General Orders GO 24-003 Candidate Firefighter Field Evaluations -Evaluation period is ONE CALENDAR YEAR following graduation -BC shall complete and submit the monthly eval form no later than the 5th day of each month -Probationary Per...
General Orders GO 24-003 Candidate Firefighter Field Evaluations -Evaluation period is ONE CALENDAR YEAR following graduation -BC shall complete and submit the monthly eval form no later than the 5th day of each month -Probationary Period is 9 months from the date of hire -After one year, when the candidate receives their completion certificate they are to report to the commissary and issued Helmet Shield Decal with company assignment, Yellow Trapezoids, Modifications made to Class D uniform. -Compay officers conduct Candidate Drill Objectives and submit the corresponding Candidate Firefighter Eval eForm on Vector Solutions prior to the End of tour of duty, Submit Form 2 to Dist Chief of Training if there are performance concerns. -For first 180 days following graduation, Candidate will not be detailed, even if its across the floor. If candidate is working a trade for someone on another company, the candidate stays on their own company and another FF takes detail to that spot. If Candidate is rehired they stay on their company. Two Candidates from the same class cannot be on same shift. GO 23-008 VOTR -All VOTRs must be submitted by 1600hrs -Select 0.0 by 1600hrs if apparatus does not leave firehouse before 1600 -Certified Vehicle Operators will receive a minimum of 0.5 hours per day -Driver Training- 2 hour max for those seeking and 1 hour max for those certified -Operational Training- 1 hour max for both seeking and certified -Under no circumstances shall lights and sirens be activated during training GO 23-007 ITR -All ITRs must be submitted by 1600hrs onto Vector Solutions -By 1700hrs BC shall verify that they were submitted on Vector Solutions -Journal serves as a back-up for documenting ITR completion GO 23-006 Confiscation of Structural FF Gear and SCBA equipment -CFD will confiscate any piece of on duty member’s equipment and SCBA equipment when a member requires transport to a hospital as a result of injury or illness at an emergency scene -Bureau of Logistics Confiscation Form (BAS 9) is completed by On-Duty Logistics representative GO 22-002 EMT and Paramedic License Requirements -Member in Region 11 Chicago EMS system must report any changes in their address to IDPH and their Resource Hospital within 72 hours of an address change -To Opt Out submit a request in March and if eligible will take effect July 1 or submit in October and if eligible will take effect February 1. Must also submit Form 2/2A -If a member has failed to maintain an EMT or EMT-P license, the member may be placed on unpaid leave for up to 20 months to obtain a license. -If youre not required to have either license and let yours lapse you must notify CFD and OPSA via Form 2/2A no later than 30 days prior to expiration GO 22-001 Requesting Certs and Access to training records from OSFM -All members must complete all OSFM certification requirements, including state exams, within ONE YEAR of completing the course. -Members shall not contact OSFM directly with any inquiries -All exam requests must be submitted at least 30 days prior to the requested exam date -All members should use their city email and not personal email when signing up -The CFD FDID# CS931 GO 20-006 Dept Vehicle Operator Certification Requirements and Procedures -Vehicle Categories -Category 1 - Engines -Category 2 - Tower Ladders -Category 3 - Aerial Towers/Trucks over 68,000lbs -Category 4 - Trucks under 68,000lbs(Squads,Snorkels,HazMat) -CFD Members are by default also certified to drive(but not necessarily operate)any vehicles in the category below them except Engines because you must be an engineer or certified to act as engineer. -Obtaining a Class B(Non-CDL)Permit - Report off duty to a secretary of state license facility and successfully complete written exam and receive a permit. -Members wanting to get Vehicle Operator Certification - complete 12 months in the field, Receive OSFM FSVO Certification, Possess a Class B(Non-CDL)permit, complete the training hours (12 hours driving 8 hours operating), Receive approval from company officer, in absence of officer BC can approve. Once hours and complete report off duty to QFA with license and permit for field test , then report voluntarily to Secretary Of State within 30 days GO 20-005 Utilization of Seat Safety Belts -Personnel riding in the patient compartment of ambulance are exempt from seatbelt requirement only while performing critical, life-saving procedures(CPR) -Report missing or defective seatbelt assemblies to BUREAU OF LOGISTICS -Any delay in repair process shall be reported through the chain of command to CFD Director of Safety GO 20-004 Discrimination/Harassment Investigation Policy & Procedures -Harassment Investigation interviews -DHR EEO Division will contact involved employees with the option to interview on-duty or off-duty. If on-duty the EEO liaison will contact manpower. Interview will be scheduled between 0830 and 0900. If member chooses On-Duty the members are to report to QFA and swipe and not their assignment. If employee chooses Off-Duty they will report directly to EEO interview location and will be paid OT on hour by hour basis GO 20-002 Social Media/Social Networking Policy -Members can express free speech as long as it does not: Interfere w/CFD operations or impairf working relationships of the Dept Impede the performance of job duties Undermine the goals and mission of the Dept Undermine the public trust or confidence in the Dept Discriminate race, ethnic heritage, sex, sexual orientation etc Harassment or Bullying -No photos or videos during emergency response or at incidents unless authorized by the Fire Commissioner. No posting photos or videos on social media -Members who post work related matters on Social media must disclose affiliation w/ CFD -Members may not make statements where member is acting or appearing to act in an official capacity or as an official representative of the CFD -Members cannot post any information or comment on ongoing investigations or lawsuits involving CFD that would jeopardize the integrity or the investigation -Members may not reproduce CFD logos, trademarks, graphics for commercial or competitive purposes -Members may not create a link from a personal social site that would direct to a Dept site G.O. 19-004 Swiping Program for Platoon, 40 Hour Sworn Members -ID card for attendance verification and to get fuel -Members carry card throughout their tour of duty. -Officers verify presence of card and ensure all members swiped and scanned. -Platoon employees only swipe one time during their tour of duty. -40 hour