These cells are found in the deepest basal layer and move up as they mature. The keratinocyte loses its nucleus as it reaches S. Granulosum and is now called corneocyte, forming “bricks and mortar” structure of S. Corneum ● Melanocyte- Melanin producing cell residing in S. Basale and transferring melanin to K.cyte in S. Spinosum ● Skin barrier function- ○ Acid mantle- pH of 5.5 preventing entry of micro-organisms ○ Lipid bilayer- Oil and water layer of S. Corneum ○ Corneocyte- Dead keratinocyte 3 Healthy Skin- Dermis Papillary dermis •Top layer containing thin collagen and elastin filaments, pressure and pain receptors •Loose connective tissue dispersed in gel-like matrix of glycosaminoglycans (GAGs) •Also contains blood and lymphatic capillaries as well as nerve endings •Comprises of phagocytic cells, mast cells and macrophages Reticular dermis •Reticular dermis contains dense collagen and elastin bundles, cells such as fibroblasts, macrophages, blood vessels, nerves and sebaceous glands •Major cell of dermis is FIBROBLASTS which produce collagen and elastin fibres, forming cross-linked network throughout dermis to maintain structural integrity 4 Mechanical vs Chemical/Thermal Mechanical Trauma (Skin Needling) Chemical/Thermal Trauma (Peels/ Laser) Non- ablative treatments Ablative treatments Epidermal integrity is preserved Causes destruction of epidermis initially Release of Growth factors Coagulation of growth factors and cytokines Increased dermal papillae Loss of dermal papillae Regenerative healing- natural collagen Cicatricial healing scar tissue 5 Skin Needling ● Also known as Collagen Induction Therapy ● Skin needling causes micro-trauma/ micro-injuries to the skin through oscillating microscopic needles. This causes controlled wound-healing response where your skin undergoes through 3 main stages of wound healing called HEMOSTASIS, INFLAMMATION and PROLIFERATION. ● Controlled wound healing induces your body’s own collagen, elastin & hyaluronic acid production through stimulation of FIBROBLAST cell. ● Also increase cellular turnover and cellular cross-talk due to increase in growth factors and cytokines 6 Rollers Vs Skin Needling ● ● ● One depth Vertical and diagonal punctures More downtime ● ● ● Multiple depths Vertical punctures Less downtime 7 Stages of Wound Healing 8 Stages of Wound Healing- Haemostasis First stage of wound healing: Clotting/coagulation initiates the process of wound healing and occurs within seconds • The first phase in hemostasis whereby vasoconstriction occurs immediately and can last 5 – 10 minutes, to restrict blood flow • Next phase in the hemostasis phase, is where platelets stick together in order to “plug” the blood vessel to stop bleeding • Coagulation occurs and reinforces the platelet plug with threads of fibrin which are like a molecular binding agent • As the fibrin mesh begins, the blood clots to keep platelets and blood cells trapped in wound area; this forms the platelet scab 9 Stages of Wound Healing- Inflammation ● First stage of wound healing characterised by heat, erythema, oedema and minor pain ● Platelets are the first respondents and seal the area to form a clot ● This stage involves phagocytosis by cells such neutrophils and macrophages to remove any tissue debris ● This process initiates production of several inflammatory cytokines to help progress the wound through different stages and “call for help” for various other immune cells. ● Macrophages initiate the release of growth factors to stimulate cell migration and division ● Lasts up to 24-72 hours 10 Stages of Wound Healing- Proliferation Wound ● After Inflammation stage, proliferation is triggered by the release of growth factors Fibroblast Activity and cytokines. This process lasts 3-4 weeks ● This triggers fibroblast cell to produce Elastin Collagen Glycosamino Glycans Loss of Elasticity Open Pores, Wrinkles & Scarring Hyalaronic Acid collagen, elastin and hyaluronic acid to produce the extracellular matrix ● Angiogenesis (formation of new blood vessels) is at its peak during this stage ● Dehydration Re-epithelialisation occurs through keratinocyte migration 11 Wound Healing - Remodelling ● This stage is also called Maturation phase ● Collagen laid down in proliferation stage now undergoes cross-linking and maturation. ● “Good quality” collagen becomes visibly long, stiff and converts in to a triple helix strand ● Can last up to 2 years 12 Features & Benefits ● Lightweight hand piece ● Steel micro-needles (32 gauge) ● High speed oscillation to trick the pain receptors and induce channels ● Disposable needle cartridges ● Adjustable needle depth for various conditions 13 Indications ● ● ● ● ● Skin rejuvenation and texture ● Pore size reduction ● Scars ● Acne scarring ● Stretch marks Hyperpigmentation Premature ageing Wrinkles and fine lines UV damage 14 Consultation ● Obtain lifestyle and medical history ● Ensure skin has been prepared with Vitamin A, Vitamin C and Tyrosinase inhibitor ● Ensure client has been using Sunblock and has not had any resurfacing treatment in the last 4-6 weeks ● Ensure client isn’t contra-indicated ● Obtain informed consent ● Take Before/After photographs ● Clients can use topical anesthetic if they prefer to do so. Skin therapists cannot apply or supply anaesthetic, but can assist with application if required (anaesthetic informed consent must be completed). Nurses and Drs can apply anaesthetic ● Ensure client isn’t allergic/ reactive to any products or materials prior to treatment ● NOTE - Please follow your state regulation for topical anaesthetic. Clients/Patients need to purchase and apply their own anaesthesia prior to tx 15 Infection Control ● Clinical hand-washing- 60 seconds wash ● Hand Sanitiser in between glove changes ● Dental Bib under head ● Dressing pack and saline solution ● Alcohol swabs ● Disinfecting the device and cords (As per your manufacturer) ● Do not touch any non-sterile environment (Avoid cross-contamination) 16 Contra-Indication ● Pregnant/ trying to be pregnant ● Breastfeeding ● Auto-immune disorders ● Keloid scars ● Eczema ● Open lesions ● Allergies ● Psoriasis and other chronic conditions ● Bacterial or fungal infections ● Skin cancer ● Acne Vulgaris Stage III-IV ● Actinic (solar) keratosis ● Herpes Simplex infections ● Scleroderma ● Haemophilia ● Cardiac disorders ● Diabetes ● Raised moles, warts, skin tags ● Pustular or nodular rosacea ● Anticoagulant medication ● Frequent resurfacing tx ● Injectable neuromodulators ● Darkened areas of the skin that look suspicious should also be avoided 17 Precautions ● Retin A (discontinued 1 week prior) ● Anti wrinkle injections (2 weeks) ● Dermal fillers (4 weeks) ● Active acne, rosacea or any inflammatory disorders ● Herpes (Antiviral 3 days pre and post) ● Roaccutane (6-12 months) ● Other treatments- IPL, Laser, fractional tx, etc, unless performed on the day 18 Homecare Skin Preparation ● Vitamin A ● Vitamin B ● Vitamin C ● Tyrosinase Inhibitors ● Sun block - Mandatory!
