentation Subclinical (30%) Fever, malaise, and parotitis (uni-and bilateral) Complications: Pancreatitis Arthritis Orchitis (Post pubertal) , Fertility unusual CSF pleocytosis (50%),