employees swipe in and out. If there is a discrepancy with 40 hour employees, notification will be made to the Bureau Head before the timekeeper will take any action -Off duty members reporting to Cook County courthouses for CFD matters will swipe in and out at courthouse. -Procedures- When scanning you get a score from 0 - 100. The lower the score represents the better recognition of the member’s hand. -Officers must record that all members have swiped. If a member is unable to swipe that needs to be recorded along with the reason unable to swipe. -Special Situations: -Rehires- Report to Officer. Officer witnesses swipe -Tardy- Report to Officer. Officer checks for fitness for duty. If fit for duty, Officer witnesses swipe. If the tardy member is an Officer, Officer’s immediate supervisor is required to witness the swipe. -Detail - Only swipe once. If you swiped at your house don’t need to swipe again -Trade- swipe in where you are working -12 hour trade - Only swipe in when you arrive, after approval of Officer. Officer witnesses swipe -Relief on Scene- Officers ensure members swipe before leaving to relieve companies at the scene -Training Details- Swipe in at the location if machine available. If not swipe in when reporting to your assignment. -Emergency Recall- Swipe in wherever you are ordered to report Outside Chicago- Not required to swipe. How would you? -Damaged/Inoperable Swiper -Report to Officer -Officer calls Air Mask and then Bn Chief -Record, on a daily basis, the date/time machine was discovered inoperable. The associated work order number and any work done on the machine plus outcome. -If the Handpunch 3000 is not working, under direction of the Bn Chief members will be required to swipe in at another fire station within their district -If the work order is not completed within 7 calendar days, House Captain or building supervisor shall contact Logistics/Airmask to inquire on status of repair -Chiefs document and forward info regarding non-compliance with this order through the chain of command. GO 19-001 Apparatus Refueling Procedures -Maintain fuel at no less than ½ full. During cold weather no less than ⅝ full -Companies are to try and get fuel between 0800-1600 -Companies get permission to go for fuel from BC -BC ensures companies from same areas are not refueling at the same time -If a company is unable to get fuel from the fuel truck then they refuel at a Mega Site -Chiefs refuel at Mega Fuel Sites. Fuel Truck does not fuel unleaded vehicles. -Fuel truck will be automatically dispatched to 3-11 fires -Fuel trucks out from 0800-1530 on their routes -Midway weekend hours 0630-1330. Weekdays 0630-2130 GO 18-010 Sexual Harassment Policy -CFD adopts City’ Diversity and Equal Opportunity Policy as the CFD Sexual Harassment Policy -Complaints can be made to CFD Supervisors, CFD EEO Liaison or City of Chicago Human Resources -Any complaint must be forwarded by CFD Supervisors to EEO Office or Department of Human Resources -Must also report any conduct which may constitute harassment, even if the conduct was not the subject of a formal complaint. GO 18-009 Investigation Disciplinary Procedures -This investigation procedure does not apply to conduct addressed in City of Chicago EEO Policy (Discrimination, Sexual Harassment, Retaliation, etc.) -Those complaints are forwarded by CFD Supervisors to EEO Office or CFD Internal Affairs -IR Number is still obtained from IAD or OEMC after hours (0800-1600) Procedure: 1. Verbally advise employee there will be an investigation. Also inform appropriate supervisor 2. Obtain IR number form IAD or OEMC. Write it on #903 and on top right hand of all documents submitted 3. Fill out the Form #903 - Allegations of Violations. Who, what, when, where, witnesses. Don’t list the rules broken. Form 2 if more details needed 4. Give #903 to accused. 72 hours for Statement of Facts (96 EMS). If no Statement put that on a Form 2 to serve as a Statement 5. Collect Witness Statements on Form 2/2As, preferably same day 6. Superior Officer investigates. Review Allegations, Statements, Witness Statements, etc. Form 2 with summary and conclusion as to facts. 7. Send it all in. Forward to appropriate ADFC/DC, who will review and make determination. Discipline Routing Sheet will be prepared and serve as cover page Allegations Not Sustained - #906 -If the ADFC/DC determines charges unfounded they will summarize such on a Form 2 -Discipline Officer reviews and makes a final decision. Can send it back If file is 906’ed by Discipline Officer case closed. File retained by IAD -Negative Drug Test for vehicle accidents can get immediate 906 for that IR Allegations Sustained 1. ADFC/DC determines which rules broken, puts it on a Form 2 and sends it to accused employee’s Bureau Commander; put in investigation packet 2. Copy and Dock Notice (if applicable) served to accused. He signs Receipt of Charges Form. All part of file 3. Charges are recorded in the Company Journal, signed receipt and original charges go into the file 4. ADFC/DC sends file to Discipline Officer w/recommendation for “appropriate discipline.” Discipline officer can contact IAD if prior related discipline There are 10 parts to that file already! Recommendations from Discipline Officer: -Unfounded (#906) - Case closed -Oral/Written - Schedule a Notification Review -Suspension/Termination - Send recommendation to accused’s Bureau Commander with summary and request to review recommended discipline. Notification Review -Accused notified 1 work day in advance -Accused provided with paperwork and informed of reasons for contemplated disciplinary action -Accused entitled to Union representation, it is the accused responsibility to obtain it -72 hours for rebuttal in writing to Chief who conducted Review -If rebuttal, Discipline Officer reviews. Any changes, schedule a new Notification Review. If not impose the discipline -After discipline is served, investigation is closed. -Discipline which results in suspension/termination generates a Personnel Action -Request Form; forward to Fire Commissioner to make final determination Then HR gets involved. Questions from Old Order 1. From where would you obtain an IR number? IAD or OEMC after hours (0800-1600 M-F) (This is step 2 in Discipline Procedure) 2. Where is the IR number included on all documents submitted? Top right of each page 3. When one of your members violates the Code of Professional Conduct, who do you tell? (What is the first step?) Inform the member and your supervisor that you are going to start disciplinary procedure 4. What is included in the Allegations of Violations (903)? Who what where when and witnesses (Preparing 903 is Step 3) 5. What should you do if more details need to be included with the 903? Addendum on a Form 2 or 2A 6. If accused employee wishes to submit