Study Notes
Clostridium botulinum (Robotulism)
- Gram-positive bacterium
Transmission
- Transmitted through improper canning of food, allowing it to flourish in anaerobic environments and produce heat-stable toxins
- Spores can be found in honey
Characteristics
- Spore-formers
- Obligate anaerobes, cannot survive in oxygen
Treatment
- Antitoxin
- Respiratory support
- Wound debridement
- Antibiotics
Clinical Manifestations
- Flaccid paralysis (vs. tetani spastic paralysis)
- Descending paralysis (starts at the top)
- Early symptoms include ptosis and diplopia
- Toxin cannot cross the blood-brain barrier (BBB), only affects the peripheral nervous system
Pathogenesis
- Botulinum toxin cleaves SNARE protein, similar to Tetany toxin
- Attacks motor neurons that release acetylcholine (ACh), an excitatory neurotransmitter
- Inhibits motor neuron release, leading to flaccid paralysis
