Pharm Comp 1. Propranolol: beta blocker, used for hypertension,angina, dysrhythmias, and heart failure. Monitor heart rate, monitor for heart failure, and medication must be tapered off. 2. Digoxin: cardiac glycoside, used to treat heart conditions (slows and strengthens the heart), used for afib. Watch for hypokalemia, monitor cardiac rhythm. Hold below 60 3. Nitroglycerin: Used to treat angina, take 3 times every 5 min if it doesn't get better, call 911, check expiration date, rotate sites, keep in a dark cool place, change every 12 hours and remove old patches. 4. Isosorbide: Long term oral version of nitroglycerin, follow the same precautions, do not use sildenafil and isosorbide together. 5. Lisinopril: ACE inhibitor that blocks the production of angiotensin II. CAN CAUSE A DRY COUGH, used to treat CHF, watch for angioedema, monitor K levels.\ 6. ASA: Aspirin, NSAID, Antiplatelet aggregate, used to treat mild to moderate pain for RA, treats inflammation, COX 2 inhibitor, lowers risk of stroke, blood clot, or heart attack. DO NOT chew, avoid alcohol. Toxicity signs include tinnitus, vomiting, fever, dehydration, double vision, and feeling faint. 7. Omeprazole: proton pump inhibitor, antiulcer 8. Heparin: anticoagulant (short acting), monitor for hypotension, tachycardia, monitor aptt levels (30-40) antidote is protamine sulfate. 9. Warfarin: anticoagulant, antidote is vitamin K, long acting treatment for DVT, check PT and INR levels, take at night. 10. Spironolactone: potassium sparing diuretic, blocks aldosterone to hold potassium and remove sodium from the body. Monitor potassium levels. 11. Ferrous Sulfate: Iron supplement, check stool, monitor for constipation 12. Lactulose: Desired affect-loose stool and decreased ammonia 13. Metformin: antidiabetic that controls glucose levels in diabetes 2 watch for signs of lactic acidosis and anorexia. Avoid consuming alcohol. 14. Allopurinol: used to decrease uric levels in gout, increase risk of kidney stones, avoid taking calcium and vitamin D, watch for liver toxicity, can affet how warfarin is metabolized. 15. Alendronate: used for osteoporosis, sitting upright for 30 min, takes about 6-8 to work, can cause esophageal erosion, slows down the cells that break down bone, and takes in the morning on an empty stomach. 16. Vancomyicin: treats infections, adverse effects include nephrotoxicity and ototoxicity and red man syndrome watch for allergic reactions. 17. Naproxen: NSAID used for pain, fever, watch for GI bleeding, watch for allergic reactions. 18. Lovenox: Anticoagulant, prevents DVT, protamine sulfate is the antidote, check potassium levels, check stool for occult blood, do not take in combination with NSAIDS. 19. Atorvastatin: Used for cholesterol, can cause rhabdo (the breakdown of muscle tissue), watch for liver toxicity, given at night. 20. Docusate Sodium: A laxative used to prevent constipation 21. Diphenhydramine: antihistamine used to treat allergic reactions. Do not drive, give with food, encourage fluids, monitor urine retention, do not crush. 22. Methotrexate: used for RA, reduced inflammation, drink 2-3 liters of water a day. 23. Morphine sulfate: opioid analgesic works by changing the brain's perception of pain, monitors lung sounds, BM, always tapers off and never stops abruptly to avoid tolerance.