a statement of facts how long does he have to submit it? 72 hours (96 hours for EMS) (Giving 903 to accused is Step 4) 7. If supervisor cannot serve allegations to member due to a lay up, how are allegations delivered? Discipline Officer delivers them 8. What must witnesses to allegations do? Submit a Form 2 stating what was witnessed (Collecting Witness Statements is Step 5) 9. When are witness statements collected? Whenever possible same day as allegations 10. What serves as a cover page for the investigation file? Discipline Routing Sheet (Step 6 is Chief investigates. Forwarding to District is Step 7) 11. If ADFC/District Chief decides allegations are unfounded, is that the end of the investigation? Not necessarily. Discipline Officer may advise to keep investigation open. 12. When can the District Chief immediately non-sustain allegations (906)? Negative results testing for drugs and alcohol 13. If allegations are sustained, who determines which rules were violated? District Chief 14. What is the discipline imposed by Discipline Officer based on? (2 items) Severity and past history 15. How far in advance is accused notified of Notification Review? One work day before 16. Who obtains Union Representation for the accused? Responsibility of accused 17. If accused chooses to submit a rebuttal to discipline, how long do they get to do so? 72 hours 18. What happens if due to rebuttal a change in discipline is recommended? Must have a new Notification Review 19. Who makes final determination as to discipline to be imposed when investigations result in suspension or termination? Fire Commissioner 20. What is entered into journal if employee is suspended? Date and time of suspension GO 18-004 Locker Privileges -Only major change is Captains instead of District determine if non-Platoon members can use lockers and use of eForm -House Captains ensure all lockers labeled with names, files, and assignment. If vacant, or for another purpose, mark accordingly -Enter any changes in locker assignment on the Locker Assignment eForm within 15 days of Personnel Order or retirement -Unassigned lockers are to remain vacant for detailed/relief members -BC ensures the 15 day rule is followed Access other than by member- (1) Inspection, in presence of member, regarding Department Rules and Regulations (2) Inspection, in presence of member, as part of routine inspection of quarters (3) Law Enforcement with a warrant or upon command by Law Enforcement (4) By Company Officer in case of member’s death. Done in presence of witnesses. Inventory is witnessed and recorded. GO 17-020 Protective Pouch for the SCBA Facepiece -White or silver paint marker mark last name and file number in 1” letters parallel to flap -Damaged Pouch- Notify Officer. Officer obtains work order number and documents it in journal. -Member cleans damaged pouch and submits it with a Form 2 to District -District forwards to Breathing Apparatus Service. They determine if negligence. If so replacement cost is responsibility of the member, -Lost/Stolen - Current policies GO 17-019 Face Piece for MSA SCBA -Air comes in through check valves into nosecup. Exhaled thru exhalation valves. Light pipes! -Spectacle kit- Notify BAS for work order. They deliver the kit to member. -Member takes kit to optometrist. -Member has 30 days to provide proof of payment from optometrist for installation of lenses into spectacle kit. -Submit a Form 2 with Receipt, thru Chain to District Damages -Officer notifies, BAS, gets work order. Documents -Notifies Bn Chief. Gets spare facepiece -Bn Chief has 5 spare facepieces of different sizes -Member submits damaged facepiece and Form 2 to District after cleaning. -District sends to BAS. They determine if negligence. (Don’t clean confiscated gear) -Lost/Stolen - Current Policies GO 17-018 Mandatory Use of SCBA -Transfill is 3 feet long -Hazardous Incident Teams have 7 harnesses with disabled PASS devices Bn 13 has 2 harness. Other Chiefs have 1 -Member records on Inspection Form; Officer records in Journal -Inspections before 0800 do not relieve Officer/Bn Chief from conducting/witnessing 0800 inspections -On Monday Officer connects Transfill to all units and documents the results in the Journal (Inspect every day, but document weekly) -Ensure all members (engr too) can effectively don within 1 minute. Conducted as company level until all members can reach goal; weekly thereafter. Drill is done while wearing turnout gear -Battery Change- Engines 1st Sunday; Trucks 2nd; Squads 3rd; Bn Chief 4th. Or when indicated by low battery icon Damages- -Contact BAS. Work order. Document in Journal. BAS issues spare. Mandatory On but not limited to: Still Alarms; Box Alarms; Automatic Alarms; Autos; Rubbish; Hazmat; C.O.; Gas Leak; Unknown Nature (e.g. unusual odors) (9 items) -IC determines perimeter at which facepieces must be donned -IC determines when safe to remove SCBA -Not required on roof but IC can deem it necessary -No SCBA required outside the working perimeter -If low air alarm goes off, exit after notifying supervisor After Incident: -Clean and inspect. Replace under 4050. Be sure to update SCBA Daily & After Use Form -Officer is responsible to have all SCBA units returned to apparatus GO 17-015 Equal Access to Adequate Private Facilities -Lack of adequate private facilities shall not be a justification for refusing to assign or detail a member to a specific facility -Individual shall have access to facilities consistent with the individual’s gender identity (no proof required) -If supervisor receives communication regarding issues with equal access, forward such in Form 2 to District Chief prior to end of tour of duty -Dept. will respond to member by next shift. Supervisor will document Dept.’s response in Form 2 to District Chief -Restrooms - A member does not have appropriate access if he or she is required to knock and announce presence prior to using a facility -Locker Rooms - In a shared space gender neutral locker room member shall wear appropriate clothing at all times -If a member encounters any problems with equal access they should send a Form 2 to CFD EEOC Liaison and/or EEO Office for City of Chicago GO 17-014 Accomodations for Expressing Milk -Members returning after pregnancy can request an accommodation for expression of breast milk from Medical -Members can request reasonable milk room accommodations. Make request to District Chief or EEOC Liaison -Supervisor who receives request forwards to District and Medical prior to end of tour of duty -CFD will provide break time but it should not interfere with member’s duties -CFD has to provide a space with seating, an outlet and a flat surface other than the floor -Manpower will take steps to ensure members requesting accommodations are temporarily assigned to facility with adequate space