Hyperthyroidism Overview
- Hyperthyroidism is a condition where the thyroid gland produces excessive thyroid hormones (T3 and T4), leading to a hypermetabolic state.
- Similar to hypothyroidism, hyperthyroidism has primary, secondary, and tertiary causes.
Hormone Production Chain
- The hypothalamus releases TRH (thyroid-releasing hormone).
- The pituitary gland releases TSH (thyroid-stimulating hormone) in response to TRH.
- The thyroid gland produces T3 and T4 in response to TSH.
Pathophysiology of Hyperthyroidism
- Primary hyperthyroidism: Problem with the thyroid gland itself. The most common cause is Graves' disease (autoimmune disorder). Antibodies cause the thyroid to overproduce T3/T4. Thyroid nodules can also cause excess T3/T4 production.
- Secondary hyperthyroidism: Problem with the pituitary gland (e.g., a tumor). The pituitary gland overproduces TSH, causing the thyroid to overproduce T3/T4.
- Tertiary hyperthyroidism: Problem with the hypothalamus. The hypothalamus overproduces TRH, which in turn causes the pituitary gland to overproduce TSH, leading to excess T3/T4 production.
Signs and Symptoms of Hyperthyroidism
- Metabolic effects: Weight loss, heat intolerance, warm skin, insomnia.
- Cardiovascular effects: Tachycardia (rapid heart rate), hypertension.
- Gastrointestinal effects: Diarrhea.
- Other effects: Exophthalmos (bulging eyeballs).
Lab Values in Hyperthyroidism
- General: Increased T3 and T4 levels.
- Primary: High T3/T4, low TSH.
- Secondary/Tertiary: High T3/T4, high TSH.
- Expected Ranges:
- T3: 70-204 ng/dL (high levels can indicate hyperthyroidism).

Treatment of Hyperthyroidism

- Medications: Propylthiouracil (PTU) to block thyroid hormone synthesis, strong iodine solution to inhibit production and release, beta-blockers (metoprolol, propranolol) to manage cardiovascular symptoms.
Nursing Care for Hyperthyroidism Patients
- Increased calorie/protein intake to combat weight loss.
- Monitor vital signs, weight, I&O.
- For exophthalmos, tape eyelids closed for sleep, use eye lubricant.
Thyrotoxicosis (Thyroid Storm)
- Acute, life-threatening complication of hyperthyroidism characterized by extremely high thyroid hormone levels.
- Causes: Infection, stress, DKA, post-thyroidectomy.
- Signs and Symptoms: Severe hypertension, chest pain, dysrhythmias, dyspnea.
- Treatment: Beta-blockers, antithyroid medications, antipyretics (e.g., acetaminophen).
- Nursing Care: Maintain a patent airway, monitor for dysrhythmias.
