HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: ADMINISTRATIVE – 037 January 1, 2022 NIMS COMPLIANT SOG Title: Formal Training Program for new Revision: Probationary Firefighter/Paramedics 2 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 4 PURPOSE To define the required formal training each new member will be required to complete prior to beginning his/her 12 month Probationary Firefighter/Paramedic period. POLICY All new members of the department will be required to complete the formal training components prior to beginning his/her 12 month Probationary Firefighter/Paramedic period. PROCEDURE The following outlines the formal training process: 1. New employee Fire Department Orientation a. One week – 40 hours 2. Fire Academy a. Nine weeks – 360 hours b. New employee will attend an approved fire academy. c. May be waived if the new employee has already been certified from the OSFM in Basic Operations Firefighter from an approved fire academy. 3. Hoffman Estates Firefighter Boot Camp a. Three weeks – 120 hours b. New employee will complete upon returning from an approved fire academy. c. If new employee did not require attending an approved fire academy the boot camp will follow the orientation week. 4. Hoffman Estates Firefighter Engine Shift Orientation a. Once Boot Camp is completed and assigned to shift, the new employee will complete an orientation period while assigned as the 4th person assigned to an engine company. b. The employee shall complete a minimum of 5 days assigned to an engine company. 1 c. This 5 day assignment will give the Company Officers the opportunity to teach the new firefighter the roles expected to be completed as part of the company. 5. Northwest Community Hospital In-system Paramedic testing a. New employee will start the registration process as soon as possible which could occur while attending the Fire Academy. b. Hoffman Estates EMS Officer or his designee will submit to NWC EMSS office the completed “System Entry Authorization Form”. c. The employee, will contact the NWC EMSS office secretary to open a file. d. The employee, will complete the SOP and Policy Manual Self-assessments. These assessments will be turned into the EMS Division Head to review before being turned into the NWC EMSS. e. The employee, will upon completion of Step C of this section, complete the NWC EMSS written test, consisting of five modules. f. The employee, will upon completion of Step D of this section, shall be granted temporary ALS privileges. g. Upon being granted temporary ALS privileges, the employee will complete the NWC EMSS skills labs. This portion will run concurrent with the Hoffman Estates EMS Field Team Leader program. h. In the event the new employee is already a Northwest Community Hospital In-system Paramedic upon hiring, they will be subject to steps I and J. i. Upon receiving NWC EMSS temporary ALS Privileges the employee will complete the Hoffman Estates EMS Field Team Leader program. During this phase of training the employee shall: i. Serve as the Team Leader on a minimum of 15 runs (10 ALS) and demonstrate the ability to comprehend, apply and evaluate clinical information relative to the role of an entry level paramedic in the NWC EMSS. ii. Perform patient assessments on 5 ALS calls with varying natures of complaint (cardiac, medical, trauma, respiratory). iii. Perform system-approved ALS interventions demonstrating technical proficiency necessary to fulfill the role of an entry-level paramedic in the NWC EMSS. iv. Correctly apply the ECG leads and interpret rhythm and/or acute changes on a 12 lead ECG for 5 live patients of various age groups. v. Document each submitted run on the currently used electronic patient care report in accordance with principles of medical documentation, attach ECG strips as appropriate, and a run critique form completed by his/her preceptor. vi. Accurately call in the patient report on 15 (10 ALS) runs using the appropriate communication methods and technology including the notification of a cardiac, stroke, and/or trauma alert as applicable. vii. Participate in or simulate the following scenario: critical Level 1 trauma patient, physician on scene giving inappropriate orders, minor refusing transport, dying patient with a DNR order, domestic violence victim, and significant exposure to a patient’s blood through 2 a needle stick. viii. Demonstrate behavior consistent with System and Hoffman Estates Fire Department expectations of an entry-level paramedic in the NWC EMSS. ix. The employee shall complete a minimum of 10 ride days on an ambulance. This may be extended as needed to meet the above requirements. j. Upon completion of step I of this section, i-ix, there will be a meeting involving the Battalion Chief of Training/Safety, EMS Officer, Paramedic Preceptor(s), and the employee to review the progress. This meeting will determine if the employee has successfully met all the requirements as determined by the NWC EMSS or if further training is deemed necessary. 6. 12 month Probationary Training Period a. Begins at the completion of the formal training components 1-5 above. b. Each Probationary Firefighter/Paramedic will be assigned to a Company Officer. The Company Officer will assign the PFF to a shift mentor. 7. Probationary Firefighter Rook Book a. Each Probationary Firefighter/Paramedic will be assigned a Rook Book. b. The Rook Book will be broken up into 3, 6, 9, & 11 month segments. 8. Daily Evaluation Process a. The Probationary Firefighter/Paramedic will complete a Probationary Daily Self-Assessment form each shift day they work. They will also complete this form for all days off. The form will be saved as mm.dd.yy Smith OFF (the date is their actual shift day). They will not need to complete the body of the self-assessment form. b. When the Probationary Firefighter/Paramedic has completed their Probationary Daily Self-Assessment Form they will let their Company Officer know they are finished. c. The Company Officer will then review the Probationary Daily Self- Assessment form making sure they check the reviewed box on the form. They can then use the information to complete a Probationary Daily Evaluation. d. The Battalion Chief/Shift Commander will only complete the Probationary Daily Evaluation Form when the Probationary Firefighter/Paramedic is scheduled off. They will complete this by saving it into the folder as mm.dd.yyy Smith OFF (the date is their actual shift day). They will not need to complete the body of the evaluation form. 9. Quarterly Probationary Firefighter/Paramedic Evaluations a. The PFF will be assigned to a Company Officer who will complete the quarterly evaluations on the following schedule: i. 1st Quarter – January 1st – March 31st ii. 2nd Quarter – April 1st – June 30th iii. 3rd Quarter – July 1st – September 30th iv. 4th Quarter – October 1st – December 31st b. Prior to the 15th of the month following each quarter, the Battalion Chief and the Company Officers will meet to discuss and provide input on the 3 Probationary Firefighter/Paramedic.