for expressing milk. -Logistics has a list. -Member who will be on a trade but needs reasonable accommodations shall submit Form 2 saying so at least 2 days in advance -Don’t touch anyone’s breast milk! GO 17-010 Kronos Workforce TeleStaff Timekeeping Pilot Program -Kronos is a stand alone software or web based browser version -TeleStaff passwords changed every 90 days or as prompted -Passwords can be rest by Manpower DDC or Bn Chief. Not by 4-DATA -It is the member’s responsibility to remain aware of any and all scheduled assignments or details -If manpower for the next day changes after 1100 hours any member affected will be notified by Bn Chief, DDC, Manpower or Timekeeper -Off Duty Members will be notified of changes by phone number on record and via city email -Discrepancy noted by Platoon Member notify thru chain of command -Discrepancy noted by 40 Hour Member, notify Time Keeper -Swipe is integral part, and ensures members are paid in timely manner -Trades - Member working shall be responsible to enter the Trade on TeleStaff 5 days prior (120 hours, 2 shift days) -Emergency Trades still have to be done on Form 2. It is member’s responsibility to ensure Emergency Trade gets approved Officers on Platoon Duty -Check in completed by 0815. Apply Driver Pay -Notify Bn Chief if change to TeleStaff required after Check-in completed (due to lay-up, detail, 12 hour trade, etc.) -By 1100 hours on current day, members should be placed in their tactical positions for NEXT DAY -No later than 1600 hours on current day, check roster for NEXT DAY for company under command. If not properly staffed, contact Bn Chief -Red or Yellow Alarm Bell symbol indicates potential manpower staffing issue -Rehires or holdovers- Contact Bn Chief when member arrives, fit for duty. Document in journal time of arrival Bn Chief on Platoon Duty -No later than 0745 hours on current day review Battalion Manpower and confer with Manpower Central regarding the manpower projections for the next day -No later than 0815 review battalion Roster to ensure all positions are filled, correct members (EMTS, Medics, Divers). If openings confer with Company Officers to correct -Out of District Detail: Talk to Manpower Central. Find out where member is going. Talk to appropriate Bn Chief to find out where to move member -Detail Coming In- Talk to Manpower Central; confer with appropriate Bn Chief; ensure member is put into proper place -No later than 1600 hours of current day, be sure NEXT DAY rosters are staffed with correct positions -Rehires- Bn Chief notifies DDC and documents in journal time of arrival -PFC- similar to Bn Chief, but nowhere near as awesome DDC Platoon- Each day detail Relief Officers and Engrs at least 1 day forward -Check entire District Roster for everyone checked in; proper staffing -Print copy of District Roster for each Bn Chief and DDC. Placed in the Chief’s glove box. -No later than 1600 hours ensure the next day is all set -Rehires- Contact Manpower Central when member arrives during Manpower Central’s Hours (0500-1300) After 1300 contact Manpower on Duty Representative -Document rehire time of arrival in journal GO 17-007 Functional Radio Signatures for On-Scene Communications -Favors function over Company assignment. NIMS -For example, Incident Commander takes “Command” -Functional radio signature stays with the role, not the member originally assigned. “Command” may change from DDC to DC to Commissioner. -When additional Fire Attack Chiefs are deployed, they will be assigned a Fire Attack Signature in numerical order (e.g. Fire Attack 2, Fire Attack 3, etc.) (This seems to contradict SOP 210, which states they shall have a floor designation in signature) -Functional Signatures are for On-Scene. Company Signatures for OEMC. -Companies dispatched with auxiliary apparatus will use auxiliary unit’s signature GO 16-008 Department Vehicle Accident Policy -Policy includes personal vehicles operated by member for official dept. business. -Tools attached at time of accident are covered. Not equipment that falls of rig with no accident. -OEMC assigns AR number. Police assign RD number. -Every accident gets BC and DDC (Accident Response Team) OFI if injuries or if ART request. Documents: 950 – Driver; 951- BC worksheet hard copy; 952- BC Narrative, on-line thru Sharepoint; 953 DDC Report, on-line; 954 Accident Witness Narratives. Also photos and other pertinent info. Classifications- A injury, B no injury -1A/B - With one other vehicle or if CFD vs CFD vehicle -2A/B - 2 or more vehicles -3A/B - Pedestrian -4A/B Fixed Object -5 - Unreported IR for previous tour supervisor if found during morning inspection; on duty supervisor if found after morning inspection -SR 1050 IL Traffic Crash Report. Law Enforcement document, no longer needs to be gathered by the ART BC Procedure: -If accident occurs, OEMC dispatches ART (BC and DDC for Fire) -If accident involves BC or DDC chief of equal or superior rank shall replace that member -When accident occurs- -Check for injuries, determine accident class, report to OEMC. -If 4B notify office if continuing in but return immediately upon completion of response. -If injuries, 42.202s -Gather all needed information. Submit signed 950 to BC. 954 completed -All forms completed and submitted by end of tour. -Make journal entries, call Fleet as needed ART (BC) -Respond non-emergency unless injuries -Ensure Ambo for injuries and/or refusals -If injuries, request OFI -Obtain AR number from OEMC -Confirm or modify accident classification -Gather facts for 951/952 Document all relevant info Get all members info. -Confirm vehicle is in safe condition. If unable to confirm call Fleet -Get RD number and law enforcement officer’s star number -Collect 950 and 954 -Prepare diagram on 951 -Complete CFD Vehicle Accident Report (952) on Sharepoint and upload all pertinent documents -If BC computer out of service complete at District. Notify DDC when forms completed -Entries in Company Journal Drugs/Alcohol Policy- -If injury -If ranking ART member determines vehicle unsafe operating condition (unless it was in park at time of accident and struck by another vehicle) -If the other vehicle’s airbags deployed or it is unsafe to drive -Behavior to suspect unfit for duty in opinion of 2 supervisors, one exempt -If there is drug/alcohol testing DDC takes member to CFD HQ and notifies OEMC of gender OFI- -Non-emergency response -Takes all sorts of pictures, including the accident report cover sheet -Must upload all photos prior to end of tour 16-008 Dept Vehicle Accident Policy (Captain Questions) -A handpump falls off a truck and goes through a car's windshield. BC & DDC On what class accident may a company continue in? 4B By what time must all required CFD Forms and documentation regarding accidents be completed and