    Clinic Case Manager

    • A PD Form 42 (Injury or Illness Report) must be completed for each injury/illness.
    • If the injury/illness is not a medical emergency, the member must immediately complete and sign the PD Form 42, make a copy, and submit the original to the Watch Commander.

    PD Form 42 Certification

    Pregnancy and Fitness for Duty

    • If the Clinic determines a member's pregnancy status is placing herself, colleagues, and/or the public in jeopardy, her Commanding Official can order a "Fitness for Duty" examination including a "Functional Capacity" examination.
    • If the Chief Physician determines an employee cannot perform the full range of police duties while pregnant, the employee will be placed on limited duty status.
    • Limited duty status continues until the member's private medical physician completes a certification form provided by the Clinic indicating the member is able to meet all the functions outlined in the form.

    Revocation of Police Powers

    • When a Clinic physician determines a member's medical or behavioral health condition prevents them from carrying their service weapon, their police powers shall be revoked.
    • An official from the Medical Services Division must complete a PD Form 77 (Revocation/Restoration of Police Powers and Notice of Duty and Pay Status).
    • Form 77 must be forwarded to the member's element, including an official from the element.
    • The official from the member's element must respond to the Clinic and possess and safeguard any equipment that must be surrendered, including the member's service weapon.

    Prescription Responsibility

    • The Department will not assume responsibility for the cost of prescriptions written by a physician not authorized by the Clinic to fill a prescription.
