IDPH Paramedic License
- The Hoffman Estates Fire Department requires all members to obtain and maintain an Illinois Department of Public Health (IDPH) Paramedic license to function within the Northwest Community EMS System (NWC EMSS).
Initial Licensing
- Department members who have successfully completed and passed all IDPH EMT-Paramedic (EMT-P) requirements will receive State Licensure as an EMT-P and be permitted to function within the NWC EMSS.
- The EMT-P license is effective for a period of four years.
- The EMT-P should maintain the original license in a place that allows for easy retrieval if necessary.
- Department EMS licensed personnel must notify the NWC EMSS and the IDPH within 30 days of any change in name or address.
Renewal / Relicensing / Reinstatement
- Department EMS licensed personnel will receive a Child Support Statement along with a renewal notice from the IDPH approximately 60 days prior to their license expiration date.
- The renewal notice will be mailed to their last listed address, and the form must be completed and submitted online or by mail with a certified check or money order.
- IDPH will not accept personal checks.
- The individual must notify the Department EMS officer when the renewal is submitted.
- The Department will reimburse the cost of the license renewal.
- Individual licensees whose license has expired less than 60 days must submit all re-licensure materials to the Resource Hospital.
- Individual licensees whose license has expired for a period greater than 60 days will be responsible for all costs relating to any retesting/reinstatement requirements.
- Failure of the employee to maintain their EMT-P certification could result in disciplinary action up to, and including, termination.
- It is the responsibility of the individual licensee to forward a copy of their renewed IDPH EMT-P license to the Department EMS officer as soon as they have received it from the IDPH.