forwarded? Before shift ends Your engine is involved in an accident with a car. The civilian does not want any medical treatment. Who documents the refusal? Upon approval contact OEMC -Follow Bn Chief’s directions on how to address problem. -Obtain work orders as necessary. -Notify Bn Chief of estimated duration. GO 14-001 Rendering Department Courtesies -0700-2100 Exempt Ranks and Dignitaries -0700-2100, member on watch will ring bell if requested from Bn Chief Everyone in front of their apparatus, in Roll Call uniform, until dismissed -No need to ring the bell between 2100-0700 unless requested. -No need to ring for Exempt at their own quarters. -When higher ranking Chief is already in quarters no need to ring. If this occurs, Officer will notify Chief why bells were not rung. Recommending a professional service. Failure to report violations of rules and regulations. Substance abuse.Absence without permission. Bad driving. Driving over hose unless ordered to do so, Inattention. Neglect. Making false report. GO 13-004 Electronic Mail(Email) Policy -Department email is subject to FOIA disclosure rules -All emails sent and received are saved by the City of Chicago -”reply all” shall only be used when all parties have the authority to receive the response and the sender has the authority to directly contact -Members shall not use department email to circumvent the chain of command. Stick to policies of Form 2s -Nothing is private. The city and CFD have the right to search through accounts. -Your city email can be used in court. Labeling “confidential” or “priveleged” does not prevent the email from being produced in a lawsuit. GO 12-008 Department Radio Communications -Narrow FD Zone, not FDW -Radios are now UHF. ADMIN for IDOT is still VHF. Reach IDOT over VHF or through OEMC -Contact EMS on ADMIN -If a call is not acknowledged by OEMC within 15 seconds, try again. Do not make more than 3 attempts with the same radio. -If 3rd call not acknowledged assume the radio is inoperative. Try another radio. -If same issue try another frequency and explain situation to dispatch -1 scanner to each house. 1 to each Bn quarters -Don’t use preliminary calls to establish contact when responding or on location -Don’t volunteer for assignments unless so directed by OEMC -A message transmitted but not responded to should be considered a message not received -Maintain a binder for radio: orders relating to radio, radio and charger manuals, CFD code plug, Inventory Form, Tracking Form, Repair Log; Evaluation Forms -Radio Equipment Operational Evaluation Form- if having operational issues with equipment or infrastructure (dead spots), send form through chain of command to Deputy Fire Commissioner of Operations at CFD HQ -Radio not working- take it out fo service. Ensure member paired with someone with working radio -If Officer radio not working, he takes a company member radio. Follow above -Radio needs repairs: Notify immediate supervisor. Work order from BAS. Complete Radio Tracking Form. Document in Journal and on Radio Repair Log -If radio needs service, send radio with Tracking Form to District. Only send case/strap if they need service, with Tracking Form -DDCs record receipt of radio. Issues spare. Makes copy of Tracking Form and transports radio to shopsvia BAS. GO 10-013 Abandoned Newborn Infant Protection Act -Newborn: 30 days old or less. Hospitals, police stations, fire houses. -Parents can remain anonymous as long as no sign of neglect or abuse. No liability for relinquishing. -If signs of abuse or neglect, mandated reporting -Firehouse personnel accept the newborn and : Provide necessary emergency care for infant, arrange for transport to nearest participating hospital -If parent returns within 72 hours inform them of the name and location of hospital -Tell parent that by relinquishing the infant anonymously they will have to petition the court if they want to prevent termination of rights and retain custody Procedures if a parent wants to drop off a newborn at firehouse: -Assess infant. -Notify OEMC of incident, go on Ambulance Assist status, request Ambulance and PFC -Advise parents of anonymity rules -Offer parent the Safe Haven Information Packet. Acceptance is voluntary. -PFC oversees transport to hospital, ensures that the Safe Haven Packet paperwork is taken with infant if parent(s) elect to fill it out. Chiefs and Company Commanders shall ensure that “Safe Haven” signage is maintained and visible at firehouses and that Information Packet is inspected, complete, and available for use. GO 10-005 Battalion Chief, Bureau of Operations -Witness one roll call in the morning at 0800 and a minimum of 2 roll calls at separate quarters after 1800 hours -Hold monthly Battalion staff meetings with your company officers and submit a report of the meetings to District by the end of tour of duty -Drill each company at least 2 times a month GO 06-013 Driver’s License Requirements I. Driver’s License Required by: A. Any employee from 1995 exam as well as anyone operating Department vehicle. -Anyone who has expired, suspended or revoked license shall immediately report said information to superiors, who shall promptly forward this information on a Form 2 through chain of command. -Company Officers verify compliance by drivers during DAILY inspections. Violation is offense under Code of Professional Conduct. GO 06-001 Smoking Policy -15 feet away -Not allowed in vehicles. Ordinance allows for $100 fine GO 04-001 Drug-Alcohol Testing -Positive presence of Alcohol on breathalyzer is.04 -Computer program randomizes 20 employees -Monday thru Friday 0900-1600 hours but not on holidays -If randomizer selects member on Daley, Furlough, Layup that employee does not have to report and no other member will be placed in their spot. -Employees who will not submit urine for testing will be considered in violation and subject to discipline -The city must provide a list of all members selected that day to Local 2 Contract Enforcement -No employee will be allowed to return to assignment without completion of the testing process -Upon completion of the test the employee can return to assignment if the test results are negative GO 03-004 Non High-Rise Incident Command -Incident Command gives a manageable span of control, specific areas of responsibility, a way to hold those assigned accountable, a means of tracking activities. Prevents freelancing. -Command system sets lines of authority, command structure and communications. It gives Command an informational system regarding available resources. -Ranking Fire Officer is Incident Commander (IC) unless the incident is strictly medical. -The IC is responsible for all activities and operations until relieved by a ranking officer. -Command Post usually Side A in conspicuous location. If not, announce over