D. , N D NMT150 2023 LEARNING OBJECTIVES Considerations for providing nutritional guidance as it relates to this week’s case presentation: Fish oil Vitamin D Zinc Vitamin A TREATMENT GOALS Address presenting symptoms and improve patient comfort Support full and speedy recovery to return to regular activities Patient education Monitor and prevent complications FOOD SENSITIVITY 59 children with Atopic Dermatitis (AD) eliminated cow’s milk for 4 wks: 80% had improvement. 91 children with AD were placed on an elimination diet: An improvement in 74%. 50 adults with AD tried 6 wks of a diet that excluded pseudo allergens: 63.5% had positive responses. FOOD SENSITIVITY Some studies have failed to show the role of food sensitivity in AD. Conventional medicine: Food allergy (sensitivity) rarely plays a role in the etiology of AD. POPULAR SUPPLEMENTS Zinc Vitamin D Fish oil Vitamin A ZINC Some studies show that supplementing with zinc may help reduce eczema severity, especially if there is a deficiency. ZINC Shellfish, beef, and other red meats are rich sources of zinc. Nuts and legumes are relatively good plant sources of zinc. VITAMIN D Vitamin D deficiency is associated with the risk of psoriasis and atopic dermatitis, and several studies have suggested the beneficial effect of vitamin D in the therapy of these 2 inflammatory skin disorders. Multiple studies suggest that supplementing with vitamin D may reduce inflammation and symptoms of eczema. VITAMIN D Cholecalciferol (Vitamin D3) o Synthesis occurs in the epidermis of the skin from UVB o Animal sources: saltwater fish, liver, meats, egg yolks, and fortified foods. Ergocalciferol (Vitamin D2) o Plant sources (mushrooms, alfalfa sprouts) and fortified foods FISH OIL Some research investigating the effects of fish oil supplements on eczema has shown promising findings. However, it’s important to note that there is a lack of research in this area, and more studies are needed. FATTY ACIDS Chains of hydrocarbons of varying length Long-chain FAs: 14-24 Cs Medium-chain FAs: 8-12 Cs Short-chain FAs: 4,500 µg RAE (15,000 IU) per day of preformed vitamin A in pregnant women has been associated with birth defects 21 CONTRAINDICATIONS & CAUTIONS o High doses of vitamin A are contraindicated in pregnancy and in fact all sexually active women of childbearing age without an effective form of birth control. o Patients with end-stage renal disease should not be given supplemental vitamin A. o People with liver disease, malnutrition, and alcoholism are at higher risk of vitamin A toxicity and there are reports of toxicity from doses of
