HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: ADMINISTRATIVE – 035 January 1, 2022 NIMS COMPLIANT SOG Title: IDPH Paramedic License Revision: 2 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 2 PURPOSE To establish the requirements and responsibilities of obtaining and maintaining an Illinois Department of Public Health (IDPH) Paramedic license to function within the Northwest Community EMS System (NWC EMSS). POLICY Department policy requires that all Department members obtain and maintain the proper IDPH Paramedic licensure to function within the NWC EMSS. PROCEDURE Initial Licensing A. All Department members that have successfully completed and passed all of the Illinois Department of Public Health EMT-Paramedic (EMT-P) requirements, as listed in the NWC EMSS policy manual, will receive State Licensure as an EMT-P and be permitted to function within the NWC EMSS. B. The EMT-P license will be effective for a period of four years. The EMT-P should maintain the original license in a place that allows for easy retrieval if necessary. C. All Department EMS licensed personnel shall notify the NWC EMSS and the IDPH within 30 days of any change in name or address either in person, by mail, phone, fax, or e-mail. D. It is the responsibility of the individual licensee to maintain their IDPH EMT-P License to function within the NWC EMSS. 1 Renewal / Relicensing / Reinstatement A. All Department EMS licensed personnel will receive a Child Support Statement along with a renewal notice from the IDPH approximately 60 days prior to their license expiration date. This statement will be mailed to their last listed address. The form should be updated with the individual’s home address. This form must be completed to initiate the renewal process either on-line via the IDPH website and fees paid by credit card, or completed and submitted by mail with a certified check or money order enclosed. IDPH will not accept personal checks. The individual shall notify the Department EMS officer when the renewal is submitted. (Detailed instructions are on the renewal notice.) B. The Department EMS Division Officer will then notify the Resource Hospital that the individual licensee has applied for renewal. C. The Department will reimburse the cost of the license renewal. D. Individual licensee(s) whose license has expired less than 60 days shall submit all re- licensure materials to the Resource Hospital/EMS Office. If all materials are in order and there is no disciplinary action pending, the IDPH will process the renewal. E. Individual licensee(s) whose license has expired for a period greater than 60 days, will be responsible for all costs relating to any retesting/reinstatement requirements and will be subject to renewal procedures as outlined in NWCH EMSS policy R-1. F. Failure of the employee to maintain his/her EMT-P certification could result in disciplinary action up to, and including, termination. G. It is the responsibility of individual licensee to forward a copy of their renewed IDPH EMT-P license to the department EMS Officer as soon as they have received it from the IDPH.