radio -If a box alarm, the IC will be at command post to give info when the DDC arrives -At all Still and Box the command post will be manned continuously by IC or a designated Chief Officer Priority of Actions: -Incidents where occupants are trapped or at risk, definite assignment to initiate rescue and primary search ASAP -In all occupied structures, initiate primary search ASAP -In structures where occupants are seeking egress, definite assignment to protect and keep stairwells open via leadouts -Incidents involving communication of fire, definite assignment will be given to prevent communication of fire via leadouts. Communications -Only first arriving fire co and first chief report via Main or Englewood. First truck reports “Side ___” -Report on the scene with address, direction of travel and conditions. If fire or smoke give height and construction type -Chief reports address and complete size-up of the incident. (dimensions, contruction, occupancy, exposures, etc.) -OEMC tells still Chief his companies when he arrives. Chief gives specific assignments. -When Command Van arrives, notify Chief and OEMC of arrival and location. Assume communications over fireground. -2-11 and greater, Chiefs will operate on Command Channel (5) -Chiefs and others with specific duty will use that radio designation (e.g. Truck 50 Roof) -If two incidents near each other, the IC of second incident will order companies to switch to Administrative (3) as alternate fireground. -Transfer of Command- If no suppression, mitigation or technical rescue components and run is strictly medical, transfer command to the ranking EMS Officer on scene -Chiefs arriving to assume command will notify OEMC of arrival. Arrival does not constitute assumption of command. Transfer of command announced over OEMC and by van -Plans Chief is designated by OEMC and is in charge of the command van Plans Chief: Tactical Board, Coordinate radio Communications, Control Staging Area, Give Assignments to Companies, Track Personnel and Equipment between staging and assignment; Research Water Supply; Sanborn Maps; Maintain Control of the Van -DDC goes to all Still and Box, assumes command. Progress reports every 15 minutes until under control, every 30 minutes thereafter. -Command Van Driver tells Plans Chief all pertinent info. Gives progress reports. Has to complete CV Form 1 and CV Form 2. Writes still time on board. Staging Area -Minimum of 2 blocks away but can be up to mile or more in extraordinary cases. Can be shopping center, a school or firehouse. No natural barriers. -Staging is set by BC ASAP but not before arriving and assuming command -Still Companies go straight to incident. Box companies go to incident until rerouted by setting of staging area. -Companies go to staging, but Chief's go to IC in person and report to Command Van over fireground. If radio traffic too heavy, Officer goes in person to IC or Command Van. -Companies arriving in staging report to van over fireground. (Ops Ch 10 (* -After staging is set, Chiefs do not report arrival over Englewood or Main. -Fireground to Command Van (unless a Mayday going on, in which case Ch 3) -Stay together ready to deploy -Consider separate staging for Fire and EMS -Inform companies to approach staging from a specific direction -Freezing weather consider indoor staging area (bus garage, firehouse, etc.) -RIT Team reports to Side (A) with tools -If there is a Mayday, chiefs use Administrative Channel 3 Front of the building is always Side (A). Clockwise from there. Hazmat Incidents -Isolate, Contain, Stabilize -Level 1 is minimum response. -Dispatch: 1 Bn Chief, 1 Engine, 1 Truck, 1 Hazardous Incident Team (Squad, 5-1-1, 5-1-1-A, Amb) -Level 2 - Moderate or high potential. Clothing above Level D. Evacuations initiated. -Dispatch: DDC, 1 Team Engine, 1 Team Truck, Safety Officer, EMS PFC; Airmask vehicle, Command Van; Notify Hazmat Coordinator -Level 3 - District Chief. Hazmat Coordinator. Notify Deputy Fire Commissioner. Any other agencies IC deems necessary. -Command Post must be upwind. -Hot, Restricted, Safe, Isolated, Decon, Triage, Staging and Command Post -Isolated is for HIT to set up. Just upwind of Restricted. -Decon is in Restricted (Warm). Exit Decon into Triage. -Be sure Staging is upwind -CPD will secure the perimeter. -IC ensures proper notifications to local, state and federal -HIT communicates with CHEMTREC, others as needed -OEMC gets wind direction from US Weather Bureau -Isolate: Locate any papers. Identify. Tell HIT what you got. -Don’t wash anything down the drains! EPA -No less than 2, but minimum from HIT, make up Mitigation Team -After Isolated Area is established, IC will direct an Amb to that area to take vitals -Clothing Levels: A- Vapor proof encapsulated. D Structural FF gear. -Decon must be established before entering any contaminated area Airplanes -IC should try to establish command post by nose of airplane, but not always feasible. -Pilot Sector 1; Co-pilot 2; Rear of wing Copilot Side 3; Rear wing Pilot side 4 Inside Plane is Sector 5 -Door in Sector 1 is the primary access point GO 00-003 Visitors to Fire Department Facilities -General Public- 0930-2100 -Union visits 1200-1300; 1600-2100 for platoon -Union visits half hour after completion of roll call for non-platoon -Visitors must be documented in Company Journal, including arrival and departure times -Members/union people must report to Officer before commencing visit -Officer is required to document it GO 00-001 Physical Violence -Order describes duty offenses, but off-duty offenses also subject to punishment. II. Definitions: A. Physical Violence: actual or attempted striking of another employee B. Aggression: communicated immediate threat of physical violence C. Provocation: Verbal abuse or communication of non-immediate threat of violence D. Retaliation: A, B, or C above committed in response to such conduct III. Procedure A. Employees subjected to above must report it to supervisor without retaliating in order to be protected from discipline. No justifying retaliation. B. Supervisors take appropriate action to avoid escalation of arguments, including temporary detailing or relief from duty if deemed appropriate. C. Utmost priority: Medical attention for injured employee(s) D. Any incidents: IR #s and comply with current disciplinary procedures. Full and thorough investigations and complete reports including all witnesses’ statements forwarded through chain of command w/out delay. E. Along with Fire Dept. procedures, Mayoral Policy on “Zero Tolerance for Violence in the Workplace” initiated and completed. F. IAD gets involved as liaison with City. Police too if it warrants it. Policy A. In case of death in immediate family (spouse, parents, children, sibling, mother and father-in law, son or daughter-in law, grandparents, grandkids, domestic partner) 40 hour employees can have 3 days off, platoon employees one shift day off. 1. Leave must be taken within 6 months of death. 