Burkitt Lymphoma ‫ بشكل سريع‬tumor‫يتميز بسرعة التكاثر والتضاعف وظهور ال‬ ▪ It is a highly aggressive and rapidly proliferating lymphoma. Three major types exist: ‫ شائعة‬/ ‫مستوطنة‬ ‫ث أنواع‬V‫ لث‬Burkitt lymphoma‫بنصنف ال‬ 1. African (endemic). ‫ حا_ت فردية‬،‫ليست مستوطنة‬ 2. Western (non-endemic, sporadic). ‫مرتبطة مع نقص ا"ناعة‬ 3. Immunodeficiency associated. ‫عمار الصغيرة‬f‫ يتميز انه بصير با‬lymphoma‫أول نوع من ال‬ ▪ Both the endemic and nonendemic forms affect mainly children and young adults. lymph nodes‫ خارج ال‬،Diffuse large B-cell lymphoma ‫شبه ال‬ ▪ In both forms, the disease usually arises at extranodal sites. ‫تحدث غالبًا بمنطقة الوجه‬ ▪ In African patients, involvement of the maxilla or mandible is the common mode of presentation. ▪ In North America, abdominal tumors involving the bowel, retroperitoneum, and ovaries are more common ❑ Pathogenesis EBV‫ مرتبطة مع ال‬Burkitt lymphoma‫بعض أنواع ال‬ endemic‫خصوصا ال‬ ً ▪ In most of African Burkitt’s lymphoma, tumors carry the EBV genome. ▪ In ~20% of sporadic cases, the EBV genome is found in the tumor cells. ‫ فيها‬translocation ‫أشهر‬ ▪ Burkitt’s lymphoma show t(8;14) chromosomal translocation( involves the MYC gene) lymphoma‫ بال‬translocation ‫هيك صرنا ماخذين ث@ثة‬ ‫ ؟‬2 ‫متذكرين شو أول‬ Endemic Burkitt Lymphoma in the jaw among African children. ‫ مكون من خ)يا متوسطة الحجم وتتميز انه الخ)يا بشبهوا بعض تقريبًا والها نواة واضحة‬Burkitt lymphoma‫ال‬ lymph ‫ا ناخذ عينة من‬W ‫ )فقاعات دهون( وين أكثر اشي بنشوفها ؟‬lipid vacuoles ‫ للخ)يا يتميز باحتوائه على‬cytoplasm‫ال‬ ‫جهر‬W‫ ونتفرج عليها تحت ا‬node ❑ Morphology - Diffuse growth pattern. - The tumor cells are uniform and intermediate in size and typically have round or oval nuclei with 2-5 distinct nucleoli. - There is a moderate amount of basophilic or amphophilic cytoplasm that often contains small, lipid-filled vacuoles (a feature appreciated on smears). lipid vacuoles‫الدوائر البيضا هي ال‬/ ‫\حظوا الفقاعات‬ ‫ موجودين‬macrophages‫ معناته أكيد حنشوف ال‬macrophages‫ بوخذها وببتلعها ؟ ال‬B‫يا بس تموت م‬8‫ طب الخ‬،‫يا بتموت بسرعة‬8‫ وكذلك الخ‬high mitotic rate ‫التكاثر بالورم عالي معناته‬ ‫يا الي ماتوا‬8‫ عشان الخ‬macrophages‫ حتتواجد ال‬sheet‫ وبرضو بال‬diffuse sheet ‫يا شبه بعض بالشكل والحجم وعلى‬8‫ ايش حنشوف ؟ حكينا انه الخ‬tumor‫طب لنعتبر انه اخذنا خزعة من ال‬ starry sky apperance ‫ كأنه شكل الفضاء مع النجوم بنسميه‬B‫ بلون ثاني فبتب‬macrophages‫ بلون موحد ومع وجود ال‬sheet‫ ال‬B‫بالتالي هيك حتب‬ ▪ A high mitotic rate is very characteristic of this tumor, as is cell death accounting for the presence of numerous tissue macrophages containing ingested nuclear debris. ▪ Because these benign macrophages are often surrounded by a clear space, they create a" starry sky" pattern. ‫ الي‬،‫ وكلهم شبه بعض تقريبًا‬tumor‫يا ال‬/‫اللون البنفسجي هم خ‬ ‫يتة‬E‫يا ا‬/‫ بتبلع الخ‬macrophages‫بالزهري هم ال‬ ❑ Immunophenotype: - These tumors express the B cell marker CD20. ❑ Prognosis - Burkitt lymphoma is highly aggressive; however, with very intensive chemotherapy regimens, most patients can be cured. Hairy Cell Leukemia In bone marrow low grade /‫يتكاثر ببطء‬ ‫ طالع منهم زوائد‬lymphoid cells ‫يحتوي على‬ ‫على سطح الخلية شكلهم زي الشعر‬ - It is a rare indolent B-cell neoplasm characterized by the presence of fine, hairlike cytoplasmic projections. - Middle-aged males ▪ Massive splenomegaly. ‫ حتأثر على تواجد‬bone marrow‫ تتواجد بال‬hairy cells‫ا هاي ال‬+ ‫نتاج للخ;يا‬C‫الخ;يا ا 90% have point mutations in the BRAF gene. ‫الطفرة الي صارت‬ ‫نه بساعد بالتشخيص‬O ‫مهم‬ ▪ Distinctive markers: B cell markers, CD11c, CD25, CD103. ▪ Indolent course with excellent response to chemotherapy Microscopic ▪ Peripheral blood smear shows abnormal lymphocytes with indistinct cytoplasmic borders and surface projections, giving the cells a “hairy” appearance. ▪ The red cytoplasmic staining seen at the lower right is tartrate-resistant acid phosphatase (TRAP) positivity enzyme‫ تستجيب لهذا ال‬hairy cell‫ال‬ T-cell lymphoma/Leukemia ❖ Mycosis Fungoides ‫ كأنه اشي عادي زي التهاب بأحد‬D‫ بتب‬،skin lesion ‫بتيجي على شكل‬ lymphoma ‫ انه‬N‫طبقات الجلد او اشي إ‬ - Cutaneous T-cell lymphoma, usually CD4+ - Erythema, plaque, and tumor phases - Epidermis and dermis infiltrated by cerebriform cells with marked infoldings of the nuclear membranes. ،‫ وحكينا عنها كثير‬B-cell lymphoma‫تعمقنا بال‬ T-cell‫هسا بدنا نحكي شوي عن ال‬ ❖ Adult T-cell Leukemia/Lymphoma - Neoplasm of CD4+ T cells ‫بكون مصحوب مع‬ - Infection by HTLV-1 Human T-lymphoma virus - Skin lesions, generalized Lymphadenopathy, hepatosplenomegaly, hypercalcemia. - Very aggressive Plasma Cell Neoplasms malignant plasma cells ‫ مكون من‬tumor ‫هو‬ ‫ وهذول بشتغلوا با
