8 Questions

Which of these is NOT a modifiable risk factor for cardiovascular disease?

Genetic predisposition

Which of the following cardiac markers is most specific for the diagnosis of myocardial infarction?

Troponins

Which of the following becomes elevated within 2-4 hours of myocardial infarction but is not specific for cardiac damage?

Plasma myoglobin

Which isoenzyme of creatine kinase (CK) is most specific for cardiac damage?

CK-MB

Which one of the following is a significant reason for using troponins over CK for diagnosing myocardial infarction?

Troponins stay elevated longer

Why are plasma natriuretic peptides useful in the diagnosis of cardiac failure?

They are elevated in ventricular dysfunction

What is the importance of plasma CK in the context of myocardial infarction?

It has isoenzymes that can be specific to cardiac damage

Which of the following is true regarding biochemistry investigations and hypertension?

They may be useful in both managing and investigating hypertension

Study Notes

Lipid & CVS Lab

Objectives of Lab

  • To know CVS markers and risk factors
  • To understand CVS markers for each CVS disease
  • To determine when to send for CVS markers
  • To assess CVS risk factors

Cardiovascular Disease

  • One of the world's biggest killers
  • Risk factors: modifiable (smoking, diabetes, abnormal lipids, hypertension, obesity) and non-modifiable

Diagnosis of Acute Myocardial Infarction

  • Importance of prompt diagnosis for early intervention
  • Use of plasma CK (total and isoenzymes: BB, MB, MM)
  • Limitations of total plasma CK (lacks cardiac specificity)
  • CK-MB isoenzyme: more specific, measured by activity or mass amount
  • Troponins: more specific cardiac markers, used for early and late diagnosis of myocardial infarction

Troponins

  • Elevated for longer than CK after infarct
  • Useful for diagnosis of myocardial infarction

Plasma Myoglobin

  • Elevated within 2-4 hours of myocardial infarction
  • Not specific for cardiac damage (also found in skeletal muscle)

Plasma Natriuretic Peptides

  • Elevated in ventricular dysfunction
  • Useful for diagnosis of cardiac failure

Biochemistry Investigations

  • Helpful in management and investigation of hypertension

This quiz assesses knowledge of cardiovascular disease markers, risk factors, and risk assessment. It covers modifiable and non-modifiable risk factors, and when to send for cardiovascular disease markers.