2. Domestic partner must be registered as such with City B. Proof of death, including copy of certificate, must be submitted within 30 days with the relationship of deceased to employee. C. 5 days for non-contiguous states for 40 hour employees 1 Indiana, Wisconsin, Iowa, Kentucky, Michigan, Missouri are contiguous II. Procedure A. When family member dies, notify immediate supervisor by phone and report all info. B. Supervisor: Notify district; prepare FD Form 920, forward thru channels to Director of Personnel C. Employee shall forward certified copy of death certificate and relation to deceased to Director of Personnel within 30 days of bereavement leave. The engineer shall attach one length of hose to an unused discharge to use as blow of line. -Bad hydrant, notify OEMC and BC who’s Battalion it is in -In Line Operations -Engineer of receiving Engine attaches a length of hose to unused discharge and secures it to be used as a blow off line -Engineer of Supply Engine adjusts discharge pressure to provide a minimum of 20psi to a max of 60psi to the receiving engine -Quick Water -While hooking up to a positive source engineers send tank water -Officer can use Deck gun, crosslay, or back step -Standpipe -Two 2 ½” lines to the FDC of the standpipe system -At least 100’ of 2 ½ hose with 1 ¼ Shut Off Pipe connects to standpipe at least ONE FLOOR below the fire -If the company officer deems the fire can be controlled with 1 ¾ it may be connected to the Shut Off Pipe -Until the height of the fire floor can be confirmed, the engineer shall send pressure to reach the highest floor -Engines pumping into sprinkler systems shall send no less than 100psi -Fast Spreading Fire- Drop two 2 ½ ‘s one with a 1 ¼ SOP and the other with a 2 ½ Adj Fog Point Of Vantage- A 2 ½ with 1 ¼ SOP stretched with 50’ of surplus line GO 93-012 Care, Maintenance, Use & Bedding Hose Cleaning and Drying of Hose- -When in storage it shall be rolled loosely in a cool dry place -If hose has not been used in 6 months, water will be passed thru it -The on duty Platoon shall clean and hang hosed used during their tour of duty up 0700 hours after 0700 the following shift is responsible -LDH does not go in Hose Tower. Only 1 ¾, 2 ½, 3 inch(Trucks) can be hung -Leading out across active Railroad every attempt shall be made to place hose under tracks Bedding Hose On Engine- -Two beds of 700’ of 2 ½ and One bed with 700’ of 4” -5 inch companies carry 1000’ of 5” and same amount of 2 ½ -Front Bumper 150’ of LDH Hose Testing- -Hand lines of 300’ will be tested for 5 min at 250psi -LDH will be tested at 200psi Marking Hose- -12” white background width of the hose placed 8” from each butt -Length Number painted Black 1 ½ to 2 inches high -Company Number painted Red to the Right of the Length Number 2” high CFD Forms- -CFD Form 11.202 Burst, Damaged, or Destroyed Hose. 3 copies -CFD Form 156 Annual Hose Statement 2 copies (district gets original and company file gets the copy) GO 93-008 Office Of Fire Investigations Dumb GO 92-009 Changing Of Platoon at or During Emergencies -When members are relieved on scene or an emergency or at COQ they are considered On Duty until they return to their own Quarters -All relief will be done on company basis and not individually(everyone or no one) -If past 8 hours, the off-going officer will complete all journal entries and submit any OT requests and document in journal -Members will not be compensated for clean up time or changing clothing. 91-010 PRIMARY, SECONDARY, AND FINAL SEARCH I. DEFINITIONS A. Primary Search- Rapid interior search in smoke filled or fire threatened areas. Time becomes primary consideration and fire extension a critical factor B. Secondary Search- complete and systematic search of entire fire area and surrounding areas while fire is being brought under control. C. Final Search- Complete and systematic search of entire structure and area surrounding structure (roofs, alleys, air shafts). Now a recovery operation. II. Primary Search A. Initiated as soon as possible by first arriving truck or squad B. Full protective gear and handi-talkies C. Assigned according to room, apartment or floor, depending on structure. D. Use of hand pumps or lines may be necessary. E. After Primary Officer reports results to IC III. Secondary Search A. Same considerations as with primary. IV. Final Search and Examination A. Same considerations, but search teams may also be assigned to exterior areas of adjacent structures, such as roofs, decks, light shafts, etc. B. Snorkels or Towers may be necessary for overview of particular areas or sectors C. IC should consider using fresh companies from staging area; also consider use of power tools and shoring. V. Responsibilities A. Primary Search and Rescue Team is designated at roll call and documented in journal after roll call but before “absent and cause” entry GO 91-001 Relief and Roll Call I. Policies and Procedures A. Members can leave after 0700 after being relieved by equal rank or by member acting up or down in that capacity. B. Obviously driver must be replaced for driver; engineer for engineer, etc. C. Relieving member must be in uniform. Equipment/gear must be in place. Reporting member must report to officer for determining fitness for duty. Officer then enters reporting member’s name and SCBA number into journal along with exact time of reporting to officer. D. Roll Call conducted at 0800. Inspect member’s personal appearance and uniform, SCBA inspection procedures. E. At roll call, review all orders, memos, directives and instructions since last roll call; review all other orders from superior officers, go over general course of day regarding work schedule, training, etc. II. Responsibilities A. Ensure smooth transition of platoons; make sure manning is maintained. 