This card is a folded pamphlet in which pertinent medical information is recorded by the resident or family member. The information will assist the paramedics with patient care in the event that the resident doesn’t remember the medical information or cannot convey the information to paramedics. This vital piece of information should help reduce community health risks, while improving efficiency and accuracy of care when the Hoffman Estates Fire Department responds to emergency medical calls at residences and nursing homes. DEFINITIONS Medical Information Card – An 11 inch by 4 inch pamphlet that is folded into a 2 ¾ inch by 4 inch rectangle. Important medical and contact information is recorded to assist paramedics in rendering EMS care. See-Through Magnetized Envelope – A 5 inch by 4 ¼ inch magnetized envelope with a see- through front and red backing. It is labeled “My Medical Information”. POLICY The Hoffman Estates Fire Department will offer medical information cards to residents. The cards will be given out to residents when attending block parties, wellness fairs, senior fairs, as well as nursing home visits. PROCEDURE It is recommended to have residents complete the following sections in pencil on the Medical Information Card: Name, Address, Date of birth, Phone number, Doctor’s information, Emergency contact information, Medications, Allergies, Medical conditions, and Advanced directives. Once the Medical Information Card is completed, it should be put in the envelope and placed on the refrigerator or where it can be easily seen by paramedics.
