ESA/SERVICE ANIMAL 1.1 ESA/SERVICE ANIMAL O WNER/A GENT has g ranted t he above named Lessee’ s request for an aid/assis tance/companion animal. Lessee unders tands there is no additional security deposit required but ag rees to the f ollo wing: 1. Onl y the f ollo wing described comf ort/service animal will reside in t he unit: <<P et Inf ormation>> 2. The comf ort/service animal mus t be properly licensed and ha ve t he sho ts/vaccinations required b y statute or regulation at all times. 3. N o comf ort/ser vice animal wit h a histor y of agg ressive, threatening or violent beha vior will be allowed. 4. The comf ort/service animal will no t be allowed out of m y unit except when under m y (or, if applicable, m y care provider’s) direct control and aut hority. 5. The comf ort/service animal will no t be chained or tied in an y way t o t he exterior of t he building.6. The comf ort/service animal will no t be allowed t o use an y part of t he building f or depositing w aste. Should t his occur accidentall y, Lessee will immediatel y pick up the waste. If it is f ound that the Lessee is no t pick up waste, a $50 fine will be char ged f or each occurence. 7. The comf ort/service animal will no t be allowed t o mak e excessiv e noise or eng age in t hreatening conduct which might dis turb the o ther residents. 8. An y animal w aste t hat ma y accumulate inside a tr ay inside t he unit will be disposed of proper ly and prom ptl y. 9. Lessee ag rees t o immediatel y notify t he Owner/A gent of an y personal injur y or property damag e caused by the animal and furt herag rees t hat an y damag es attributed t o the comf ort/service animal will be paid prom ptl y b y the Lessee. 1. An y chang e of comf ort/service animal will require a ne w agreement. 2. Lessee or an y gues t or invitee of Lessee shall indemnify and hold Owner/A gent, and its em ployees, har m-less from and ag ainst an y actions, suits, claims and demands, including leg al fees, cos ts and expenses, arising from damag e or injury to an y person or property of o thers b y any comf ort/ser vice animal. 3. Lessee ag rees not to lea ve t he animal unattended f or more than 24 hours. In t he event t hat t he animal is lef t unattended for more t hat 24 hours, t he Agent ma y enter t he premises of t he Lessee, remove such animal and tur n it over t o the shelter or o ther appropriate aut horities. 4. This ag reement shall become an addendum t o the original lease ag reement between Lessee and o wner/ag ent. By signing belo w, t he abo ve mentioned Lessee certifies t hat the comf ort/service animal has no his tor y of agg ressive, threatening or violent beha vior and ag rees to the abo ve pro visions. Lessee unders tands that per mission t o keep t he comf ort/service on t he premises can be re vok ed if t here is a f ailure to comply wit h the r ules and regulations abo ve or if Lessee per mits support animal t o become a nuisance. U ponre vocation of t his notice of t his Agreement, t he Lessee mus t permanentl y remove t he animal from t he premises within seven (7) da ys from t he date of t he notice.
