Psychosocial Adaptation to Chronic Illness and Disability
- Disability: loss of health due to damage, disorder, deficiency of bodily, mental, or spiritual functions. Inability to work, self-handle, or communicate.
- Disabilities include: difficulty seeing, hearing, speech impediments, walking, bathing, dressing, eating, housework, Alzheimer's, dementia, autism, cerebral palsy, dyslexia; increased risk of depression/anxiety.
- Psychosocial adaptation: adapting harmoniously to life's changes, enhancing well-being and long-term survivorship.
- Key concepts in psychosocial adaptation to chronic illness and disability (CID): stress, crisis, loss and grief, body image, self-concept; uncertainty/unpredictability; quality of life.
- Stress: increased frequency/severity of stressful situations in individuals with CID.
- Sources of stress: daily threats to physical/mental well-being, bodily integrity, independence, autonomy; family, social, vocational roles; future goals/plans; economic stability
- Crisis: sudden/progressive medical impairments/disabilities (e.g., myocardial infarction, spinal cord injury) or life-threatening diagnoses that are highly traumatic.
- Psychological consequences are long-lasting, leading to PTSD
- Loss and grief: mourning process for lost body/function after CID onset; patient exhibits feelings of grief, bereavement, and despair; chronic sorrow depicts experience of individuals with CID.
- Body image: unconsciously held schema of one's body. Impact of diseases like CID can affect perceptions of physical appearance/capabilities.
- Self-concept: body image and self-identity are linked. Individuals with CID also experience social affectation reflected in their bodies
- Uncertainty and unpredictability of the disease process affects individuals psychologically, specifically from heightened levels of perceived uncertainty about a medical condition such as cancer or MS, this leads to decreased psychosocial adaptation.
- Quality of life (QOL): the ultimate psychosocial outcome of rehabilitation practice; a global, multifaceted construct, incorporates intrapersonal, interpersonal, and extrapersonal domains.
- CID-Triggered Responses: Shock/denial, anger, bargaining, depression, acceptance. (These are stages of grief and apply to the response to chronic illness.)
- Emotional reactions: anxiety, fear, anger, strength-weakness, sadness, sorrow, insufficiency, failure, guilt, hope-despair, depression, projection, pathological addiction/regression.
- Behavioral Reactions: support search, expertise in disease, hiding, loneliness, social withdrawal/isolation, nonadherence to treatment.
- Factors affecting psychosocial responses to illness: 1. Factors related to the patient's physical disease (sudden/slow onset; life-threatening/non-threatening); 2. Factors related to the individual's characteristics (life history, experiences, personality, defense mechanisms, coping mechanisms); 3. Environmental factors (society's view of the disease, marital status, family dynamics, social value judgments, and cultural characteristics.)
- Psychological Support: Empathic attitude towards the patient/family. Friendly/supportive communication consistent with ethical principles. Building a sense of trust and flexibility. Respecting patient confidentiality; correcting any incorrect information. Encouraging free expression of feelings, thoughts, and concerns. Strengthening communication/social interactions. Cultivating a sense of self-control in their lives and realistic acceptance/hope for oneself. Allowing time for adaptation to the illness. Psychosocial support to the patients/relatives is effective. Group therapy and medication that regulate the nervous system may be helpful to the patient.
