BPPV Maneuvers: Understanding Different Techniques to Manage Vertigo

Benign Paroxysmal Positional Vertigo (BPPV) is a disorder that affects the inner ear's balance system, leading to episodes of sudden, rapid rotational vertigo triggered by changes in the position of the head. Several maneuvers have been developed over the years to either position otoconia correctly within the affected semicircular canal (repositioning maneuvers) or encourage habituation to the misplaced otoconia (habituation exercises). These techniques aim to resolve or significantly reduce the symptoms associated with BPPV. This article aims to discuss various BPPV maneuvers, including the Epley maneuver, Semont maneuver, Half-Somersault of Foster maneuver, Brandt-Daroff exercise, and Lempert roll (also known as the BBQ maneuver).

Epley Maneuver

Dr. John Epley introduced the original Epley maneuver in 1992, which involves a series of carefully timed head movements aimed at moving displaced otoconia in the posterior semicircular canal (SCC) to its correct location in the utricle, a small fluid-filled chamber located near the affected SCC. The home version of this maneuver allows patients to perform the technique independently at home, which has been shown to be safe and effective.

How to Perform the Epley Maneuver

  1. Sit on a bed or chair.
  2. Place a pillow under your shoulders.
  3. Turn your head 45 degrees toward the affected ear.
  4. Fold forward quickly, allowing your head to rest on your shoulder opposite the affected ear.
  5. Hold for 30 seconds before turning your head 90 degrees away from the affected ear.
  6. Return your head to the starting position.
  7. Complete two more sets of similar movements, with each set separated by a 30-second pause.

Semont Maneuver

Proposed in 1995 by Dr. Alain Semont, the Semont maneuver is a repositioning technique that manipulates the position of the head to shift otoconia from the affected posterior SCC to the healthy utricle. Similar to the Epley maneuver, the Semont maneuver is used to restore stability to the inner ear.

How to Perform the Semont Maneuver

  1. Sit on a bed.
  2. Turn your head 45 degrees toward the affected ear.
  3. Lie down quickly onto your unaffected ear using a pillow for support.
  4. Hold this position for 30 seconds.
  5. Roll your body onto your other unaffected side.
  6. Turn your head 45 degrees away from the affected ear.
  7. Lift your upper body off the bed while keeping your head facing downward.
  8. Rest your upper body on your hands to complete the rotation.
  9. Hold this position for 30 seconds before returning to the starting position.

Half-Somersault of Foster Maneuver

An alternative to the traditional Epley maneuver, the Half-Somersault of Foster maneuver was developed by Dr. James Foster and is particularly useful for treating those who cannot perform the full range of movements required by the Epley or Semont maneuvers.

How to Perform the Half-Somersault of Foster Maneuver

  1. Sit on a bed.
  2. Turn your head 45 degrees toward the affected ear.
  3. Lean forward 45 degrees, resting your forearms on your knees.
  4. Swing your legs around to your left side.
  5. Lower your legs until your feet hit the ground, creating a half-somersault position.
  6. Bring your legs back in front of you to return to the starting position.

Brandt-Daroff Exercise

Introduced by Eric H. Brant and Robert B. Daroff, the Brandt-Daroff exercise is considered a vestibular rehabilitation strategy that helps to gradually adapt to the presence of otoconia within the affected semicircular canal.

How to Perform the Brandt-Daroff Exercise

  1. Sit on a bed or chair.
  2. Turn your head 45 degrees towards the affected ear.
  3. Sit upright on the edge of the bed.
  4. Lie back, taking care not to strain your neck or spine.
  5. Roll onto your unaffected side and hold for 10-30 seconds.
  6. Return to the seated position.
  7. Repeat the sequence 3-4 times on each side, with a break between positions.

Lempert Roll (BBQ Maneuver)

The Lempert roll, also known as the BBQ maneuver, focuses on stimulating gravity's effects to align otoconia particles within the posterior semicircular canal.

How to Perform the Lempert Roll

  1. Sit on a bed or chair.
  2. Turn your head 45 degrees towards the affected ear.
  3. Rapidly tilt your head backward, bringing it closer to your shoulder.
  4. Slowly return your head to the starting position and repeat the movement several times on each side.

These maneuvers play an essential role in managing BPPV symptoms, helping many people regain balance and reduce vertigo attacks. Although these techniques are generally safe, some precautions should be taken, such as performing them under medical supervision for the first time and avoiding them entirely if you have severe health issues or a recent stroke.
