Benztroplne (Cogentln} 0.5-2 mg TID (start QHS) I SIDE EFFECTS High incidence of peripheral and central antichollnergic effects: dry mouth, _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ _ co nstipation, u rinary retention, blurred vts1 on, niydriasi!., somnolence, co nfusion, tachycardia Trihexyphenidyl 1-5 mgTID (start 1 mg QHS) NOTES Avoid use in elderly patients Adenosine receptor antagonist: used in combination with carbidopa/levodopa to reduce "off" episodes. lstradefylline (Nourianz) 20 mg PO daily, can titrate to a max of 40 mg dally Dose must be adjusted if used with CYP3A4 inhibitors or tobacco smoking WARNINGS Hallucinations, dyskinesias, impulse control disorders SIDE EFFECTS Nausea, constipation Alpha/beta agonist: used for neurogenic orthostatic hypotension, Droxidopa (Northera) Start at 100 mg PO TID, can titrate every 24-48 hour to a max of 1800 mg/day Take the last dose at least 3 hours prior to bedtime (to avoid supine hypertension during sleep) BOXED WARNING Supine hypertension: monitor supine BP prior to and during treatment; elevate the head of the bed and measure BP in this position; if supine hypertension cannot be managed by elevation of the head of the bed, reduce dose or discontinue SIDE EFFECTS Syncope, falls, headache MAO-B Inhibitor Drug Interactions While talcing, do not eat foods high in tyramine (aged or matured cheese, air-dr ied or cured meats, including sausages and salamis, sauerkraut, fava or broad bean pods, tap/draft beers, Marmite concentrate, soy sauce and other soybean condiments). Avoid these foods during and for two weeks after discontinuation of the medication. 888 Do not use with products containing dopamine, tyrosine, phenylalanine, tryptophan or caffeine. Do not use with other drugs that increase the risk of serotonin syndi;ome. See the Drug Interactions chapter. • Rasagiline is a CYP1A2 substrate; limit dose to 0.5 mg daily with ciprofloxacin or other CYP1A2 inhibitors.
