Department of Prosthodontics Dr. Zena J Wally Faculty of Dentistry Prosthodontics University of Kufa Year 3 2023/2024 Types of Removable Partial Denture Removable partial denture (RPD) is a dental prosthesis that substitute teeth and associated structures in partially edentulous arch made from acrylic resin and can be removed and replaced by patient. Types of acrylic RPD 1-Temporary (Interim) RPD: any removable dental prosthesis fabricated for placement immediately following the removal of a natural tooth/teeth. The partial denture is constructed before the extraction of unwanted teeth and is inserted immediately after their removal. Advantage of interim RPD treatment: 1- Esthetic and psychology. 2- Stabilization of a wound: act as protecting pack for a wound. 3- Improve masticatory function. 4- It’s easier to place artificial teeth in their position. 1 2-Transitional RPD: a removable prosthesis serving as an interim prosthesis to which artificial teeth will be added as natural teeth are lost and that will be replaced after post extraction tissue changes have occurred. A transitional denture may become an interim complete dental prosthesis when all of the natural teeth are removed from the dental arch—called also complete denture 3-Treatment RPD: dental prosthesis used for the purpose of treating or conditioning the tissues. Such as: Implant healing or alteration of Vertical Dimension /Occlusion 4- Conventional Partial Denture. Uses of Acrylic Removable Partial Denture 1- Esthetic or appearance 2- Space maintenance it is important to maintain the space for: a- In young patient: the space maintained until adjacent teeth have erupted. b- In adult: it can prevent migration of adjacent teeth and over eruption of opposing teeth. 2 3- Reestablishment of new occlusal relationship a- To establish a new occlusal relationship or vertical dimension. b- To condition teeth and ridge tissues to support the permanent restoration. 4- Conditioning the residual ridge: the patient wears a temporary partial denture for period of time before construction of the final prosthesis. 5- Conditioning the patient: A temporary restoration may be made to aid the patient in making a transition to complete dentures. Disadvantages acrylic partial denture: 1- Less patient response and tolerance for acrylic denture than metallic denture. 2- Poor thermal conductivity. 3- Lower strength therefore: a- Easily broken. b- Must be constructed in thick sections. 4- Tendency for warping. 5- Design difficulties: 3 a- Ideal tooth support is difficult b- Ideal indirect retention is difficult. c- Unnecessary tissue coverage. d- Impossible to use more sophisticated components (precision attachments, stress breaker). 5-Flexible denture Nylon flexible denture Main composition: Polyamide nylon thermoplastic material. Advantages of nylon flexible denture 1. Soft and inherent flexibility: o prevents the gum from being rubbed. o Ability to engage undercuts for retention. o No need for periodic adjustment of clasp. 2. Not warp or become brittle. 3. unbreakable. 4. Good biocompatibility: because it is free of monomer and metal. 5. No porosity, so no bacteria can build up within it. 6. No gingival inflammation. 7. More comfortable. 8. Absorb small amounts of water to make the denture softer and tissue compatible. 4 9. light weight. 10. Better chewing efficiency 11. Better esthetics: o Translucent (allows natural gum to show). o Clasps rest on the gums surrounding the natural teeth. o No clasp on the tooth surface. o No metal framework. 12. More retention and stability. 13. Retention depends on the tissue and only a small portion on abutment tooth. 14. Ease of fabrication. 15. Reduces chair side time (shorter fabrication time). Disadvantages of nylon flexible denture 1. Intended for temporary applications. 2. De-bonding of the acrylic teeth from nylon denture base (mechanical bond). 3. Discoloration. 4. High surface roughness. 5. The procedure is technique sensitive. 6. Difficult to adjust and polish. 7. Lower hardness. 8. Cannot be relined and cannot be repaired. Indications of nylon flexible denture 1. Bilateral undercuts. 2. Allergy to acrylic resin. 3. Patients who do not want a fixed restoration and metal clasps. 4. No enough bone for fitting dental implant. 5. Bruxism. 6. Patients with compromised neuro-motor function. 7. Single denture. 8. Obturators. Contraindications of nylon flexible denture 1. Unilateral distal extension. 2. Low vertical dimension and closed bite. 5 Difference between Acrylic denture and flexible denture 6
