HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: Fire Prevention Guideline – 005 January 1, 2022 NIMS COMPLIANT SOG Title: Fire Prevention: Permits Revision: 2 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 2 PURPOSE The purpose of this document is to provide standardized guidelines for the issuance and review of permit applications received by the Fire Prevention Bureau, also known as the FPB. POLICY It shall be the policy of the Hoffman Estates Fire Department to follow this guideline to ensure proper inspections, documentation and report filing of all permits received by the Fire Prevention Bureau. The following are permits generally reviewed by the Fire Prevention Bureau but not limited to just these. Automatic fire sprinkler systems residential and commercial Fire alarm systems Kitchen hood and duct suppression Kitchen hood and duct mechanical Fire clean agent suppression systems Temporary heat source Fire lock box Fire equipment box system Pyrotechnic display includes special effects Firework display PROCEDURE Permit applications are typically received by two methods. Either through the front counter or by mail directly to the Fire Prevention Bureau. When permits are received by the front counter, the front counter will assign a permit number. Once permit numbers are assigned 1 they will remain with the file for eternity. These permits will then be sent via interoffice mail to the Fire Prevention Bureau for review and issuance. When applications are received by mail directly to the Fire Prevention Bureau, those applications will immediately get sent to the front counter, the front counter will then follow the procedure listed above. At no time should a permit application be reviewed, signed in or issued without first being issued a permit number. Once the application is received with a permit number by the Fire Prevention Bureau it shall be placed in a drawer of the permit file cabinet and documented on the clip board as received. The assigned inspector will review the permit and documentation. Once the review is completed and the permit is ready for pick up. It shall be sent to the front counter for processing. Once the permit is issued, inspections will commence. These inspections will include but are not limited to the following: FPSI- Fire Pre Site Inspection FSI- Fire site Inspection FUFI- Fire Underground Flush Inspection FCI- Fire Ceiling Inspection FRI- Fire Rough Inspection FHI- Fire Hydro Inspection FFI- Fire Final Inspection FHWI- Fire Homeowner Walk Inspection After each inspection, the inspector shall input the inspection into Superion within 48 hours of each inspection including closing out the permit in Superion to ensure proper monthly reporting. If any inspection forms are generated for any reason, those inspection forms shall be placed in the permit file along with all original documentation and kept with the permit for the life of the permit file.
