Southern Nevada Fire Operations STANDARD OPERATING PROCEDURES EMERGENCY OPERATIONS Effective date: Supersedes: PERSONNEL ACCOUNTABILITY 05/17/2017 02/28/2008 SOP# SNFO 01 Page 1 of 5 PURPOSE One of the Incident Commander’s responsibilities is to track and control the position and function of all incident personnel. This capability becomes a major accountability and safety factor and must be done within a standard, practiced system. PROCEDURE TACTICAL WORKSHEETS A critical part of maintaining overall control of the incident is the IC’s ongoing capability to effectively run an on-line inventory and status keeping system of resources assigned to the incident. The tactical worksheet becomes a major deployment management tool in completing that responsibility. Tactical Work Sheets should record the following data: • Resource status—staged, assigned • Resource assignment—division/group/task • Resource-tracking accountability—companies inform command when they have completed their assignments and/or are leaving a specific area to work another area • Organizational Chart— Graphic representation of Incident Command structure • Simple drawing of the incident area or occupancy and assigned resources • Tactical priority check list and benchmarks of completion • A check-off time line of elapsed times in 10 minute intervals to mark and manage event time; and to re-evaluate conditions, the strategy, and the length of time firefighters have been operating in the hazard zone. ACCOUNTABILITY RESPONSIBILITIES Each person involved in an incident whether at the strategic, tactical, or task level must make a strong personal commitment to follow all policies and procedures regarding accountability. Command is responsible for the strategic level of accountability by the tracking of all crews and Division/Groups by location and function on a tactical worksheet. • Command must know who is in charge of each Division/Group, crews assigned to each Division/Group, where each Division/Group is located, and what each Division/Group is assigned to do. Southern Nevada Fire Operations STANDARD OPERATING PROCEDURES EMERGENCY OPERATIONS Effective date: Supersedes: 05/17/2017 02/28/2008 PERSONNEL ACCOUNTABILITY SOP# SNFO 01 Page 2 of 5 • Command will consider air supply when making tactical assignments including rotation of crews. • Command must obtain Personnel Accountability Reports (PAR) from Division/Group supervisors and crews. Division/Group supervisor is responsible for the tactical level of accountability by tracking of crews assigned to their Division/Group. • Division/Group supervisors must know the location and function of assigned crews. • Division/Group supervisors must be in his/her assigned area to maintain close supervision of assigned crews. • Division/Group supervisors must obtain PARs of all crewmembers of all companies assigned to his/her Division/Group. • Division/Group supervisors will utilize the PASSPORT system to ensure accountability for their assigned personnel. Company officer is responsible for the task level of accountability and must know where each firefighter is located, and what each firefighter is doing. • Company officers shall maintain a current PASSPORT of personnel responding on their assigned apparatus at all times. The Company Officer shall carry with them the Primary PASSPORT. The Secondary PASSPORT shall be left on the dashboard of their respective unit. • Company officers must obtain PARs for their crews, which is a confirmation that all members assigned to his/her crew are accounted for and have an adequate exit air supply. • Company officers must inform command when they have completed their assignments and/or are leaving a specific area to work another area. Accountability communications (Simplified Accountability) Company Officers and/or working Division/Group Supervisors shall vocalize/communicate the number of personnel under their direct supervision to the Incident Commander any time personnel are assigned to operate inside, on top, or underneath a structure or work in an unknown or IDLH atmosphere. This procedure shall ensure an accurate accounting of personnel. Southern Nevada Fire Operations STANDARD OPERATING PROCEDURES EMERGENCY OPERATIONS Effective date: Supersedes: PERSONNEL ACCOUNTABILITY 05/17/2017 02/28/2008 SOP# SNFO 01 Page 3 of 5 Company Officers and/or working Division/Group Supervisors shall vocalize/communicate to the Incident Commander the number of personnel under their direct supervision who exit a structure, hazardous area, or who no longer are operating within an IDLH atmosphere. During escalated operations when a stationary tactical Division/Group Supervisor (non-IDLH) has been established in an expanded Division/Group (3 or more crews assigned), PARs will be reported within the Division/Group. These reports do not need to be relayed to the IC over the tactical radio channel. • Examples of IDLH or special hazards:     • Offensive/Defensive Fire Operations Hazardous Materials Incidents Trench Rescue Confined Space Rescue Example of vocalizing/communicating the number of interior IDLH personnel:      E-14 is assigned to "Fire Attack" by the Incident Commander E-14's Captain acknowledges the assignment: "E-14 is Fire Attack entering with 4" (advising Command and RIT that he/she will make interior fire attack with four personnel) "Fire Attack has 'knockdown' exiting with 4" (indicating assignment complete, E-14 is exiting the building with four personnel) T-1 is assigned "Roof Division" for vertical ventilation T-1's Captain acknowledges the assignment: "T-1 is Roof Division on the roof with 5 for vertical ventilation" (advising Command and RIT that T-1 will be on the roof with five personnel performing vertical ventilation)  "Roof Division has completed vertical ventilation, off the roof with 5"  E-98 is assigned "Primary Life Search"  E-98's Captain acknowledges the assignment: "E-98 is Search Group entering with 3, conducting Primary Life Search" (advising Command and RIT that E-98 will Southern Nevada Fire Operations STANDARD OPERATING PROCEDURES EMERGENCY OPERATIONS Effective date: Supersedes: PERSONNEL ACCOUNTABILITY 05/17/2017 02/28/2008 SOP# SNFO 01 Page 4 of 5 operating inside the structure with three personnel beginning a Primary Life Search)  "Search Group has completed Primary – All clear, E-98 exiting the building with 3 All Members • • • Must ensure they are accurately listed on the PASSPORTS for their unit. Stay with his/her crew at all times. Maintain a constant awareness of his/her exit air supply. All Crews • • • • • • Work for Command or Division/Groups - no free lancing. Crews arriving on the scene should remain intact. A minimum crew size will be considered two or more members. Each member must have a radio. All crews entering the hot zone/hazard area must have a leader. All crews will go in together, stay together, and come out together. PERSONNEL ACCOUNTABILITY REPORT (PAR) The Personnel Accountability Report (PAR) involves a roll call of all personnel assigned to crews and Division/Groups that are working in the hot zone. The PAR is a confirmation that all members are accounted for and have an adequate exit air supply. For the Division/Group supervisor, a "PAR" is an accounting for all crewmembers of all companies assigned to his/her Division/Group. For the Company officer, a PAR is an accounting of all crewmembers assigned to his/her company. Reports of PAR's should be conducted face-to-face within the Division/Group or company whenever possible. When all crew members are accounted for, the officer reports to Command the following information: 1. Present assignment or designation 2. Unit name (or Units) 3. The fact that the crew has PAR Example: “Fire Attack, E55 has PAR.” or “Division 1, E7 with R7 and E9 has PAR with 10.” Southern Nevada Fire Operations STANDARD OPERATING PROCEDURES EMERGENCY OPERATIONS Effective date: Supersedes: PERSONNEL ACCOUNTABILITY 05/17/2017 02/28/2008 SOP# SNFO 01 Page 5 of 5 A Personnel Accountability Report (PAR) will be required for the following situations: • As companies report an all clear • As companies report knock down • Any change from offensive to defensive • At every 20 minutes of elapsed time • Any report of a missing or trapped fire fighter • Any sudden hazardous event at the incident - Flashover, Backdraft, Collapse, Mayday, Abandon, Withdrawal, etc. • Any time Command feels it is necessary
