Palliative Care Experience in the United States Dr. Meryem ERCEYLAN PALLIATIVE CARE AND HOSPICE AT UPENN Dr. Meryem ERCEYLAN Contents Palliative Care Palliative Care Team Hospice Hospice Service Home Care Palliative Care Team Physicians, Advanced practice providers (nurse practitioners and/or physician assistants), Social workers, Pharmacists, Spiritual care providers (chaplains). Palliative Care Team Palliative Care clinicians provide assistance in treating complex symptoms, offer extra emotional and psychosocial support, and the opportunity to discuss and establish patient’s treatment goals. Palliative Care Team How does this team work? This team does not have a ward, they work throughout the hospital. For example, the patient stay in the cardiovascular service. The cardiovascular specialist evaluates the general condition of the patient and fills out a form. As a result of the evaluation (scoring), it is determined that the patient is a palliative patient and the information is recorded in the system. The palliative care team schedules a meeting and visits the patient. The needs of the patient and family are determined and support is provided by the team member. Palliative Care Team the multidisciplinary, interprofessional team works collaboratively with patient regular care team to help patient and patient's family achieve their treatment goals. Hospis Service Manager Asistant manager NP (nurse practitioner) RN (registered nurse) LPN (licensed practical nurse) CNA (certified nursing assistant) MD (mediciane doctor) Chaplain (spiritual support) Social worker Reiki therapist Hospis Service Palliative care can be provided along with curative treatment and may begin at the time of diagnosis. Over time, if the doctor or the palliative care team believes ongoing treatment is no longer helping, if the doctor believes the person is likely to die within six months, patient could transition to hospice care. Usually, a hospice patient is expected to live 6 months or less. Hospice care focuses on the care, comfort, and quality of life of a person with a serious illness who is approaching the end of life.
