Medication: Acetaminophen Trade: Tylenol Class: Aniline Analgesic and Antipyretic Indications: Mild-Moderate pain, Fever Dosing: Fever/infection/pain control Adult: 325-1000mg po Peds: 15mg/kg po Pharmacology: Acetaminophen is thought to act primarily in the CNS, increasing the pain threshold by inhibiting isoforms of cyclooxygenase, COX-1, COX-2, and COX-3 enzymes involved in prostaglandin (PG) synthesis. Pharmacokinetics: Onset: 15-30 mins, Peak effect: 30-120 mins, Duration 3-4 hours, Half-Life: 1-3 hours Contraindications: History of liver disease, inability to swallow/protect airway Side effects: none known side effects when used as directed, if used in prolonged manner can cause liver failure/dysfunction, jaundice, abdominal pain, and nausea Pearls/notes: safe to use during pregnancy, best effect when alternated with NSAID every 4 hours.
