    • Daily Briefings: Institutional Head/District Director ensures staff are aware of incident reporting procedures to regional and national headquarters, and that staff contact information is updated. Daily security briefings are mandated, their frequency and method determined by the District Director.

    • Quality Control: Correctional Manager/Parole Officer Supervisor (or Area Director if supervisor involved) ensures quality control on reports.

    • Incident Reports: Security Intelligence Officer or delegate completes incident reports in the Offender Management System (OMS).

    • Inmate Injuries: The Chief, Health Services, assesses injury severity as either non-serious bodily injury or serious bodily injury (see Reporting of Serious Bodily Injuries).

    Statement/Observation Reports

    • Documentation: All staff document witnessed incidents/behavior in a Statement/Observation report.

    • Individual Completion: Statement/Observation reports are completed individually and signed.

    • Supervision Review: Completed reports are submitted to the supervisor for review and signature; if unavailable, submitted to a designated manager.

    • Timely Submission: Reports must be completed prior to leaving the facility/office. When delays are unavoidable, supervisor approves and the reason documented.

    • Volunteer/Contractor Involvement: Supervisors instruct volunteers or contractors to document incidents/behavior in Statement/Observation reports.

    Incident Notification Timings (Annexes C, D, E)

    • Immediate Reporting (Annex C): For events like complete lockdowns, deaths, serious bodily injury, health emergencies, or incidents attracting media attention (during regular business hours), the named Regional Headquarters staff member completes a Duty Officer report (CSC/SCC 1004) and sends to GEN-NHQ Incident-Reporting Officer. Within 1 business day, a Security Intelligence Officer/Parole Officer Supervisor will complete an incident report in the Offender Management System (OMS). After hours, this is done by the Deputy Director or delegate at National Monitoring Centre using the Duty Officer report and sending it to an appropriate distribution list.

    • One Working-Day Reporting (Annex D): For incidents in Annex D, regional staff need to report within one business day; regional headquarters then notifies National Monitoring Centre or report goes directly to OMS (depending on core business hours).

    • Three Working-Day Reporting (Annex E): Incidents in Annex E are reported within three business days to Security Operations at Regional and National Headquarters, and an incident report is subsequently completed in the Offender Management System.

    Other Reportable Incidents

    • Community Incidents (not Annexes C, D, E): A casework record or incident report is completed within OMS.

    Reporting of Serious Bodily Injuries

    • Investigation Required: All serious bodily injuries are investigated by the Correctional Service of Canada (via the Corrections and Conditional Release Act, sections 2 and 19).

    • Classification: Chief, Health Services, consults with Regional Director, Health Services, to confirm the injury as serious or alter it to non-serious, if applicable; the reason is recorded.

    • Community-based Facilities: For community-based incidents, the District Director obtains sufficient information (from health professionals) for injury classification determination.

    • Updates: Updates to the severity of the injury (increase or decrease) are recorded in the incident report.