87-008 SUBSTANCE ABUSE: DRUGS/ALCOHOL I. Disallows use of drugs or positive presence of drugs or alcohol, failure to be fit for duty in opinion of two supervisors (one a Chief of exempt rank), possession of drugs or alcohol on duty, conviction for any section of Controlled Substance Act. A. Urine test or breathalyzer. II. Medical does urine test for following: A. Candidate pre-assignment and pre-cert to Career Service status B. Reinstatement after any absence in excess of 30 days by resignation or leave of absence or termination C. Duty injury involving lay-up as normal medical/hospital procedure D. Driver or Operator of vehicle involved in any accident resulting in injury or significant damage E. Behavior which in opinion of two supervisors (one exempt rank) is suspicious F. Pursuant to terms of Supplemental Disciplinary Recommendation G. Pursuant to annual medical exams and/or required of any apparatus operator certification H. Upon promotion III. Testing A. Medical will test specimen and if positive presence they double check. Employee can request a test done on same specimen, at his expense, by a testing facility other than the one used by Medical. No Expiration Date. Can be used multiple times. Can be used in any situation that a N95 would be used. Oxygen in air needs to be at least 19.5% -Clamshell -Protective Pouch -CBRN canister- previously known as “Live Canister” provides limited protection against Chemical, Biological, Radiological, Nuclear agents. Has a maximum effective work time of 8 hours. If exposed to liquid CBRN agents it is 2 hours. It is one time use only. Oxygen in the air must be at least 19.5% -CBRN canister kit- a complement of CBRN canisters in a pelican case. (1) canister for each on-duty member. Sealed with a Security tag until required for use -Every Saturday member are to drill on the use and operation of the APR -Damage or Deficiency notify BAS OO 20-001 Use of The Cardiac Monitor to Confirm The Withhold of Resuscitative Measures -BLS company, Request ALS response from OEMC. Place AED pads on patient and follow prompts. Continue assessment and treatment per SOP’s until ALS company arrives and assist ALS company. -ALS company, If physically unable to attach leads due to obvious traumatic death, contact Med Control and doctument OO 19-004 Speed Swivel -Speed Swivel is for when the Female connection to the FDC is missing -Bn Chief is responsible for care, use, inspection and storage of speed swivel. -It’s a fitting that fits over the ball bearing raceway to provide a female connection. It has a plastic end cap to protect the threads. -Part of Chief inventory. Stored in the Chief’s vehicle behind the protective barrier and readily available for use. Chief responsible for daily inventory of item. -Damage or wear and tear call Air Mask. Lost/Stolen current policies. -Use- When the FDC is missing its female swivel, the speed swivel can be connected to the ball bearing raceway. Make sure raceway is clear of debris and prepare speed swivel by turning locking collar counter-clockwise until all 4 locking mechanisms have retracted back into the housing -Place it on the FDC with the 2 ½” F connection facing out -Turn the locking collar clockwise until it’s hand tight (no tools) -Now it’s ready to have 2 ½” M conected OO 19-003 Repeat of Emergency Alert Notifications -Emergency Alert Notifications (e.g. Emergency- Downed Wire in Side C) shall be repeated every 15 minutes on tactical channel (4). Notification will be by Command Van (if on scene) or by Incident Commander -Repeat will include the threat and its location -If the Emergency Alert has been secured or is no longer a threat, the IC can cancel the timed announcements (Downed line no longer energized. Cancel announcements) OO 18-020 Cold Weather Sub-Zero Operations -Cold weather < 32 degrees. Sub-zero < 0 degrees -Members can wear extra clothing but their visible gear has to be compliant with the uniform order. -Members required to have back up bunkers and uniform at fire house or on rig -Extra clothes means extra dehydration. Be aware, hydrate. -Petroleum jelly for exposed skin. Dry face piece and regulator. -Water in boots? Don't empty out until you return to quarters. -Rigs: Fuel no less than 5/8. Doors unlocked. Land lines plugged in. -Pump intakes and piping drained. -Transfer valve stays in volume, Tank to pump closed. All but two 2 ½" discharge capped. Hose bed covered. -Air tanks top off at 120 PSI. If they need to be bled do it at 0800 and 2000 and document it in the journal. Check for abundance of moisture in air tanks. -Retarders off in slippery conditions -Trucks- make sure the hydraulic fluid reservoir is full. Be sure ladder locking mechanisms (dogs) are not frozen. M-500 operational. Turntable controls covered. -Don't shut rigs off outside if below 40 degrees -In cold weather Engr when pumping at a fire needs to lead out a second 50' length 2 ½" and let water trickle through so you know you have another line available. -Securing hydrant, remove one cap. Listen for hiss. If operational second cap. Consider in-line operations if you have a working hydrant. -Fully open hydrant by turning 8 turns. Side intakes whenever possible. -If supplying a siamese standpipe or a sprinkler connection be sure that the water us getting past the connection (not stuck behind block of ice) -Use a salvage cover to protect apparatus from water spray. But be sure not to go below the window line or block the side air intakes. -Frozen hose can be thawed by steam truck or secured to the top of the aerial. Ground ladders, too. -Trucks need to exercise the aerial during long operations, make sure it is working -Hydrant must be drained to 48" -Steam truck can be requested at incidents. Don’t use torches on our equipment. -Don't rub frostbite. Warm environment. Sterile dressing. Preplan any repositions. -Engine -Attach 2 ½” line TWO floors below the fire and lead out sufficient hose. No 1 ¾”. Connect to the Floor Below Pipe -1 Eng FF on Standpipe. 1 Eng FF controls 1 ¼” Shut off pipe -Initially position the Floor Below Pipe 3 feet out open window. -Charge the line and make adjustments as need to provide 50 psi at tip -Truck -Gain access to apartment/area below fire -Clear out any blinds, curtains, etc. Don’t break windows! -2 members operate the T-Bar handles OO 18-001 Stand-Alone Personal Alert Safety System (PASS) Device -Inspection form at beginning of tour and after activation at incident -CFD currently has two styles of PASS - Firefly II and MSA Motion Scout -Used in situations where use of SCBA with integrated PASS is impossible or impractical. -Trucks -2 Squads- 6 BN Chiefs - 8 HIT - 6 -Daily basis, inspect for damage. RUn thru functional test of alarm stages. Each Friday during 0800 Roll Call, BC shall have FS&R Companies conduct complete functional test of PASS devices carried by COmpany and BC -Kept in BC buggy, ready for use -If Company has low battery, they call BAS and get work order. Call BC and request spare PASS. BC issues spare until problem resolved. -Damage- Officer notifies BAS for work order. Officer notifies BC for spare, he issues spare until problem resolved. Radio Code Plug on case. 10 security tags -Case must be sealed and have security tag until ready for use -Damages to case report to Officer. Contact BAS -If security tag is compromised or torn off check the radios. If all good, replace security tag. If standby is deployed by IC to assist RIT, IC shall request an additional standby ambulance. -If RIT ambulance initiates patient care to a civilian prior to standby ambulances arrival, standby ambulance will assume patient care so RIT ambulance may resume RIT responsibility.
