C.C (chief complaint) : تكتب كﻼم المريض وتحدد Most common C.C are PAIN, ULCER, SWELLING. مكان الشكوى E.G.: pain on upper right quadrant. C.C ممنوع تحدد السن ف ال تجربة المريض مع طبيب اﻻسنان الحشوات P.D.H (past dental history): ﻻ تهمنا في الجراحة والذي يهمنا هو الخلع وهل كان معه أي مضاعفات سواء للخلع او E.G.: extraction of upper first molar without any.التخدير complication (no postoperative bleeding, no مثﻼ هل حصل له دوخه بعد التخدير او هل postoperative pain or simple extraction).حصل له نزيف بعد الخلع You can write SIMPLE EXTRACTION of upper first molar H.P.I (history present illness): مهم جدا من هنا نحدد التشخيص …….
