    Study Notes

    Renal Module: Key Concepts

    • Course Code: IMP/07/20318
    • Phase: I
    • Year/Semester: Second Year/Semester 3
    • Course Duration: 5 weeks
    • Professor: Olfat Anwar Abd El Aty
    • Email: [email protected]

    Renal Anatomy

    • Shape: Bean-shaped
    • Location: Retroperitoneal, in the upper part of the posterior abdominal region, on both sides of the vertebral column
    • Extent: In supine position, extends from T12 superiorly to L3 inferiorly; The right kidney is slightly lower than the left due to the liver's position.
    • Length: 4.5 inches
    • Breadth: 2.5 inches
    • Thickness: 1.5 inches
    • Weight: 150 grams

    Kidney Structure and Features

    • Renal Angle: The area on either side of the human back, found between the lateral borders of the erector spinae muscles and the inferior borders of the 12th rib, where the kidney is palpable.

    Covering of the Kidney

    • Fibrous capsule: Innermost layer, continuous with the renal sinus.
    • Perirenal fat: Cushions the kidney, helps maintain its position.
    • Renal fascia (Zuckerkandl fascia): A connective tissue condensation outside of the perirenal fat, encloses the kidney and suprarenal glands.
    • Paranephric fat: Outermost layer, rich in fat tissue, part of the retroperitoneal fat.

    Blood Supply

    • Renal artery: Arises from the abdominal aorta, typically divides into 5 segmental arteries.
    • Renal vein: Emerges from the hilum, drains into the inferior vena cava.
    • The left renal vein receives: Left adrenal vein, Branch of the inferior phrenic vein, and the left gonadal (ovarian) veins.
    • Lymphatic drainage: Passes to the para-aortic lymph nodes.

    Nerve Supply

    • Renal sympathetic plexus: Afferent fibers enter the spinal cord at the 10th, 11th, and 12th thoracic nerves.
    • Parasympathetic fibers: From the vagus nerve, travel through the renal plexus.

    Kidney Differences (Right vs. Left)

    • Medial border: Concave on both kidneys, containing the hilum.
    • Lateral border: Convex on both kidneys.
    • Renal vessels: Variations in the positioning and relationships of the renal artery, renal vein, and ureter within the hilum.
    • Relationship to other organs: The right kidney is lower than the left due to the liver's position.

    Factors Stabilizing the Kidney

    • Intra-abdominal pressure: Pressure and arrangement of adjacent organs.
    • Covering fascia and fat: Extensions of connective tissues and fat.
    • Renal vessels: The connection of the renal vessels to the abdominal aorta and inferior vena cava.

    Clinical Significance

    • Renal cell carcinoma: Tumor growth can destroy the renal vein, especially on the left side, leading to varicosities in the left testicular region.
    • Ptosis of the Kidney: Rapid loss of the pre-renal fat can cause the kidney to drop into the pelvis which leads to the kinking of the ureters, resulting in severe pain.

    Other Anatomical Aspects

    • Transpyloric (Addison's) plane: A horizontal plane halfway between the suprasternal notch and the symphysis pubis, at the level of L1.
    • Parallelogram of Morris: Landmark using lines to define the location of the kidneys on the posterior abdominal wall.

    Learning Outcomes

    • Anatomy of the kidneys' shape, surfaces, borders.
    • Location and formation of the renal angle.
    • Differentiating between right and left kidneys.
    • Coverings of the kidneys.
    • Blood and lymphatic drainage of kidneys.
    • Nerve plexus.
    • Surface anatomy of the kidney
