Where a person is unable to manage daily activities of living in the long term without support mechanisms in place z Types of disability Disability can occur at any time, to any body in any way shape or form ❖ Injury ❖ Illness ❖ Congenital Nursing considerations in hospitals and the community z ❖ Communication ❖ Working with a multi disciplinary team ❖ Incorporating your knowledge to individual needs ❖ A patient’s disability will certainly make your admission and care planning more comprehensive The z newly disabled Let’s consider the implications of the disability for the individual: ❖ what's their lifestyle, what's their work and workplace like, what’s their home like? ❖ How might they adjust mentally and physically: putting body and mind considerations together? ❖ Rehabilitation process: what would that look like? ❖ Is it illness that is now their life trajectory? ❖ Might this be life changing? Life changing for an individual & adaptation z ❖ Loss of a sense ❖ Loss of limb ❖ Paraplegia or hemiplegia ❖ Disability from long term illness z Rehabilitation: a team effort z Do it well and you never know how someone might manage ☺ z Community effort ❖ The community effort involves individuals, society, and policy to make it work a starting point is to consider health and how individuals are affected by their health but also how their health affects their lifestyle option… is there inequity..? z Social determinants of health 1. Neighbourhood and built environment 5 priorities of providing better health outcomes for disabled people z The Health of Disabled People Strategy New Zealand 1. Embed self-determination of disabled people and their whānau as the foundation of a person and whānau-centred health system. 2. Ensure the health system is designed by and accessible for disabled people and their whānau and provides models of care that suit their needs. 3. Ensure the health system is part of a coherent cross-government system that addresses broader drivers of poor health and wellbeing. 4. Build health workforce capacity and capability to meet the needs of disabled people and their whānau. 5.
