SYSTEM EXAMINATION TEMPLATE: Respiratory Examination WIPE Wash hands, Introduce yourself and gain consent, Position patient, Expose Hi, I’m Victoria and I’m a Paramedic. Today I’m going to be conducting a respiratory assessment which will involve me looking, listening, and feeling for a few things, is that ok? If at any time you do feel uncomfortable, please let me know and we can make any adjustments. Expose patient: General Impression Colour: cyanosis - evidence of peripheral or central cyanosis? Pallor? Work of breathing: shortness of breath? Accessory muscle use? Cough: type of cough – muffled, wheezy, ineffective à COPD, loose productive à bronchitis/pneumonia Stridor: obstruction of larynx or trachea (on inspiration), wheeze: à asthma, congestive heart failure Sputum: colour, volume and type à blood, pus etc Hands Colour: Peripheral cyanosis? à underlying hypoxaemia Temperature: warm, well perfused? Cool hands may à poor peripheral perfusion Capillary Refill:
